The diagnosis can be made by CT-scan or NMR and can be clinically confirmed by aspiration of saliva. It is now generally accepted that the best treatment exists of resection of the sublingual gland by intra-oral approach and drainage of the cyst content to the oral cavity. In an experimental set-up we treated a patient with ethoxysclerol with excellent result. It reports on the variations in ATP content in 3 days old plaques of a well-defined surfaces with or without the use of 0.2% chlorhexidine mouth-wash twice daily. The results, expressed in bacterial equivalents, show a reduction of dental plaque metabolism after 3 days of chlorhexidine rinses to 19.5% of its value in the control tests without mouth-washes. To date only 8 cases have been reported in the literature. The case of a 12-year-old Hispanic-American boy with ameloblastic odontosarcoma of the mandible is presented. The patient had a tumor diagnosed previously as benign ameloblastic fibroma that underwent transformation to ameloblastic odontosarcoma. The patient has been followed for 19 years since the surgical removal of the tumor. Radiographic and clinical examination confirm that there is no evidence of recurrence. A porous hydroxylapatite block, Interpore 200, was used for contour augmentation at the labiomental fold. The weakening effect of the osteotomy, a foreign body-related chronic osteomyelitis, and repetitive angulation of the mandible during mastication play a role in the pathogenesis of this fracture. Treatment consisted of removal of the porous hydroxylapatite block, removal of the screws and plates, surgical debridement of the necrotic tissue, antibiotics, and immobilization with an arch bar on the buccal side and a lingual acrylic splint to prevent rotation of the upper border of the mandible. Computed Tomographic (CT)-examination showed a cystic mass in the lacrimal gland fossa of the right orbit. The lesion was removed in toto. Histological examination showed evidence of old and recent bleedings and a chronic inflammatory reaction with numerous foreign-body giant cells without epithelial lining. The diagnosis of an orbital hematic cyst was established. The pathologic and pathogenetic features of this lesion are discussed. The development of gingival overgrowth appeared in a range of 8 to 365 days, with a mean of 75 days, from the beginning of the immunosuppressive treatment with the drug. During the course of treatment, after a time period ranging from 30 to 365 days (mean 180 days), in 50% of the patients there was a total regression of the gingival pathology and in 36.4% a partial regression. It has not been possible to correlate the occurrence of gingival overgrowth with the presence of plaque or calculus and with the cumulative dosage of the drug. St.-Jan, Brugge, between 1/10/90 and 1/10/91 were evaluated for pre- and postoperative temporo-mandibular joint (TMJ) symptoms. Fewer TMJ symptoms were found postoperatively, than preoperatively (14.4% versus 19.7%). In the mandibular retrognathism group there were twice as much TMJ symptoms preoperatively in the low and normal mandibular angle patient group than in the high mandibular angle group (26% versus 13%). After surgery, there was a decrease of TMJ symptoms in the low and normal angle patient group (83% improvement). In the high angle absolute mandibular retrognathism group however, more new TMJ symptoms were seen postoperatively (21%). Bimaxillary surgical correction of a high angle absolute mandibular retrognathism case may provoke condylar resorption. This work analyzes results obtained from patients who are carriers of these premalignant lesions treated with 1,060 and 1,320 nm Nd-YAG and CO2 lasers.
