The authors report a case of spontaneous CSF rhinorrhoea secondary to an ethmoidal osteodural defect, diagnosed by digital cisternography and confirmed by the surgical operation. Excluding the aortic valve].","authors":"P Douek,&nbsp;D Revel,&nbsp;M Amiel","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>MRI has become the reference technique for the diagnosis and assessment of thoracic aortic aneurysms and subacute or chronic aortic dissections, and in the postoperative surveillance of the thoracic aorta. Several MRI techniques can now be used to investigate the thoracic aorta. The technique most widely used at the present time is Spin Echo imaging, which allows a multi-plane morphological approach to the thoracic aorta. It is often completed by a dynamic gradient echo sequence (cine-MRI) and, more recently, by ultra-rapid sequences (Turbo-Flash) following the injection of contrast agent. Phase-coding has also been proposed for the various intraluminal velocities. In the emergency situation, the examination of choice is less clearly defined due to the development of new techniques (transoesophageal ultrasonography, spiral computed tomography, MRI). With continued improvements in MR angiography, velocity mapping, myocardial tagging, imaging speed and display, it is anticipated that MRI will play an ever-increasing role in the morphological evaluation of many cardiovascular anomalies. The need for quantification in cardiology has led to the development of a variety of techniques to assess cardiac size and function. In attempting to answer these clinical questions, MRI provides major advantages several: extraordinary flexibility, powerful contrast mechanisms, sensitivity to flow and motion, and freedom from ionizing radiation, contrast agent and acoustic windows. Because of accurate initial results in quantifying cardiac chamber size, global and segmental function, it is likely that MRI methods will play an increasing role in the evaluation of cardiac structure and function. Recently, MRI has emerged as an important tool in the evaluation of great vessel disease, particularly in the evaluation of both congenital and acquired abnormalities of the aorta. Early evidence suggests that magnetic resonance may help in distinguishing constrictive pericarditis from restrictive cardiomyopathy (e.g., amyloid heart disease). Cardiac-MRI lends itself to assessment of intracardiac masses. Spin-echo imaging alone is often sufficient for diagnosis. The intradiscal vacuum phenomenon is frequently observed in degenerative disk disease and crystal-induced diskopathy. This has obvious significance to the radiologist, who, on observing a narrowed disk space or collapsed vertebral body, might otherwise consider infectious or neoplastic spondylitis, a likely possibility. The authors report a retrospective series of 14 cases of primary tuberculosis in a paediatric population (7 girls and 7 boys) with a mean age of 3.5 years (range: 4 months-16 years) observed over a 3-year period. After reviewing the current radiological features of patent primary tuberculosis, the authors emphasize the contribution of chest CT scan in latent forms with a normal chest x-ray and a difficult bacteriological diagnosis. Imaging remains an essential tool in early diagnosis, therapeutic management and active surveillance of this form.
