{"title":"Understanding the self-management experiences of people with heart failure with preserved ejection fraction (HFpEF), their caregivers and the health care professionals who support them: Systematic review and qualitative meta-study.","authors":"Julia Frost, Samantha Barbara van Beurden, Jess C Bollen, Valerie Wells, Colin Greaves, Chim Lang, Rod Taylor","doi":"10.12968/bjca.2024.0084","DOIUrl":"10.12968/bjca.2024.0084","url":null,"abstract":"<p><strong>Aims: </strong>To understand the self-management experiences of people with heart failure with preserved ejection fraction (HFpEF), their caregivers and the health care professionals (HCPs) who support them.</p><p><strong>Methods and results: </strong>A systematic review of qualitative and mixed methods literature, published to July 2023. 4560 abstracts describing patients, caregivers, and HCPs experiences of self-management strategies for HFpEF were identified and screened. Of the 349 full texts identified we included seven papers from three data sources (in 47 patients, 81 caregivers, 129 HCPs) in our final dataset. Key theories, methods and results were extracted, reviewed and synthesised using meta-study techniques.We identified that HFpEF is difficult for HCPs to diagnose and difficult for patients and their caregivers to understand. Patients and caregivers can struggle to access support, which can lead to a 'cycle of decline' over time in patients' outcomes /quality of life. However, self-management can be optimised with adequate support, when the burdens of understanding HFpEF and self-management are acknowledged, when personalised goals are set, and feedback is provided to optimise individualised self-management.</p><p><strong>Conclusion: </strong>The limited capacity of HCPs to support HFpEF patients and caregivers to develop effective self-management understanding and practice can lead to disempowerment and loss of control. Tailored interventions are required to maximise HCP enablement of patients and their caregivers to develop flexible and sustainable self-management strategies. Driveline infections are a significant complication of these devices, occurring in 30–40% of cases, which could lead to life threatening sepsis. This study aimed to review UK driveline dressing protocols and infection rates to determine if national standardisation is possible. Six UK heart transplant centres were asked to share their driveline dressing protocols for comparison. National driveline infection data were obtained from the NHS Blood and Transplant database. Infection rates, patient demographics and device types were compared using Chi squared tests, with a P value of <0.05 indicating statistical significance. The study found 38 driveline infections over a 3-year period, with an average infection rate was 4.4% per year. There were no significant difference in infection rates between device types. Dressing protocols varied between centres, but all centres followed international guidance. The timepoint where the highest number of infections were diagnosed was 12 months post implantations, accounting for 23.7% of infections. There were no significant differences in infection rates between centres in 2019–20 or 2021–22, but a significant difference was observed for 2020–21 (P=0.013). UK driveline infection rates are below the average reported in the literature. Despite some differences in dressing protocols, all centres examined in this study maintained a level of infection comparable to reported rates.
