{"title":"Hyperbaric Oxygen and Outcomes Following the Brain Injury: A Systematic Review.","authors":"Javed Iqbal, Abdullah Naeem, Kainat Jahangir, Yumna Ali, Yusra Mashkoor, Ahmer Ashraf, Dalia Mehmood, Maria Mehmood, Lucke-Wold Brandon","doi":"10.47363/jnrrr/2023(5)178","DOIUrl":"10.47363/jnrrr/2023(5)178","url":null,"abstract":"<p><strong>Introduction: </strong>Hyperbaric oxygen therapy (HBO2) aims to address ischemia resulting from brain injury by subjecting patients to an atmosphere that dramatically raises the concentration of inspired oxygen (100% O2 at greater than 1 ATA). This results in elevated levels of oxygen in the plasma, which in turn boosts the delivery of oxygen for diffusion to the brain tissue.</p><p><strong>Objective: </strong>To study the efficacy of hyperbaric oxygen (HBO)-based modalities in brain injury.</p><p><strong>Method: </strong>Preferred reporting items for systematic reviews protocol was applied to perform literature search regarding this analytical review.</p><p><strong>Results: </strong>In our study, fifteen studies are included in this review, involving 1067 people. The mean age group of patients enrolled was 57.0±11.6 and the mean NIHSS score was 10.5±8.7, of which 21 participants had moderate to severe neurological impairment. The total number of HBO treatments was 8 to 70 times (28.3±17.9), at the end of the 6-month follow-up period. mRS (modified Rankin scale) ≤3 was found in 25 cases, of which 12 patients with high-grade aSAH recovered. Poor prognosis was prevalent in patients who experienced delayed cerebral ischemia, this was true for 22.7% of patients in this study. In 3 studies conducted by Rockswold, ICP (mm Hg) was significantly lower in the HBO2 group after the treatment than pretreatment. (p<0.05). 4 studies showed an improvement in GCS score after HBO2 therapy.One trial (Imai 2006) reported that three patients in the HBO group died due to pneumonia (two) and heart failure (one) and one patient died in the control group due to heart failure. Methods: In this study, descriptive statistics using summary measures were used to describe the socio-demographic profiles of neurology patients who sought consultation through telemedicine in the Jose R. Reyes Memorial Medical Center, Department of Neurology from September 2021 to October 2021. T-tests and analysis of variance (ANOVA) were used to determine statistically significant differences between the type of visit and amongst the top four diagnosis seen, respectively. Results: The respondents are composed of 54% females and 46% males with ages ranging from 19 to 83 years old. One-third of the respondents are new patients, and the rest are follow-up visits. However, South Asian countries have lower mortality levels compared with other world countries. COVID-19 has revealed the vulnerabilities of the health system as a whole. In addition, the high burden of non-communicable diseases in South Asia multiplies the complexities in combating present and future health crises. The advantage offered by the younger population demographics in South Asia may not be sustained with the rising burden of noncommunicable diseases and lack of priority setting for improving health systems. Thus the COVID-19 pandemic has provided a window for introspection, scaling up preparedness for future pandemics, and improving the health of the population overall. The COVID pandemic arrived in South Asia at a much later stage compared with other countries. As such, the South Asian countries may have been able to learn from countries that had early peaks, and therefore achieved better preparation in terms of the public health response. For example, this could have resulted in a lower viral load due to more people wearing masks. South Asian countries lift lockdowns despite rising case numbers. Healthcare facilities and hospitals are stretched due to the increase in the number of cases. It has been reported that testing levels in Pakistan and Bangladesh have fallen dramatically. Social distance is much prevalent in sangha members of Buddhism and caste system in Hindu and its associated communities. That help much to such communities during COVID-19 pandemic. Further requires strong leadership and great political will to allocate substantial resources to prepare for future pandemics. The region needs to scale up the existing social ‘safety nets’ rapidly, such as cash transfers for food. South Asian countries have also resorted to the use of online portals, social media, working from home, online learning, direct benefit transfers, delivery of health services through ‘virtual doctors’, deploying facial recognition, and use of thermal scanners for identification of infected people. Mild cognitive impairment (MCI), which still lacks a specific therapy, is a clinical syndrome associated with increased risk of dementia. This study aims to assess the effects of deep TMS (dTMS) on a group of 10 patients diagnosed with amnesic MCI. Methods: We compared the effects of TMS COG treatment (dTMS delivered with H7 helmet for ten daily sessions together with cognitive training of memory and attention), with those of COG treatment (cognitive training alone) of the same duration. Results: Neuropsychological evaluation at baseline, after TMS COG treatment, after COG treatment and at six months follow up, compared with ANOVA, revealed a significant group-by-time interaction (𝑝 = 0.05), favoring the TMS COG treatment for memory tests. The improvement was kept after six months. Other neuropsychological tests were not significantly affected by treatment. GBS symptoms can range from a mild episode of weakness to devastating paralysis and respiratory failure. The exact cause of GBS is unknown, but it is often thought to be due to a preceding viral infection or rarely due to vaccination. To date, there has only been one reported case of Guillain Barre Syndrome associated with the administration of Ad26.COV2.S vaccine (Janssen/Johnson & Johnson COVID-19 vaccine) [3]. Here, we describe the case of a 59-year-old woman who received the Johnson & Johnson COVID-19 vaccine and subsequently developed symptoms consistent with GBS. Unfortunately, due to failure to obtain lumbar puncture (LP) and electromyography (EMG), it is only possible to diagnose Guillain Barre syndrome with Level 3 diagnostic certainty using the Brighton criteria [6]. Public health measures like quarantine or national lockdown were adopted, with negative psychological and clinical effects on patients with chronic diseases. To investigate the impact of lockdown on patients with multiple sclerosis (MS), we developed a 36 items self-report questionnaire. Methods: Questionnaires were emailed to 120 patients with MS and 100 with chronic migraine (CM) as a control group, matched for age, sex, and education. The questionnaire was divided into five domains, evaluating concerns about 1) MS and therapy during COVID-19 pandemic, 2) personal and family caregiving, 3) working activities, 4) general and disease-related emotions during the lockdown, 5) future expectations concerning health status, social life, and working activity. Results: patients with MS had higher scores than those with CM in domains 4 and 5, investigating respectively general and disease-related emotions and future expectations (p= 0.05 and 0.02 respectively). About half of the patients with MS expressed some concern about the need to continue their therapy during the pandemic compared to people with CM (p= 0.0002). It may cause hypopituitarism immediately or several years later, depending on the degree of tissue destruction. Sheehan’s syndrome though rare is still one of the commonest causes of hypopituitarism in developing countries like ours. The presence of an intercurrent infection and administration of thyroxine exacerbated her corticosteroid insufficiency. Intracranial hypertension (IH) manifested as bilateral optic disc swelling with reduced visual acuity, bilateral sixth nerve palsies, and impaired consciousness. Intracranial hypertension (IH) has been associated with hypocortisolism caused by either primary adrenocortical insufficiency or corticosteroid withdrawal. The author describes a case of a young lady with IH with acute SS who presented on 3rd day postpartum after lower uterine cesarean section with acute severe symptomatic hyponatremia which was complicated by postpartum hemorrhage.
