We retrospectively evaluated 19 patients with metastatic colorectal cancer (mCRC) who underwent plasma-based CGP (Guardant360 CDx) between November 2023 and May 2025. ctDNA was detected in 18 of 19 patients (95%) with previously treated mCRC. The median turnaround time from blood collection to result was 11 days (range: 7-15). The median number of gene alterations was 5 (range: 1-29), and pathogenic/likely pathogenic alterations were identified in 15 patients (79%). ctDNA was preferentially selected over tissue CGP in patients with small tumor burden, lack of contemporary tumor sample, or double cancer (considering spatial and temporal heterogeneity). Three patients initially identified with RAS mutant tumors by tissue were subsequently identified as RAS wild-type by plasma CGP. Those were treated with anti-epidermal growth factor receptor (EGFR) therapy, resulting in partial response or stable disease. These observations suggest that plasma-based CGP may provide additional molecular information that could be clinically informative in mCRC in a real-world setting. Case 1 was a 70-year-old woman who underwent total colonoscopy at our center. An 8-mm lesion classified as 0-IIa+IIc in the Paris classification was detected in the descending colon and removed with CSP. Case 2 was a 60-year-old man who underwent total colonoscopy at our center. A 6-mm lesion classified as 0-Is in the Paris classification was detected in the rectum and removed with CSP. Both lesions resulted in well-differentiated adenocarcinoma with unidentified horizontal and vertical margins pathologically. We safely performed underwater endoscopic mucosal resection (UEMR) for the post-CSP scar because residual cancer could not be ruled out completely. Pathological results showed that there was no residual cancer in either case. We report two cases of successful salvage UEMR for early colorectal cancer with unidentified horizontal and vertical margins after CSP. Salvage UEMR after CSP in the early stage may facilitate the exploration of residual cancer or the resection of residual cancer.
","PeriodicalId":56349,"journal":{"name":"Journal of the Anus Rectum and Colon","volume":"10 3","pages":"500-506"},"PeriodicalIF":1.5,"publicationDate":"2026-07-25","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC13423437/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148632700","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

We conducted an exploratory trial of a Doppler ultrasonography method using a high-frequency probe with superb microvascular imaging (SMI) to assess intestinal perfusion during colorectal cancer surgery.

Methods: In this single-center prospective pilot study, we enrolled patients undergoing colorectal cancer resection with anastomosis. Intraoperatively, SMI assessed intramural microvascular flow to determine the perfusion boundary, followed by ICG-FI. The SMI boundary was defined as the line where vertical vessels penetrating the muscularis propria were no longer visible. The primary endpoint was the difference in blood flow boundaries between ICG-FI and SMI. The secondary endpoints were inter-observer reproducibility of SMI boundary identification and safety within 30 days.

Results: Seven patients were analyzed for efficacy (one excluded because of invalid SMI data) and eight for safety. The median difference in blood flow boundaries between ICG-FI and SMI was -4 mm (range, -60 to 0 mm), with SMI identifying a more proximal boundary. Inter-observer variability for SMI boundary detection showed a median absolute difference of 2 mm (range, 0-4 mm). No procedure-related adverse events occurred.

Conclusions: Our findings revealed differences in perfusion boundary location between SMI and ICG-FI, and intraoperative SMI acquisition and boundary identification were technically feasible. Analyzable data were obtained in seven of eight patients, with high inter-observer reproducibility and no procedure-related adverse events. Further prospective validation with standardized SMI acquisition and interpretation is required before clinical implementation.
","PeriodicalId":56349,"journal":{"name":"Journal of the Anus Rectum and Colon","volume":"10 3","pages":"410-420"},"PeriodicalIF":1.5,"publicationDate":"2026-07-25","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC13423438/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148632643","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
