Neonatal gastric perforation (NGP) was first described back in early 19th century. Since then the reported numbers has kept increasing, but the mortality has been reducing due to improvement in the neonatal intensive care. The increase in number may be due to the survival of low birth weight and premature newborns. The incidence of NGP is 1 in 5000 live births, accounting for 7% of all neonatal gastrointestinal perforations [1]. Nearly 47% of NGP were considered idiopathic while rest were mainly due to mechanical ventilation, feeding tubes, necrotizing enterocolitis, duodenal/jejunal obstruction, antenatal drugs (aspirin) and oesophageal atresia/trachea oesophageal fistula [2,3]. The symptoms are sudden deterioration of the clinical condition with difficulty breathing and abdominal distension. A simple X-ray of the abdomen is the first line investigation to visualize the pneumoperitoneum. Complete blood count, C - reactive protein, and arterial blood gas will also support investigations in diagnosing the risk factors and future management. The main treatment is ventilatory support and prompt surgical repair of the perforation. This case report is of a case of NGP in a preterm newborn with a single perforation in the greater curvature of the stomach, which was successfully treated with surgery.","PeriodicalId":73565,"journal":{"name":"JOSPT cases","volume":"216 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-08-02","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"135064552","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

We describe a case of an infant with chronic lung disease and pulmonary hypertension secondary to omphalocele. This report narrates the effects of treprostinil [1,2], a pulmonary vasodilator prostaglandin, during a continuous subcutaneous infusion used to control pulmonary hypertension with a metabolic profile obtained via an indirect calorimeter (IC) (CCM Express, MC Diagnostics, St. Paul, MN). Treprostinil is a prostanoid that targets the prostacyclin pathway resulting in vasodilation, inhibition of smooth muscle proliferation and platelet aggregation [3]. The systemic effect of treprostinil may modify energy metabolism in a dynamic manner providing a challenge for caretakers to provide appropriate nutritional support.","PeriodicalId":73565,"journal":{"name":"JOSPT cases","volume":"46 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-06-12","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"136311250","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Of obscure pathogenesis, intranodal palisaded myofibrobastoma may concur with overexpression of cyclin D1 or viral oncogenesis and manifests as a painless, gradually progressive tumefaction. Pathognomonic morphological features appear as proliferation of spindle shaped cells with nuclear palisading, a collagenous pseudo-capsule and compressed circumscribing peripheral lymph node tissue. Also, amianthoid fibres delineating fine, peripheral spokes, focal pauci-cellular zones and extracellular or intracellular fuchsinophilic bodies may be discerned.","PeriodicalId":73565,"journal":{"name":"JOSPT cases","volume":"14 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-05-23","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"135287169","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This has the appearance of white striated patches with poorly defined borders caused by a developmental error leading to myelination of retinal ganglion cell fibers by ectopic oligodendrocytes. The prevalence of MRNFs is low, ranging from 0.34% [1] to 0.98% [2], and there is no variation according to the origin or sex of the patients. Apart from associated pathologies, this type of fiber remains stable over time [2]. It is considered as a rare and mostly benign congenital anomaly. The objective is to show that in young children, the presence of extensive myelin fibers should be monitored more regularly by ophthalmologists.","PeriodicalId":73565,"journal":{"name":"JOSPT cases","volume":"7 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-05-18","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"135813089","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
