We tried to assess the association between cerebral microbleeds, hemorrhagic complication and functional outcome following intravenous thrombolysis in Egyptian acute ischemic stroke patients. We evaluated 66 acute ischemic stroke patients treated with intravenous thrombolysis for cerebral microbleeds using T2* weighted Magnetic Resonance Imaging Gradient echo. Distribution, number, and predictors of microbleeds were assessed. The effect of microbleeds presence and burden on development of hemorrhage after thrombolysis and 90 days functional outcome was evaluated. Out of 66 stroke patients treated with intravenous thrombolysis, 33 patients had microbleeds. Multivariate analysis shows that hypertension, diabetes mellitus, atrial fibrillation, smoking and leukoaraiosis were independently associated with microbleeds. Post-thrombolysis symptomatic intracerebral hemorrhage occurred in 12/66 (18.1%). Multivariate analysis shows that high burden microbleeds (≥ 10), leukoaraiosis, stroke severity, delayed thrombolysis were independently associated with intracerebral hemorrhage. Post-thrombolysis hemorrhage was statistically higher in microbleeds group (51.5%) than non-microbleeds group (9.1%) (p < 0.001). Parenchymal hemorrhage represents (58.8%) of hemorrhagic cases in microbleeds group in comparison to (33.3%) of non-microbleeds group (p = 0.62). Parenchymal hemorrhage represents (50%) of hemorrhagic cases with microbleeds < 10, while it represents (100%) of hemorrhagic cases with microbleeds ≥ 10. Favorable modified Rankin Scale (0–2) was more prevalent in non-microbleeds group (72.7%) than microbleeds group (45.5%) at 90 days (p = 0.024). Favorable outcome at discharge and at 90 days was statistically more prevalent in patients with microbleeds < 10 (p = 0.004). High burden cerebral microbleeds should be considered a risk for parenchymal hemorrhage following intravenous thrombolysis. These compounds have shown effectiveness against conditions such as obesity, diabetes, and high cholesterol, which are known risk factors for cardiovascular diseases, including stroke. Stroke is currently the second leading cause of death globally, and cerebral small vessel disease (CSVD) accounts for 20% of all strokes, and it often causes cognitive impairment and gait abnormalities in older adults. Anthocyanins and their metabolites can cross the blood–brain barrier and affect signaling pathways, gene expression, and protein function at the molecular level. In addition to their ability to enhance vascular flow, anthocyanins can also help mitigate the risk factors associated with CSVD by counteracting oxidative stress in the body. Despite its impact on prognosis and quality of life, PSP remains underdiagnosed and undertreated, with limited data on its prevalence and risk factors. The purpose of this study is to assess the prevalence and determinants of post-stroke psychosis (PSP), as well as to compare PSP and non-PSP patients in terms of improvement and daily living outcomes. This nested case–control study included 2,624 acute stroke patients from a university hospital between May 2017 and April 2022. Patients who developed PSP within 6 months post-stroke were identified as cases (n = 108), and 119 patients without PSP were randomly selected as controls. Comprehensive assessments included clinical, laboratory, and imaging evaluations at baseline. After 6 months, follow-up evaluations were conducted, including neurological examinations, psychiatric assessments, and stroke severity assessments using the Barthel index (BI). The psychiatric assessments included the Hamilton Depression Rating Scale for depression and the Mini-Mental State Examination for cognitive status. The prevalence of PSP was 5.4%. risk factors significantly associated with PSP included older age, male patients, lower education level (≤ 5 years), hemiplegia, sphincter affection, cortical lesion, brain atrophy, small vessel disease, ischemic stroke, post-stroke dementia, and seizures. Non-specified psychosis and delusional disorder were the most common psychosis subtypes. There was a significantly higher percentage of excellent patients in the non-PSP group compared to those who had PSP regarding the observed improvement in the patient's condition. Also, there is a higher percentage of deteriorated patients in the PSP group (46.6%) compared to another group (18.9%) regarding BI. PSP is a prevalent post-stroke complication associated with distinct risk factors and poor functional outcomes. This case–control comparative study was conducted on 180 children aged from 5–18 years diagnosed with epilepsy according to the definition of the International League Against Epilepsy on ASMs for more than 3 months and were classified into 90 epileptic children on ASM monotherapy and 90 epileptic children on ASM polytherapy, in addition to 90 healthy children age- and sex-matched who served as controls. After obtaining basic data, laboratory investigations were performed, including serum calcium, phosphorus, alkaline phosphatase, parathyroid hormone, vitamin D, and serum sclerostin. When we compared epileptic patients to the control group, there was a statistically significant low level of vitamin D, calcium, and phosphorus and a high level of sclerostin among both epileptic groups with mono or polytherapy. Sclerostin has a statistically significant negative correlation with vitamin D, alkaline phosphatase and parathyroid hormone. Additionally, it has a negative correlation with serum phosphorus, but without a significant correlation. On the other hand, sclerostin has a statistically positive correlation with age and serum calcium, but without a significant correlation. Multiple linear regression analyses were conducted to predict the contributing factors of sclerostin. Only duration of treatment and BMI were significant predictors of high levels of sclerostin. In contrast, the other factors failed to show any significant contribution. This study aimed to predict neurodegeneration in patients with epilepsy (PwE) through assessment of the