Almost immediately after Roberto Bolli asked me to write a short editorial, I knew the theme had to be about changes I've seen at the American Heart Association over the last few years, as well as forecasting the road ahead. I suppose Roberto asked me to write this because I am the incoming Chair of the Basic Cardiovascular Sciences (BCVS) Council and am supposed to have a vision of what I want to accomplish during my tenure. Or maybe this is compensation for my last submission to Circulation Research being rejected. I'm okay with that. Really.\u0000\u0000Mid-July is not a time when one wants to visit the California desert, where the outdoor afternoon temperatures are high enough to denature DNA. That is unless the American Heart Association holds their annual BCVS Scientific Sessions in Rancho Mirage at the luxuriously air conditioned Rancho Las Palmas Resort. The BCVS annual meeting is a recurring treat for cardiovascular researchers that never disappoints, and this year was no exception. A delightful characteristic of the BCVS conference is seeing the personalities of the organizers infused into the program, so that the content is renewed annually with fresh perspectives.\u0000\u0000The scientific program assembled by conference cochairs Gerald Dorn and Peipei Ping was spectacular and unlike anything we have seen before at a BCVS meeting. Featuring internationally renowned faculty drawn from the proverbial 4 corners of the earth, the conference ran like clockwork at an unrelenting pace. While Friel acknowledges that history is always a fiction, he is also committed to the project of recording historical facts in another tune. Friel's self-conscious use of such double historical activities enables him to make us question the grand narrative, as shown in Lombard's nationalist accounts of the life of Hugh O'Neill. Unlike Lombard, Friel makes the history of Hugh O'Neill as that of a divided figure, who sustains a delicate balance between supporting his fellow chieftains and a semblance of loyalty to the English Queen. It is ironical that while Friel sees himself as a postmodernist who deconstructs Lombard's nationalistic version of O'Neill, he also accepts Lombard's view that history is a narrative made by a historian-maker. Friel shares with Lombard many historical ideas, but the O'Neill biography Friel is writing in this play is similar to the biography O'Neill himself wants to be written, that is, the story of a contradictory man torn between the cultural values of Englishness and Irishness. Just as Friel's O'Neill finds himself divided between the local hostilities of Anglo-Irish relations and the wider prospects of European politics, so Friel posits himself in a carefully balanced dialectic between Lombard's theory and O'Neill's wish (to depict himself as a contradictory man). Friel's use of meta-historical dramatic technique enables him to exhibit a postmodern skepticism of historical narratives. But the play cannot simply be assimilated into the canon of postmodernism. This study assessed the ability of medical and nurse-practitioner students to answer clinical questions using an information retrieval system. It also attempted to identify the demographic, experience, cognitive, personality, search mechanics, and user-satisfaction factors associated with successful use of a retrieval system. METHODS Twenty-nine students completed questionnaires of clinical and computer experience as well as tests of cognitive abilities and personality type. They were then administered three clinical questions to answer in a medical library setting using the MEDLINE database and electronic and print full-text resources. RESULTS Medical students were able to answer more questions correctly than nurse-practitioner students before and after searching, but both had comparable improvements in the number of correct questions before and after searching. Successful ability to answer questions was also associated with having experience in literature searching and higher standardized test-score percentiles. CONCLUSIONS Medical and nurse-practitioner students obtained comparable benefits in the ability to answer clinical questions from use of the information retrieval system. Guidelines on the appropriate search strategy to use for systematic reviews have been proposed. However, research evidence supporting these recommendations is limited.</p><p><strong>Objective: </strong>This study investigates the effectiveness of various systematic search methods used to uncover randomized controlled trials (RCTs) for systematic reviews. Effectiveness is defined as the proportion of relevant material uncovered for the systematic review using extended systematic review search methods. The following extended systematic search methods are evaluated: searching subject-specific or specialized databases (including trial registries), hand searching, scanning reference lists, and communicating personally.</p><p><strong>Methods: </strong>Two systematic review projects were prospectively monitored regarding the method used to identify items as well as the type of items retrieved. The proportion of RCTs identified by each systematic search method was calculated.</p><p><strong>Results: </strong>The extended systematic search methods uncovered 29.2% of all items retrieved for the systematic reviews. The search of specialized databases was the most effective method, followed by scanning of reference lists, communicating personally, and hand searching. Although the number of items identified through hand searching was small, these unique items would otherwise have been missed.</p><p><strong>Conclusions: </strong>Extended systematic search methods are effective tools for uncovering material for the systematic review. Cultural appreciation of the importance of libraries encourages their funding; lack of such appreciation discourages it. Medical library development is driven by culture in general and the culture of physicians in particular. Nineteenth-century North American medical library funding reflected the impact of physician culture in three phases: (1) Before the dawn of anesthesia (1840s) and antisepsis (1860s), when the wisdom of elders contained in books was venerated, libraries were well supported. (2) In the last third of the nineteenth century, as modern medicine grew and as physicians emphasized the practical and the present, rather than books, support for medical libraries declined. (3) By the 1890s, this attitude had changed because physicians had come to realize that, without both old and new medical literature readily available, they could not keep up with rapidly changing current clinical practice or research. As part of the survey process, the CSMS assesses the quality of the hospitals' libraries. In 1987, the CSMS adopted the Medical Library Association's (MLA's) \"Minimum Standards for Health Sciences Libraries in Hospitals.