Given its histological particularity, the diagnosis of this tumor still represents a challenge for pathologists and surgeons. Sarcomatoid carcinoma of the mandible is a very rare and aggressive entity. To date, only sporadic cases have been reported in the literature. Aim: To report a rare case of aggressive mandibular sarcomatoid carcinoma treated with radical surgery and adjuvant chemoradiation, and discuss the re-lated literature. Case Report: We introduce a rare case of mandibular sarcomatoid carcinoma occurring in a 34-year-old woman. Treatment consisted of radical surgery and re-excision for positive margins, adjuvant radiation therapy (66 Gy in 33 fractions in simultaneous integrated boost delivered using Volumetric Modulated Arc Therapy) and Cisplatin-based concurrent chemotherapy. The tumor displayed aggressive behavior and high metastatic po-tential, causing tumour recurrence and extensive lung metastasis. Conclu-sion: Sarcomatoid carcinoma of the mandible is an extremely rare malignant entity, with a high tendency for local recurrence, distant metastasis. It is a very effective way to detect lung cancer by pathological pictures of lung cancer cells. Therefore, improving the accuracy and stability of diagnosis is very impor-tant. In this study, we develop an automatic detection scheme for lung cancer cells based on convolutional neural networks and Swin Transformer. Microscopic images of patients’ lung cells are first segmented using a Mask R-CNN-based network, resulting in a separate image for each cell. Part of the background information is preserved by Gaussian blurring of surrounding cells, while the target cells are highlighted. The classification model based on Swin Transformer not only reduces the computation but also achieves better results than the classical CNN model, ResNet50. The final results show that the accuracy of the method proposed in this paper reaches 96.16%. Design: Prospective \u0000randomized clinical trial. Patients: 48 patients were randomized to \u0000either adjuvant postoperative radiology alone versus concurrent \u0000chemoradiotherapy (weekly cisplatin 40 mg/m2 for 6 cycles) “with \u0000resected high-risk salivary tumors of the large and minor salivary gland”. Main Outcome Measures: Recurrent locoregional Free survival, distant \u0000free survival, and overall survival. Results: Out of the 48 participants \u0000in the study 31 patients had parotid gland tumors. 23 patients received solely \u0000adjuvant radiation while 25 patients received concurrent chemoradiotherapy. In \u0000the chemoradiation group, platinum-based regimens were employed in all. The mean \u0000age in both groups was 48 years. Adenoid cystic carcinoma was the primary \u0000pathogenic form of both arms 56% (28 cases). Stage II patients were 35% and \u000032%, stage III was 39% and 48% and stage VIa were 26% and 20% in the radiation \u0000arm and chemoradiotherapy arm respectively. 40 of 48 patients (83%) had close \u0000or positive surgical margins and 30 of 48 patients (62%) have a perineural \u0000invasion. Both risk variables are more or less well balanced in both arms with \u0000no statistical difference. The 2- and 4-year estimates of the locoregional \u0000recurrence-free survival rate in the chemoradiation group were 95% and 73%, \u0000compared to 77.4% and 43.6% in the radiation arm respectively (p = 007). In the \u0000two-and four-year-old chemoradiation arm distant free metastases were 100% and \u000059% compared to 68% and 39% respectively in the radiation arm (p = 0.08). The \u0000overall survival estimates for 2 and 4 years were 93% and 78% respectively in \u0000the Chemoradiation Group but in the radiation-alone group were 95% and 48% \u0000respectively. The statistically significant differences were p = 0.009 by \u0000log-rank testing. Treatment was generally tolerated, although, in the \u0000chemoradiation group adverse symptoms, mainly mucositis increased. He was diagnosed as a case of isolated esophageal \u0000Hodgkin’s lymphoma that was diagnosed based on endoscopic and radiological findings. Clinical \u0000manifestations, radiological imagines and histopathological features were \u0000described.