retinal nerve fiber layer (RNFL), ganglion cell complex (GCC) thickness, and central macular thickness (CMT) using optical coherence tomography (OCT). A cross-sectional study was done on 60 patients with idiopathic epilepsy and 30 healthy volunteers. They were subjected to a full neurological examination, ophthalmological assessment, and OCT for assessment of retinal layers, and cognitive examination using Addenbrooke’s scale. PwE had lower cognitive scores, including memory (13.97 ± 2.52), attention (15.95 ± 1.85), language (24.08 ± 1.71), and fluency (6.10 ± 2.05), compared to controls (20.53 ± 3.5), (17.13 ± 1.53), (24.83 ± 0.99), and (8.87 ± 2.39), respectively. There was a significant thinning in average RNFL thickness (84.27 ± 7.66), inferior RNFL thickness (99.33 ± 10.19), average GCC thickness (83.17 ± 9.76), and superior GCC thickness (84.83 ± 7.27) in the epilepsy group compared to controls (105.70 ± 8.73), (104.93 ± 9.75), (101.50 ± 4.84), and (100.53 ± 4.09), respectively. PwE had significantly higher focal macular volume loss (1.17 ± 1.22) versus (0.11 ± 0.21) and a higher insignificant global macular volume loss (1.88 ± 2.32) versus (1.37 ± 0.65) in controls, respectively. Superior GCC thickness was significantly lower in the uncontrolled patients (82.53 ± 6.23) compared to the controlled patients (87.13 ± 7.60), while CMT was significantly lower in the polytherapy group compared to the monotherapy group. There was a significant positive correlation between the age of epilepsy onset and verbal fluency (r = 0.382, p = 0.003). Epilepsy duration had significant negative correlations with memory (r = − 0.364, p = 0.004), inferior RNFL thickness (r = − 0.324, p = 0.012), perifoveal thickness (r = − 0.353, p = 0.006), and inferior (perifoveal) thickness (r = − 0.365, p = 0.004). PwE receiving anti-seizure medications (ASMs) have reduced GCC, RNFL, and CMT and lower cognitive functions compared to controls. Elbohoty","doi":"10.1186/s41983-024-00858-0","DOIUrl":"https://doi.org/10.1186/s41983-024-00858-0","url":null,"abstract":"The present study seeks to deepen understanding of the negative impacts of infertility, with a spotlight on the experiences of Egyptian women who faced prolonged periods of infertility, and emphasizing the role that faliures of in vitro fertilization/intracytoplasmic sperm injection (IVF/ICSI) cycles may play in exacerbating their psychiatric disorders and impairing their sexual functions. Embarking on a cross-sectional approach, this study was operationalized within the outpatient sectors of the Obstetrics and Gynecology Departments, synchronized with Psychiatry Departments, at Tanta University Hospitals from September 1, 2018, to December 31, 2022. The targeted population encompassed women experiencing prolonged infertility (≥ 5 years), categorically divided into two groups for meticulous analysis: Group I (with a history of IVF/ICSI failure) and Group II (without any IVF/ICSI attempts). Diagnostic tools, including the Arabic versions of the Hamilton Depression Scale (HAM-D), Hamilton Anxiety Scale (HAM-A), and the Female Sexual Function Index (FSFI), were deployed to scrutinize the mental health and sexual function outcomes of the participants. A total of 236 women participated in this study (126 in Group I and 110 in Group II), with primary infertility prevalent in 73.31% of cases, averaging a duration of 9.28 ± 3.40 years. Our findings indicated a substantial presence of depression (58.47%), anxiety disorders (35.17%), and sexual dysfunctions (43.64%). Notably, significant differences (p < 0.0001) emerged between the groups on the HAM-D, HAM-A, and FSFI scales, with endometriosis showcasing the worst effects. The findings conclusively pinpoint a pronounced link between prolonged infertility and exacerbated mental health conditions, alongside impaired sexual functions, thereby negatively affecting the quality of life of the affected women. The objective of the present study was to determine the demographic and clinical characteristics of epilepsy in west Algerian population. We enrolled 226 patients with epilepsy and collected data from medical records. We analyzed socio-demographic data, personal and family history, age at onset, seizure types, neurological and psychological findings, etiologies, syndromic classification and comorbidities. In our cohort, the mean age was 27.26 ± 17.57 years and there was no gender predominance. The mean age of seizure onset was 17.11 ± 16.27 years. Genetic epilepsy was the most observed (45.13%). Focal seizures (69.02%) were more frequent than generalized seizures which were mostly represented by absence and tonic–clonic seizures. We reported 51.45% of patients with comorbidities and 16.65% with a history of traumatic brain injury. This is the first report describing the clinical profile and the socio-demographic features in a cohort of patients with epilepsy in the western Algeria. There has been growing evidence that children with DMD have been at higher risk of developing symptoms of neurodevelopmental disorders. We aimed at defining the cognitive difficulties and their categories in patients with DMD, compared to healthy controls, and assessing their relation with the functional severity. This work was a multi-center, observational, case–control study conducted on DMD patients. Age and sex-matched healthy subjects with no neurologic, psychiatric, or medical comorbidities were included as normal controls. There was a statistically significant difference in cognitive patterns between the studied groups. We have observed a significant relationship between cognitive difficulties and functional severity assessment in our patients. There was a statistically significant difference between the studied cases regarding basic characteristics and correlation between cognitive functions and demographic data. The decline in cognitive functions in DMD patients compared to healthy controls was established. Education was the most affected domain in patients, with more speech delay and dropping out of school.
