The flap was designed from the proximal end of rasceta and the donor sites were sutured directly. The size of flaps was 3.0 cm ×1.5 cm-4.0 cm×2.2 cm. The superficial branch of the radial artery in the flap was used to bridge the finger artery. And the vein of proximal and distal ends in the finger was bridged by the subcutaneous vein. The proper palmar digital nerve defect was bridged by palm skin graft of median nerve. The appearance, feeling and joint function of fingers was followed-up regularly after operation. \u0000 \u0000 \u0000Results \u0000All transfering flaps survived and all cases were followed-up for 7 to 33 months. The donor sites got primary healing with straight scars. The appearance and texture of the flaps were satisfactory. Two-point discrimination ranged from 8 to 11 mm. The pain sensation, warmth sensation and touch sensation of the flaps got better. And the appearance and functions of severed fingers recovered well. \u0000 \u0000 \u0000Conclusion \u0000The Flow-through flap pedicled with superficial palmar branch of radial artery is easy to harvest and anastomose, which is masked and a small incision for the donor site. Wei","doi":"10.3760/CMA.J.ISSN.1001-2036.2019.04.005","DOIUrl":"https://doi.org/10.3760/CMA.J.ISSN.1001-2036.2019.04.005","url":null,"abstract":"Objective \u0000To investigate the clinical efficacy of one-stage repairing the wounds after excision of soft tissue malignant tumor in lower limb with deep inferior epigastric perforator (DIEP) flaps. \u0000 \u0000 \u0000Methods \u0000From August, 2016 to July, 2018, 11 patients(8 males and 3 females; aged 35-76 years) with the soft tissue defects in the lower limb underwent immediate reconstructive operations with DIEP flap after their tumor resection. The defects ranged in area from 11 cm×7 cm to 25 cm×10 cm after extended resection, including 5 cases of bone exposure. The DIEP flap area ranged from 11 cm×8 cm to 26 cm×18 cm. Among them, 9 cases were treated with free transplantation and 2 cases with pedicle transplantation. The donor sites were closed directly. The patients’ limb function, tumor recurrence and the healing of surgical incision had been followed-up. \u0000 \u0000 \u0000Results \u0000Eleven flaps survived and the donor sites healed directly. One flap had skin pigmentation due to postoperative vascular crisis, and the other flaps had satisfied texture and appearance. All patients were followed-up from 6 to 30 months. The lower limb movement was normal. All the donor sites had a good healing with no pain and abdominal complications. No tumor recurrence was observed during the followed-up period. \u0000 \u0000 \u0000Conclusion \u0000DIEP flap have many advantages, such as large cutting area, which is suitable for large-area wound repair after extended excision of soft tissue malignant tumor in lower limb. At the same time, the perforator vessel is constant and the caliber are thick, also the donor area is concealed and can be sutured directly. Then the free peroneal artery perforator flap was used to reconstruct tissue defect caused by tumorectomy. There were 6 cases of squamous-cell carcinoma of palatine, 2 cases of adenoid cystic carcinoma of palatine, and 6 cases of maxillagingiva squamous cell carcinoma. The incised area of flap was 4.0 cm×5.0 cm to 7.0 cm×8.0 cm. There were 5 cases of hard palate soft tissue defect repair and 9 cases of simultaneous repair of soft and hard palate. Followed-up by outpatient service in 3-12 months after operation, postoperative maxillamorphology, phonetic function, swallow function, opening degree and prognosis of the patients were evaluated. \u0000 \u0000 \u0000Results \u0000All 14 implanted flaps were alive. One flap had vascular crisis, and rescued by surgical exploration and timely rescue. Three flaps were prolonged healing. All donor sites were sutured directly. All surgical incisions healed primarily. Half a year after the operation, the appearance of maxilla was formed gradually. The phonetic function, swallowing function, opening degree and movement of lower leg were all recovered normal. One year after the operation, epithelization was done in 6 cases. And there was no tumor recurrence. \u0000 \u0000 \u0000Conclusion \u0000The peroneal artery perforator flap has long vascular pedicle, larg diameter, high survival rate after vascular anastomosis and relatively concealed donor area. Anatomic observation on 10 legs from fresh human cadaver were performed. The location of cutaneous branches of the posterior tibial artery was observed and its diameter and length was measured. Five legs were prepared to investigate the cutaneous branches of posterior tibial artery. The anastomosis of cutaneous branches of posterior tibial artery was observed by PVA-bismuth oxide perfusion for molybdenum target X-ray arteriography in 5 perfused legs. The cutaneous branches with diameter over 0.2 mm in 10 legs of latex perfusion microdissection were included in the statistical analysis. The data were clustered and analyzed to find the location of distant and near cutaneous branches, which was