Aim of the work: Comparing the role of Nigella sativa ( NS) versus calcium and vitamin D (Ca+Vit D) in the protection from Glucocorticoid-induced osteoporosis in a rat model. Material and Methods: Fifty-one male albino rats weighing 120-150 gm were divided into 4 groups: Group I (Control), Group II (dexamethasone group), Group III (dexamethasone plus Ca+Vit D), and Group IV (dexamethasone plus NS oil). Results: Rats of group II revealed erosion cavities in compact bone with eroded surface and widened Haversian canal and osteocyte lacunae. The cancellous bone showed thin disconnected bone trabeculae containing empty lacunae with wide bone marrow spaces. Areas of faintly stained bone matrix and minor fractures could be detected. These results were confirmed by a significant decrease in the thickness and number of lacunae containing osteocytes in cortical and cancellous bone. Also, there was an apparent decrease in osteopontin and an increase in caspase-3immune reaction. Moreover, a significant increase in alkaline phosphatase and IL6 was detected. Groups III and IV showed improvement in cortical and trabecular bone structure. The expression of osteopontin, caspase-3, alkaline phosphatase, and IL6 was also improved. Conclusion: This study demonstrated that supplementation of NS oil was better than calcium and vitamin D in osteoporotic lesions of rat femur due to its effect on bone remodeling. It acts as a calcium source and has antioxidant effects. In this study, we evaluate contemporary clinical practice and recent trends and outcomes in managing anterior circulation CIAs. Methods: This study is a prospective observational study that included 40 patients. All patients were evaluated by multidisciplinary team (MDT) consisting of neuro-endovascular radiologists, vascular neurosurgeons, and neurologists for assessment of the therapeutic options. Results: Twenty cases were managed surgically, while 18 were treated by various endovascular techniques. Surgical and endovascular modalities were used as a hybrid approach to treat 2 cases. Recurrence or residual of treated aneurysms was seen only in the endovascular cohort (Four cases out of the 18 cases treated by endovascular techniques). Four documented intervention related mortalities occurred during the first 6 months. Conclusion: CIAs managed surgically showed less recurrence rate than those treated by different endovascular techniques. CIAs treated by endovascular modalities showed much lower length of hospital stay and better modified Rankin scale after 6 months. Appropriate preoperative evaluation to discriminate between benign and malignant adnexal masses helps to decide the best management and improves survival rate. The most accurate diagnostic method is the contrast MRI to differentiate benign from malignant adnexal masses. Contrast MRI is contraindicated in patients with chronic kidney diseases as well as it may cause allergic reactions. In addition, contrast studies need more time and are expensive. So, Non-contrast MRI may be a diagnostic alternative method when the administration of intravenous contrast medium is not possible. Aim of the Work: Detection of the sensitivity and specificity of abbreviated non-contrast MRI compared with contrast MRI study in evaluation of adnexal masses by using non-contrast and O-RADS scoring systems with detection of inter-reader agreement. Patient and Methods: This study included 30 patients had adnexal masses with age ranging from 18 to 78 years, referred to MRI unit in Ain Shams University Hospitals from February 2022 to January 2023. Comparison between Non-Contrast MRI and ORADs scoring system was done to interpret sensitivity and specificity of non-contrast MRI study. Comparison between readers was done to interpret inter-reader agreement Results: Non-Contrast MRI study compared with Contrast MRI study showed high specificity (92.3%), high sensitivity (100%) and high accuracy (95%). There was no statistically significant difference found between contrast & non-contrast MRI with high inter-reader agreement. Conclusion: Non-contrast MRI has high accuracy and high inter-reader agreement for characterization of adnexal masses. Aim of the work : To determine the value of radiological assessment of vascular complications in COVID-19 infections and its relation to D-dimer levels as important tools for early diagnosis and proper management of these patients. Patients & Methods: The study included 100 patients diagnosed as COVID-19 positive with embolic manifestations depending on their symptoms, signs, and laboratory assessment, which included D-dimer levels. They underwent radiological assessment of their vascular complications, including CT, MRI, and CT angiography. Results: Among 100 COVID-positive patients with thromboembolic symptoms, 55 had evidence of pulmonary embolism, 13 had strokes, 4 had mesenteric arterial occlusion and 28 patients had no radiological evidence of vascular manifestations. D-dimer levels were increased (0.50 mcg/mL or greater) in 70 percent of positive COVID patients, including 89% of those with pulmonary embolism, 84% of those with stroke, 75% of mesenteric thrombosis cases, and in 7 patients (25% of those with symptoms suggestive of vascular thrombosis without radiological imaging of thromboembolic manifestations), with a sensitivity of 88.6%, specificity of 75%, positive predictive value of 90.9%, and negative predictive value of 70%. The integrity of this CI can be assessed intraoperatively and postoperatively through objective measures which include telemetric measurement of electrode impedance and electrically evoked compound action potential. Aim of the work: To compare electrode impedance values and electrically evoked compound action potential (ECAP) thresholds intraoperatively and postoperatively to asses whether significant changes take place overtime or not. Patient and Methods This study was carried on 25 subjects with age ranging from (3-60) yrs. 21 of the subjects were implanted unilaterally with a MED-EL devices and 4 implanted with cochlear devices. Impedance values and ECAP thresholds were monitored at the time of surgery then postoperatively at 1 st day tunning, 1 month, 2 months and 6 th months after 1 st day tunning. Results: Intraoperative impedance value was the lowest among all readings measured in all electrodes. The highest value was that measured at the 1 st day tunning after surgery, after which impedance value continued to decrease significantly when compared to the 1 st day tunning. ECAP thresholds showed the highest value intraoperatively after which the ECAP continued to decrease. However, the difference between readings was statistically nonsignificant.
