Based on atypical clinical signs and various radiological results the period to obtain a diagnosis and initiate the appropriate treatment is often very long. This case reports on a 32-year old female patient with knee pain on flexion and stress, which rarely occurred at night. Radiofrequency ablation (RFA) was contraindicated for IAOO in the trochlea femoris due to the direct subchondral location. {"title":"[Influence of the COVID-19 pandemic on the physiotherapeutic and medical follow-up care after reconstructions of the anterior cruciate ligament].","authors":"Benjamin Bartek,&nbsp;Tobias Jung,&nbsp;Antonia Schwiedernoch,&nbsp;Carsten Perka,&nbsp;Yannick Palmowski","doi":"10.1007/s00132-021-04197-3","DOIUrl":"https://doi.org/10.1007/s00132-021-04197-3","url":null,"abstract":"<p><strong>Background: </strong>The aim of this study was to examine the influence of the COVID-19(coronavirus disease 2019) pandemic on the medical and physiotherapeutic follow-up care after reconstructions of the anterior cruciate ligament.</p><p><strong>Methods: </strong>116 patients (72 men and 44 women) who had received a reconstruction of the anterior cruciate ligament between September 2019 and December 2020 were included in this study. These patients were divided into two groups depending on the date of surgery: one group that had received surgery in 2019 before the COVID-19 pandemic and one group that had undergone surgery in 2020 and was directly affected by the COVID-19 pandemic. All patients were interviewed using a standardized questionnaire regarding the influence of the coronavirus pandemic on the medical and physiotherapeutic follow-up care, as well as the moment of return to work. In addition, the range of motion of the respective knee 3 months postoperatively was analysed based on physical examination.</p><p><strong>Results: </strong>At 3 months postoperatively, patients who had undergone surgery in 2020 showed a clear trend towards a higher frequency of extension deficits of ≥ 5° (18.8% vs. 4.3%, p = 0.097) or an inability to bend the knee ≥ 120° (23.3% vs. 10%, p = 0.197) compared to those who had received surgery in 2019. Patients who had undergone surgery in 2020 reported significantly longer delays for appointments, a higher number of futile attempts to get an appointment and a higher number of cancelled appointments, regarding both medical and physiotherapeutic follow-up care. 34.9% of the patients who received surgery in 2020 indicated that they were able to reduce the duration of their sick leave due to the increased possibilities of working in a home-office situation during the pandemic. Alternative treatment options due to the pandemic were offered by 13.3% of the physiotherapists and 12.2% of the physicians.</p><p><strong>Conclusion: </strong>Although the physiotherapeutic and medical follow-up care was not directly affected by a \"lockdown\", the pandemic led to significant restrictions, which are also reflected in a clear trend towards worse clinical outcomes. {"title":"[Spinal manipulation therapy in low back pain : Why? When? Where? How?]","authors":"Hermann Locher","doi":"10.1007/s00132-022-04226-9","DOIUrl":"10.1007/s00132-022-04226-9","url":null,"abstract":"<p><strong>Background: </strong>Manipulation and mobilisation for low back pain are presented in an evidence-based manner with regard to mechanisms of action, indications, efficacy, cost-effectiveness ratio, user criteria and adverse effects. Terms such as non-specific or specific are replaced by the introduction of \"entities\" related to possible different low back pain forms.</p><p><strong>Efficacy: </strong>MM is effective for acute and chronic low back pain in terms of pain relief, recovery of function and relapse prevention. It is equally effective but less risky compared to other recommended therapies. MM can be used alone in acute cases and not only in the case of chronic low back pain where it is always and necessarily part of a multimodal therapy programme, especially in combination with activating measures. The users of MM should exclusively be physician specialists trained according to the criteria of the German Medical Association (Bundesärztekammer) with an additional competence in manual medicine or appropriately trained certified therapists. Indication is based on the general justification as a result of anamnesis and clinical examination. With appropriate indications, imaging primarily serves to exclude contraindications. The use of X‑ray diagnostics to determine the treatment technique or direction is obsolete, especially in infants. Detection of segmental dysfunction as the therapeutic indication is of scientific interest only. Other possible objectives of imaging are detection of a non-functional cause of pain or prediction of therapeutic success. In either case, study results to date contradict general imaging, as there is no significant correlation between detected changes and existing or developing pain. The aim of this study was to investigate the correlation between the Oxford Elbow Score (OES) and Single Assessment Numeric Evaluation (SANE) retrospectively. The study examined 86 patients between December 2018 and February 2019 using the Pearson correlation coefficient between SANE and OES. The current method of diagnosing and treating these disorders does not appear to be successful.</p><p><strong>Diagnostics: </strong>A functional approach based on the model of the functional system incorporates all aspects relevant to disorders of the loco-motor system. Most disturbances the system can compensate. If compensating mechanisms are not sufficient, the system decompensates, and symptoms develop. It is important to search not only for structural changes but also for somatic dysfunction, psychosocial factors, and changes in neuromodulation. On this basis, treatment strategies can be developed. If there are complex disturbances on different levels, a multimodal interdisciplinary treatment is indicated.