\" In 1990, professional librarians were added to the survey team and, later, to the CSMS CME Committee. Librarians participating in this effort are recruited from the membership of the Connecticut Association of Health Sciences Librarians (CAHSL). The positive results of having a qualified librarian on the survey team and the invaluable impact of adherence to the MLA standards are outlined. As a direct result of this process, hospitals throughout the state have added staffing, increased space, and added funding for resources during an era of cutbacks. Some hospital libraries have been able to maintain a healthy status quo, while others have had proposed cuts reconsidered by administrators for fear of losing valuable CME accreditation status. Creating a relationship with an accrediting agency is one method by which hospital librarians elsewhere may strengthen their efforts to ensure adequate library resources in an era of downsizing. Green","doi":"10.5860/choice.44-2739","DOIUrl":"https://doi.org/10.5860/choice.44-2739","url":null,"abstract":"The Centers for Disease Control and Prevention (CDC) have assembled a large and varied collection of still images, image sets, and multimedia files to create the Public Health Image Library (PHIL™), a Website available at phil.cdc.gov. PHIL provides an organized, universal electronic gateway to the CDC's pictures. Taken from various public health sources including the CDC itself, most images are in the public domain and are not copyrighted. It is recommended, however, that credit be given to the source when pictures are used. \u0000 \u0000Educators, scientists, public health personnel, the media, and people worldwide may find the collection useful. Subjects include viruses, bacteria, fungi, parasites, people with various diseases, statistical charts, and notable public health figures, as well as CDC personnel. The database covers a wide range of dates, and searches can be limited by date. \u0000 \u0000Search terms may be entered in a search box, or users can click the Search button to view hierarchically organized categories of people, places, and sciences. Each category has several subdivisions. By clicking on the plus sign to the left of an entry, users display the subdivisions along with the number of images in the category. The Science category, for instance, includes among other topics “Anatomy, Gross and Microscopic,” “Diseases,” and “Organisms and Infectious Agents.” Not all entry terms have images. In order to view the images associated with a category, users click on the category label and then click on the large right-pointing arrow so that the selected category appears in the “Search on These Categories” box. Then, clicking the “Go” button brings up thumbnail views of the images assigned to the selected category. If multiple categories are selected for searching, a Boolean “AND” is assumed, and only pictures assigned to all selected categories will be retrieved. A “Text Based/Advanced Search” option allows users to search on multiple terms and to limit by image type (still images, multimedia files, etc.) and by date. Results may be sorted by creation date, submission date, title, or image identification (ID) number. A search by image ID number is also available. \u0000 \u0000Search results are displayed as thumbnail photos (10 per page) in a small table on the left side of the screen. The table also includes columns that give the title, description, and assigned categories for each image. Viewing the thumbnails on a twelve-inch monitor requires scrolling to see all four columns. By clicking on a thumbnail, a full-size version of the image is displayed on the right of the screen. Images can be opened in another window or downloaded. Function buttons at the top of the larger frame may be used to move forward or backward in the list of retrieved images, to get information about the image, or to return to the search mode. {"title":"Non-librarian health professionals becoming librarians and information specialists: results of an Internet survey.","authors":"C R Fikar, O L Corral","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objectives: </strong>To obtain basic information about non-librarian health professionals who become librarians and information specialists.</p><p><strong>Methods: </strong>The survey was a Web-based questionnaire. A non-random sample of persons was obtained by posting messages to several large Internet electronic discussion groups. Individuals who met the selection criteria and were willing to participate filled out a Web-based form designed using common gateway interface (CGI) programming.</p><p><strong>Results: </strong>118 forms were analyzed. Three subgroups of participants were identified and statistical comparisons among these groups were carried out for many of the quantitative questions. Information concerning reasons they left their original field; factors influencing their choice of the field of library and information science; reactions of family, friends, and colleagues; and interactions with patrons and other information about this group was obtained and summarized. A health sciences background was seen as helpful in the new career as information specialist. Most people were happy with their new profession despite negative reactions from colleagues, relatives, and, occasionally, patrons. Feelings of regret and abandonment of their patients were noted by some. Many persons did not know that peers had made similar career changes.</p><p><strong>Conclusions: </strong>A health sciences background imparts an expertise in both the vocabulary and subject matter of medicine that non-biomedical individuals would not ordinarily have. Although becoming a librarian may be perceived as a very positive career change for an individual, societal opinion and pressure can make such a career change difficult.
