{"title":"Elevated serum prostate specific antigen (PSA) related to asymptomatic prostatic inflammation.","authors":"L Hoekx, W Jeuris, E Van Marck, J J Wyndaele","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Prostatic subclinical inflammation is associated with elevated serum prostate specific antigen (PSA) level when there is contact with and disruption of the glandular epithelium of the prostate. This aggressiveness of the inflammation was graded on a 4-point scale proposed by Irani (1). Patients with an elevated serum PSA level and no malignancy in their transrectal prostate biopsy, have a lower risk of biopsy missed prostate carcinoma when there is an inflammation with a high aggressiveness score. The density itself of the inflammatory cells in the prostatic tissue was not associated with the serum PSA level. Although rarely seen in our countries it is well-known in areas where infections with Wuchereria bancrofti are endemic. Chyluria can also have non-parasitic causes of which only a few cases have been described. We briefly discuss etiologies and pathogenesis of chyluria and present a case of non-parasitic chyluria successfully treated by surgery. A review of the literature reveals only ten similar cases. History of a previous testicular surgery should not preclude the possibility of a torsion in that testicle. {"title":"A cystine lower pole renal calculus treated with holmium: YAG laser using a flexible 9,5F transurethral ureteroscope.","authors":"K Maes, H Goethuys, L Baert","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>We report a case of a 1.5 cm cystine staghorn calculus of the right lower pole in a 32 year female known cystinuric patient. With a 200 microns Holmium laser probe through a 9,5 F flexible ureteroscope the calculus was fragmented in small particles. An internal ureteral stent was inserted at the end of the procedure. All but one small residual fragments were evacuated spontaneously after removal of the stent. This case shows that flexible ureteroscopy combined with the Holmium laser is a safe and efficient procedure to treat medium size renal cystine calculi. {"title":"Urodynamic and electrophysiological testing in pediatric neurourology.","authors":"R J Opsomer, P Clapuyt, P De Groote, P J Van Cangh, F X Wese","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>In our institution, children with voiding disorders are submitted to a detailed evaluation comprising a careful clinical examination, laboratory tests, imaging (kidney ultrasonography and voiding cystourethrography) and urodynamic tests. Depending on the etiology of voiding dysfunction, children are subjected to a non invasive or an invasive urodynamic evaluation. Non invasive urodynamic testing comprises a free urinary flowmetry associated or not with an EMG recording of the pelvic floor. Invasive urodynamic testing consists of a pressure-flow study or a video-XR-urodynamic test. In rare instances, genitourinary electrophysiological investigations will be performed. {"title":"[Aponeurotic suspension of a vesico-urethral anastomosis after radical prostatectomy].","authors":"J L Jorion","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>In order to try to enhance postoperative continence rates after radical retropubic prostatectomy, a fascial sling suspension was performed at the time of the operation in 75 patients (group 2). Postoperative continence was compared to that of the 63 previous patients (group 1). Complete continence rate were 43 of 75 (57%) in group 2 and 17 of 63 (27%) in group 1 (p = 0.0003) at 1 month, 57 of 75 (76%) in group 2 and 36 of 63 (57%) in group 1 (p = 0.0149) at 3 months, 65 of 75 (87%) in group 2 and 46 of 63 (73%) in group 1 at 6 months, 69 of 75 (92%) in group 2 and 50 of 63 (79%) in group 1 at 9 months, and 72 of 75 (96%) in group 2 and 51 of 63 (81%) in group 1 at 12 months.
