Her husband noted that her right pupil appeared to be dilated. However, on repeat questioning, the patient offered that she had just returned from a Caribbean cruise. She was asked about the use of a scopolamine patch. At that point, she recalled that she had removed a scopolamine patch from behind her right ear and had removed her contact lses just prior to the onset of symptoms. She noted that she had experienced some difficulty removing the contact lens of her right eye. She had removed the contact lens and the scopolamine patch with the right hand. The blurry vision began minutes later. The patient was offered confirmatory testing with pilocarpine drops, but she declined, as she felt very confident in the association of the association of the scopolamine patch with the onset of symptoms. She was discharged with follow up with ophthalmology. Her symptoms resolved fully within 24 hours of ED discharge. Poisoncontrol was contacted and recommended symptomatic treatment only. The patient’s painresolved while in the emergency department. Seed and rind preparations have been used as an herbal remedy in East Asia. A number of research articles point to ongoing investigations into the antiinflammatory and possible antiviral properties of Poncirus species. {"title":"Case Report: Cold thermal injury and syncope from pressurized dust cleaning spray","authors":"J. Air spray cleaners contain halogenated gas, which serves as a propellent to blow dust from electrical and computer devices. The halogenated compound can cause euphoria and can be used as an inhaled abused substance. Use of such substances can be associated with syncope, sudden death, hypoxia and cold thermal injuries. Airway swelling has been reported. Orofacial and digital frostbite has been described as a clue to the detection of the use of such inhalants. However, adults are more likely to present with complications requiring emergency medical treatment. One of the challenges in the management of mastoiditis is recognizing symptoms and beginning treatment with antibiotics early to prevent progression of disease and potential surgical management. This case report details an unusual case of a 41-year-old woman whose indolent symptoms went unrecognized for three weeks before presenting to the emergency department with unilateral ear pain and hearing loss. Imaging revealed coalescing mastoiditis with erosion of the medial cortex into the jugular foramen and enhancement suggestive of meningitis. Pelvic floor dysfunction negatively impacts female's quality of life and if it is associated with female with disability, it increases the severity of their physical, mental, and social well-being and can hinder their full and effective participation in society. Most commonly presented in the fifth or sixth decade of life with a prior history of nephrolithiasis, obstructive uropathy, or recurrent urinary tract infections. Here, we discuss a 45-year-old male and a 73-year-old female who came with abdominal pain and weight loss for two months. An enlarged and distorted renal outline, with altered echotexture and calculus in renal pelvis was revealed on ultrasound examination.
