A level above 70% was the declared goal in early resuscitation of septic shock for over a decade. Recent evidence leads to doubts and levels higher than 80% may represent harmful conditions. This study ´s aim was to identify favourable ScvO2 levels in treatment of septic shock. \u0000Methods: Electronic data of patients suffering from septic shock who have been admitted to the surgical intensive care unit of a university hospital were analysed surveying a period of six years with focus on the association of ScvO2 levels with in-hospital mortality. \u0000Results: Data from 238 individuals were included. No difference was found comparing initially measured values of ScvO2 of survivors to non-survivors. Patients whose levels of ScvO2 never exceeded 70% (n=28) had a higher mortality rate (73.2% vs. 54.3%, p<0.05). On day three patients with values above 75% (n=32) had higher mortality rates (59.4% vs. 38.5%, p<0.05). A mortality rate of 100% was detected if ScvO2 levels exceeded 84% (n=6). \u0000Conclusions: ScvO2 develops from a therapy guiding parameter to a prognostic marker. Reaching levels of at least 70% within the first 72 h of disease is favourable in regard to prognosis. Exceeding 75% after day two is associated with higher mortality. These findings require further confirmation. At this point it can be assumed that a varying, time-dependent ScvO2 approach might be required for clinical decision-making.

Anambra State and Nigeria as a country have experienced a continuous lack of access and healthcare underutilization of biomedical health services for malaria treatment and has suffered immensely due to poverty. Malaria more heavily affects people who are in poverty, as they have no means of paying for healthcare. It is a common infectious disease that is widespread in most tropical regions of the world, especially Asia and Sub-Saharan African countries where malaria infects about 515 million persons a year and is responsible for 1.5 to 2 million deaths per annum among children under age 5. Malaria has been linked to poverty, since it is known to affect people and regions in poverty that cannot afford malaria treatment (WHO, 2015).

We present a case of a 19 year old male with disseminated methicillin-resistant Staphylococcus aureus (MRSA) Infection who was treated with IV vancomycin and developed fever and neutropenia. After discontinuation of IV vancomycin and substitution with clindamycin, both fever and neutropenia resolved within 72 h. Diagnosis of vancomycin-induced neutropenia and fever was considered as there was no alternative cause of this. In addition, quick resolution of fever and neutropenia upon removal of causative drug further supported the diagnosis. Since vancomycin is widely used in clinical practice, this adverse effect should be monitored carefully. Many studies report immune mediated mechanism to be the cause of this adverse effect. However, further studies need to be done in this regard to establish the exact mechanism.

Its clinical implications are still to be determined. The aim of this study was to analyze the impact of inhaled imipenem on forced expired volume in 1 sec (FEV1) and number of exacerbations in adult cystic fibrosis patients. Patients colonized by A. xylosoxidans with more than two intravenous antibiotic cycles and/or ≥ 5% FEV1 impairment during the last year were included. \u0000We included six patients with a mean age of 24.8 years. At the beginning of treatment FEV1 was 1.18 l (52%) with a mean of 3.4 exacerbations in the previous 12 months and 2.24 (p=0.04) the following 12 months. Despite the existence of antifungal treatment IA mortality rate remains extremely high and may reach up to 80%. Previous studies have suggested important role of platelets in antifungal host defence. In vitro data show that platelets are able to attenuate germination, hyphal elongation and viability of Aspergillus fumigatus. Interaction of platelets with Aspergillus fumigatus induces differential expressions of fungal genes associated with stress response regulation, cellular transport and metabolism. \u0000A new class of antifungals, echinocandins (caspofungin, micafungin and anidulafungin), have entered the market. Anidulafungin displays strong antifungal activity against Candida and Aspergillus species and has very few side effects due to its specific inhibiting effect on the fungal cell wall synthesis. Anidulafungin is currently licensed for the treatment of adult patients with invasive candidiasis. Clinical significance of anidulafungin for IA treatment has to be further evaluated. \u0000Recently published studies have shown that efficiency of antifungal substances can be enhanced when combined with platelets. In this review we discuss the literature on the potential combined effect of platelets and anidulafungin against Aspergillus fumigatus infections.

The situation has become worrying since the 1970’s and continues to be more complicated. The objective of this work was to realize a historical overview of the classification, evolution, and dispersion of Leishmania parasites and Sandflies in Morocco. The status of the different leishmaniasis could be illustrated in three epidemiological entities: zoonotic cutaneous leishmaniasis (ZCL) due to L. major transmitted by P. (Phlebotomus) papatasi, Anthroponotic cutaneous leishmaniasis (ACL) due to L. tropica transmitted by P. sergenti and cutaneous and visceral leishmaniasis (VL) due to L. infantum transmitted by P. ariasi, P. perniciosus, and potentially by P. longicuspis. Actually, VL occurred mainly in Northern Morocco. The ACL is widely distributed in the central and western Morocco. The ZCL occurred mainly in the south and south-east of the Atlas Mountains.

Yeasts are always present in the vagina in small numbers, and symptoms only appear with overgrowth. The vaginal swab specimens were collected from the diabetic women with signs and symptoms of vaginitis. The isolated Candida species were identified by the morphological and cultural characteristics. The isolated and identified Candida strains were included C. albicans, C. dubliniensis, C. krusei, and C. tropicalis from diabetic women with signs and symptoms of vaginitis. The isolates were subjected to determine the in vitro anticandidal activity against antifungal drugs. All Candida strains isolated from diabetic women with vaginitis were 100% sensitive to Amphotericin-B and C. krusei strains showed 100% sensitivity to ketaconazole drug. Overall 32% of Candida strains showed multidrug resistance against the antifungal drugs we used.

It can cause three types of infections: pulmonary cryptococcosis, cryptococcal meningitis and cutaneous cryptococcosis. Cutaneous cryptococcosis may represent the dissemination of a systemic infection (especially from nervous or pulmonary primary site of infection) or may be the only localization, due to a direct inoculation into the skin, because of a traumatic injury. Primary cutaneous cryptococcosis (PCC) is rare and mainly affects elderly patients, from rural areas, with history of cutaneous injuries and activities predisposing wounds or exposure to bird droppings. Immunosuppression may be a predisponing factor. The most utilized treatment is fluconazole, but often, especially for ulcers or deep wounds; surgery is required for complete tissues repair. Herein, we present a case of PCC in an immunosuppressed patient, with destroying ulcers involving deep tissues, completely resolved after fluconazole treatment, without surgical intervention.
