This posture involves moderated hemodynamic effects. A discrete right ventricle impairment characterised by a decreased compliance was observed and may be due to a moderate compression by the sternum and was improved by fluid challenge. Augmentation of the delta-down component of the systolic blood pressure variation in ventilated patient is a sensitive indicator of intra operative hypovolemia. According to results, cardiac filling pressure is more affected in the knee chest position than in prone position. These pathologies may have an acquired or congenital origin. In the last eventuality, they are frequently associated with other malformative disorders involving the vertebral column, the thorax, the kidneys or the heart. The clinical picture is various and many systems can be affected. Therefore, preoperative assessment of patients implies a multifactorial evaluation. {"title":"[Techniques for intubation when head and neck cannot be moved].","authors":"V Crinquette, P Ravussin, O Moeschler","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The inability to extend the head may be due to a blocked cervical spine or to any cervical instability imposing to maintain the head straight. Exposure of the glottis during intubation may be difficult and can be ameliorated by a stable general anesthesia, some pressure on the larynx and by charging the epiglottis. When mouth aperture is superior to 40 mm, a lighted stylet, a laryngoscope with a prism, a fiberoptic laryngoscope (Bullard) or the PCV laryngoscope represent a possible alternative to the Mac Intosh laryngoscope. If mouth aperture is superior to 20 mm but inferior to 40 mm, a ENT or PCV laryngoscope or a fiberoptic intubation are recommended. One should remember that the intubation is easier if the diameter of the ET tube is small. If the mouth aperture is inferior to 20 mm, nasal intubation (if intubation is indicated) is mandatory using fiberoptic intubation or a retrograde technique or even nasal blind intubation. The study includes 29 patients with clinical observations during their hospitalisation period and a follow up for one year after the emergence from the state of coma. The psychoanalytic methods were used, taking in consideration the emotional and personal history of each patient. We attempted to demonstrate the persistence of the unconscious psychic life during coma and the early period after awakening from the coma. In addition, we felt the existence of a sort of direct confrontation with the unconscious psychic life in some patients emerging from a comatose state.
