Violation of the knee joint utilizing an intra-articular start point represents a risk for septic arthritis of the knee in the post-operative period. Previous retrospective studies of open tibia and femur fractures demonstrated that post-operative knee sepsis is rare, occurs in the chronic phase after injury, and due to extent of soft tissue injury rather than immune compromise. Here, we review current literature on post-operative knee sepsis, and present a case of acute septic arthritis of the knee following supra-patellar nailing of an open tibia fracture in a patient on chronic immune suppression. In this unique case, co-morbid patient factors likely led to this manifestation of a rare complication.","PeriodicalId":73665,"journal":{"name":"Journal of clinical cases & reports","volume":"17 2 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2018-10-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"82936343","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Various removal methods have been described. Special tools and techniques are required to remove rings and bands from fingers in order to prevent finger ischemia and necrosis. Non-cutting removal techniques are preferable, but when unsuccessful, cutting or destructive techniques may be necessary. Techniques are material dependent and directly related to hardness and malleability. We present a case of digital entrapment in the lug hole of a tire, extricated utilizing a Dremel tool and provide a summary of other ring removal techniques.","PeriodicalId":73665,"journal":{"name":"Journal of clinical cases & reports","volume":"86 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2018-10-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"80810691","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Methods: The patient’s adherence to (Cal/BD) cutaneous foam was objectively monitored by a chip in the medication dispenser and the patient was subsequently visited at his home to observe the dog’s licking process and obtain clinical photos. Conclusion: Topical antipsoriatic drugs containing corticosteroid/calcipotriol preparations are first-line treatment for mild-to-moderate psoriasis, but adherence rates to topical antipsoriatic drugs are low and a barrier for full efficacy of the treatment. Physicians should be aware that a dog licking off psoriasis scales can lead to nonadherence to topical treatment as well as expose the dog owners and their dogs to a risk of adverse events.","PeriodicalId":73665,"journal":{"name":"Journal of clinical cases & reports","volume":"256 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2018-10-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"79518869","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Portraying alarming results on knowledge and attitudes on breast cancer among healthcare professionals from and university hospital. This review aims to explore the current performance of healthcare professionals towards breast cancer awareness in a potentially developed era. Materials and Methods: This Cross-Sectional pilot tested study was conducted at a university hospital targeting all healthcare professionals between June 2017 - December 2017. Designed structured questionnaires were directly distributed to cover demographic data, high-risk factors, knowledge and attitudes towards breast cancer. The target population was 600. Results: The total respondents were 532(88%). Age ranged between 19-59 years with the mean age of 28 years. These where 200(37%) final year medical students, 118(22%) interns, 60(11%) residents 22(4%), specialist 30(5%), 28(5%) consultants, and 74(13%) nurses. High risk group accounted for 120(23%) of all participants. The highest scores were obtained from the younger group of staff, the internsstudents (91%, 97%) as compared to the specialistconsultant categories (75%, 82%). Nurses unexpectedly had shown lower scores in knowledge (71%) which may suggest that the practical education in nursing practice supersedes knowledge in special fields. High risk group did not show significant difference in the overall knowledge scores (84%) yet, they scored higher in attitudes towards BCAP (94%). Conclusion: The necessity for (BCAP) cannot be overstated. The impressive and remarkable improvement of breast cancer awareness coupled with the availability and readiness of human resources assist in the planning, implementing and setting guidelines and strategies for breast cancer early detection programs. It can be declared that our community is equipped with knowledgeable medical staff, who can perform adequately run Breast Cancer Awareness Programs.","PeriodicalId":73665,"journal":{"name":"Journal of clinical cases & reports","volume":"63 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2018-07-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"82082190","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Neurologic adverse reactions are frequently observed after ifosfamide, which metabolites may cause acute ifosfamide-induced encephalopathy (IIE). There is no standard management in this clinical setting. Herein we report a case of 28 year old male with a B-cell lymphoma unclassifiable, with features intermediate between diffuse large B-cell lymphoma and classical Hodgkin lymphoma. The patient developed a severe IIE with catatonia, as well as fecal and urinary incontinence during combined systemic ifosfamide-based chemotherapy. The infusion was interrupted; hydration and forced diuresis were implemented. Drugs with known sedative effect on the central nervous system were eliminated. Nevertheless, no improvement was observed. At this point a decision was made to administer methylene blue (MB). Two doses were infused at intervals of 6 hours with fast improvement and elimination of IIE symptoms on the second day after MB infusion. Noteworthy, no toxicity attributable to MB was observed. Described treatment was based on previous case reports and small retrospective series and obtained results fully confirmed the conclusions of these publications.","PeriodicalId":73665,"journal":{"name":"Journal of clinical cases & reports","volume":"23 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2018-07-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"74787451","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Methods: All adult traumatic brain injury patients admitted to the SICU between December 2013 and March 2014 were included. The third edition of the BTF guidelines was taken as a standard to assess compliance. A computerized ICU database established in 2011 allowed access to the patient records and all data was cross checked with the hospital patient record system. Results: A total of 28 patients were admitted to the SICU during the audit period. 15 variables were checked from the database and corroborated from the hospital records. The variables were collected at 24 hours after admission. All patients were with low Glasgow coma scale (GCS) and admitted to the ICU for neuro monitoring. Of the 15 variables recommended the compliance rate was 60.7%. The ICU mortality for this group of TBI patients during this period was 25%. Conclusions: Our audit shows a higher compliance rate and an acceptable mortality rate. Further studies will be required to see the mortality benefit between groups as well as development of local protocols to improve adherence to the guidelines suggested.","PeriodicalId":73665,"journal":{"name":"Journal of clinical cases & reports","volume":"9 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2018-07-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"87317489","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
