{"title":"[Analysis of various epidemiological parameters in patients with diabetes mellitus type I in the city of Rio de Janeiro, Brazil].","authors":"M de B Gomes, L M Vieira, E P Marques, E N Sarno, A R Chacra","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The authors analyzed epidemiological data of 157 type I diabetes patients living in the city of Rio de Janeiro, Brazil. The mean age at the time of study 16 +/- 8.1 years (arithmetic mean +/- standard deviation) and the age at the time of the diagnosis was 12.5 +/- 7.4 years. The diabetes history span was 3.1 +/- 4.3 years and the interval between appearance of symptoms and diagnosis was 30.7 +/- 30.4 days. Neither seasonal nor sex dependent differences were observed, but there was an inverse correlation between the average monthly temperature in Rio de Janeiro and the number of newly diagnosed cases. A first grade family history was positive in 15.9% of patients (11% of type I diabetes). The autoimmunity history was positive in 4.1% of patients (mainly thyroid disease). An infectious disease history was positive in 7% of patients (mainly mumps). {"title":"[Persistence of colonization time in patients infected by Salmonella].","authors":"E N Berezin, E da S Carvalho, C K Farhat, I M Mimica, L Mimica, T A Raphaelian","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Salmonella is an important etiological agent of hospital infection in children, reaching endemic levels in some Brazilian states during the seventies and the eighties. We have prospectively studied twenty five children between four and one hundred eighty days old acutely infected with salmonella non typhi to determine the duration of carrier status and its clinical repercussion. After the diagnosis, the children were submitted monthly to clinical examination, and cultures were collected from skin, oropharynx, urine, stools, genitals, nostrils and auditive conduct. During the follow-up, eighteen (72%) children still had positive culture at four weeks after the diagnosis, ten (40%) at sixteen weeks, four (16%) at twenty weeks, and one (4%) at twenty four weeks. In eleven children, we performed biotype and antibiotic susceptibility study of the bacteria recovered at the diagnosis and during the follow-up. In every child the biotype of the bacteria recovered at the diagnosis and during the follow-up was the same. These data indicate that there is a persistent excretion of salmonella that can last for 24 weeks. Such bacterial elimination may be a dissemination source either to hospital or to home contacts. {"title":"[Psychic changes in systemic lupus erythematosus: a multidisciplinary prospective study].","authors":"E C Miguel Filho, R M Pereira, G Busatto Filho, R G Shavitt, R Hirsch, L C de Sá, P C de Arruda","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Despite the high prevalence of psychic symptoms in lupus patients, there are few systematic studies in this area. Through a multidisciplinary approach, the authors developed a prospective study to characterize and correlate psychopathological aspects with clinical and laboratory data concerning neural manifestations of the disease. Out of 23 patients studied, 12 showed psychic alterations, which were interpreted as primary manifestations of the disease. All of them presented organic mental syndromes (DSM-III-R) in which cognitive symptoms were the most prominent, followed by affective, catatonic and hallucinatory features. The neurologic findings (seizure, migraine and muscular atrophy), as well as the ophthalmologic alterations (hemorrhage and soft exudates) were frequent and concomitant with the psychic features. The laboratory findings were: LE cells 50%; anti-Sm: 16%; anti-U1 RNP: 50%; anti-Ro/SS-A: 50%; anti-nDNA: 58%; decreased CH50 or fractions (C3, C4): 67%; anti-P: 18%; antigangliosides IgG: 67%; antigangliosides IgM: 78%. The cerebrospinal fluid analysis showed: increased cellularity: 18%; elevated protein: 36%; antigangliosides IgG: 67%; antigangliosides IgM: 33%; immunocomplexes: 36%. {"title":"[Ruptura of the thoracic esophagus due to closed abdominal trauma].","authors":"A S Cabral Júnior, G Furlanetto, P F Silva, J R Baratella, N F Safatle","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The authors present a rare case of closed abdominal trauma in a five year old girl resulting from a washtub fall on her causing three lacerations in the middle third of the esophagus, identified 48 hours after the trauma. The stitcher surgical treatment of the lacerations associated with gastrostomy and lengthy parenteral nutrition did not prevent the recurrence of the esophagus-pleural fistula, and an esophagectomy plus cervical esophagostomy was required. After a 10-month follow-up, the digestive passage was reconstructed by an esophagocoloplasty. At present, after 5 years of follow-up, the patient is cured. The authors discuss the causes of esophagus rupture in children and its etiopathogeny. Thirty-one reservoirs were implanted in 29 patients. Complications were observed in 19.3% of the cases imposing the removal of 12.9% of them. No reservoir have been lately removed because of infection or obstruction.