called the gathering point of cutaneous branch vascular plexus. Secondly, the measured data of distal and near segments containing cutaneous branches were compared by t-test. Then the distribution of cutaneous branches of posterior tibial artery on the tibiofibular side was compared by Chi-square test. It was considered to be significant if P value was under 0.05. \u0000 \u0000 \u0000Results \u0000①There were 4.3 cutaneous branches raised from the posterior tibial artery. There was no significant difference on the tibial and ribula side distribution of the cutaneous branches from the posterior tibial artery(P>0.05). ②The distal cutaneous branch clusters was located at about 1/5 of the distal leg and there were 3.6 cutaneous branches raised from the posterior tibial artery. While the proximal clusters was located at 1/3 of the proximal leg and there were 0.7 cutaneous branches raised from the posterior tibial artery. There were no significant differences in the diameters (P=0.28) and pedicle length (P=0.14) between distal and proximal cutaneous branches. ③ There were the large cutaneous perforators (≥1.0) mm from the posterior tibial artery at (6.37±1.22) cm proximal to the medial malleolus. The diameter and pedicle length of the distal perforators were (1.11±0.09) mm and (6.53±1.51) mm respectively. ④ The vascular chains parallel to the posterior tibial artery were formed via anastomosis of the adjacent cutaneous perforators. \u0000 \u0000 \u0000Conclusion \u0000The cutaneous expenditure of posterior tibial artery is constant, with a certain pedicle length and diameter. There are 2 relatively dense vascular plexus of cutaneous branches. The dominant perforator was selected according to CTA before operation. The origin, course, locations at the level passing through the fascia lata of the perforators and digital flaps were located and marked under the guidance of AR navigation technology. The flaps area were 6 cm×8 cm-8 cm×12 cm, and the donor sites were closed directly. The effect of the AR navigation was explored intraoperatively. The function of reconstructed tongue and donor site were followed-up 3 to 6 months post-operatively. \u0000 \u0000 \u0000Results \u0000All navigation-positioned perforators were confirmed by AR preoperatively. Intraoperative exploration showed that the perforators were accurately located. All flaps survived, and no vascular crisis occurred. The donor sites healed primarily with linear scars. The tongue was plump, and the function of voice and swallowing were favorable. \u0000 \u0000 \u0000Conclusion \u0000CTA combined AR navigation has the advantages of accurate location, easy to use and low cost in tongue reconstruction with free ALTP flaps. Cause of injury: 1 electric injuries, 2 tumor-related wounds, 1 deep burns, 2 mechanical trauma, and 1 necrotizing fasciitis. All patients were underwent free flap transplantation. There were 5 cases of anterolateral thigh artery perforator flap, 1 case of superficial circumflex iliac artery perforator flap, and 1 case of first dorsal metatarsal artery perforator flap. The application of venous super-drainage technology was carried out according to needs and specific conditions. Two sets of venous passages were prepared in both recipient and donor site to form a double set of venous reflux super-drainage mode. Blood supply, swelling, exudation, secondary exploration and survival rate of the flap were observed after operation, and regularly followed-up. \u0000 \u0000 \u0000Results \u0000All 7 flaps survived. Venous super drainage technique was applied in 7 cases. No arteriovenous crisis occurred after the operation. The flaps had good blood circulation, slight swelling, less exudation, rapid edema regression and no secondary surgical exploration. Followed-up for 2-18 (average 10.5) months, there was no infection recurred. Flaps survived well, and the donor sites healed well without sensory loss. A total of 55 New Zealand white rabbits were randomly divided into two groups (First group, n=30 and Second group, n=25). There were 5 subgroups (n=6) in the first group, which were autograft and custom-anatomic nerve conduits (CANC) with different channel (1-CANC, 2-CANC, 3-CANC, 4-CANC) that implanted to repair the rabbit sciatic nerve defect (10 mm). The electrophysiological, triceps muscle wet weight recovery rate, histological study and ankle index analysis were used to evaluate the treatment of each group at 12 and 24 weeks postoperatively. There were 5 subgroups (n=5) in the second group. The simultaneous retrograde tracing method was applied to compare with the number of mismatched nerve fibers at 24 weeks postoperatively. All data were recorded and analyzed by One-way ANOVA method, the Turkey’s method was used to compare the differences between each subgroup. The difference was considered to be statistically significant if P<0.05. \u0000 \u0000 \u0000Results \u0000The autograft group showed the best recovery in the electrophysiology, histology study and ankle index at 12 and 24 weeks postoperatively (P 0.05), but diameters of nerve fiber and myelin thickness were higher in 2-CANC and 3-CANC [(10.67±0.56) μm,(10.65±0.53) μm, respectively] compared with 1-CANC and 4-CANC groups [(8.43±0.63) μm, (9.03±0.55) μm, respectively]. The differences were similar in electrophysiological, wet weight recovery rate of triceps muscle, histological study and ankle index analysis. Simultaneous retrograde tracing showed that the autograft group had highest total number of labeled profiles, but no significant difference of the total number of labeled profile was showed among the CANC groups. However, the 1-CANC group[(7.1±2.4)%] showed highest percentage of the FB-NY-neurons than other CANC groups[(2.7±1.9)% in 2-CANC, (2.5±2.3)% in 3-CANC, and(2.2±1.2)% in 4-CANC](P<0.05). \u0000 \u0000 \u0000Conclusion \u0000The autograft group showed the best results among all groups. Compared with the 1-CANC group, the 2-CANC and 3-CANC group obtained more mature regenerated nerve fibers and with a fewer mismatch rate. Fifty of them were performed conventional anatomical procedure, the other 4 were prepared with sectional anatomical technique. The seats and branches of the PPDN in the medial of the great toe were observed; The width and thickness of the nerve were measured at the first metatarpophalangeal joint(FMPJ), along with its proximal and distal sides 0.5 cm. The origin and origin of fascia were observed by foot dissection. Masson staining was used to observe the tissue changes of the nerves in the FMPJ. \u0000 \u0000 \u0000Results \u0000The PPDN of the medial great toe run between the flexor pollicis longus tendon and the abductor pollicis tendon at the proximal, issued (4.21±0.12) final branches. And governed the sensation of the medial half of the great toe. The width of the nerve at the FMPJ was (3.50±0.09) mm, which was significantly increased compared with that of the near [(1.58±0.04) mm] and far [(1.56±0.03) mm] from the joint. The difference was statistically significant (P<0.05); The thickness of the nerve in the proximal segment was (0.83±0.04) mm, and that in the distal segment was (0.82±0.03) mm. Compared with that in the FMPJ [(0.67±0.02) mm], the difference was statistically significant (P<0.05). A deep fascia was observed on the superficial surface of the PPDN at medial great toe, which was stretched between the tendon sheath of the flexor pollicis longus tendon and the tendon of the abductor pollicis muscle. Firstly, the potential binding sites between miR-299 and COL4A3-3'UTR were predicted using bioinformatics. Then, the wild and mutant COL4A3-3’UTR sequences were amplified by PCR and cloned into psiCHECK-2 plasmid to construct corresponding recombinant vectors. The vectors were validated by enzyme digestion and gene sequencing. Finally, the cells were resuscitated, amplified, transfected and divided into 4 groups: COL4A3-WT+miR-299/NC group, COL4A3-WT+miR-299-inhibitor/NC-inhibitor group, COL4A3-MUT+miR-299/NC group and COL4A3-MUT+miR-299-inhibitor/NC-inhibitor group. Each group contains 3 holes, respectively. Luciferase activity in each group was determined using a dual-luciferase assay kit. The statistical analysis was conducted and differences between groups were compared by t test. Probabilities lower than 5%(P<0.05) were considered statistically significant. \u0000 \u0000 \u0000Results \u0000Enzyme digestion and DNA sequencing showed that the dual-luciferase reporter gene vector of psiCHECK-2-COL4A3 was constructed successfully. Luciferase assay demonstrated that in wild COL4A3 gene, luciferase activity reduced in the miR-299 transfection group (The average R/F value was 59.38%) compared with the NC group (The average R/F value was 100.00%), with a statistical significant difference (P 0.05. \u0000 \u0000 \u0000Conclusion \u0000The dual-luciferase reporter gene vector of the 3’UTR of the COL4A3 gene is constructed successfully. Sixteen cases were males and 9 cases were females. Seventeen defects were in thumb dorsal and 8 cases were in thumb pulp. A modified dorsal island flap of index finger was used and the dorsal metacarpal superficial vein fascial flap could be harvest and combined to repair the thumb pulp defect if necessary. A relaying perforator flap pedicled on the second dorsal metacarpal artery was raised through the same incision to cover the donor site without skin graft. Followed-up was made by clinic, telephone and WeChat. \u0000 \u0000 \u0000Results \u0000The patients were followed-up for 6 to 18 months. All flaps survived completely without complications. The color, texture and contour of the flaps was good. Only 1 linear scar was left in the dorsum of the hand and no skin grafts. The second dorsal metacarpal artery flap was used to cover the donor site. Twenty-one cases (84%) were satisfied with the postoperative appearance of the thumb. The function was assessed as excellent in 16 fingers, good in 6 fingers and fair in 3 fingers. No complication occurred in the donor site. \u0000 \u0000 \u0000Conclusion \u0000It is possible to use the modified dorsal island flap of index finger to repair different types of thumb tip defects.
