{"title":"Lung Cancer Screening in Community-Based Practice in Puerto Rico: A Survey of Puerto Rico Pulmonologists.","authors":"William Rodríguez-Cintrón,&nbsp;Sulimar Morales-Colón,&nbsp;Juancarlo Martínez-González,&nbsp;Joan Albors-Sanchez","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>Lung Cancer (LC) in Puerto Rico (PR) is the fifth most common malignancy (5.2%), the third most common among men (5.9%) and the fifth among women (4.6%), with a mortality of 11.3%. Despite current data demonstrating the importance and clinical value for lung cancer screening LDCT Screening among high risk patients remains low regardless of the potential to prevent thousands of lung cancer deaths per year. Due to significant disparities in health care in PR it is believed that LDCT use for lung cancer screening in PR is not been enforced in the private sector.</p><p><strong>Methods: </strong>A self-administered anonymous survey was provided to a group of pulmonologists at the annual meeting of the PR Pneumology Society. The survey contained questions regarding characteristics of their practice and implementation of lung cancer screening. Provided information was tabulated in percentages.</p><p><strong>Results: </strong>A total of 31 pulmonologists participated in the administration of the survey. Most participants had their medical practice in the metropolitan area (52%), which is the most populated area with best access to physicians and health care services. The sample from the north area comprised 19% of the subjects. All respondents were affiliated to health care institutions. As most of them served 1-3 health care centers (96%) with access to specialized equipment such as Chest CT. Most of the physicians (99%) had availability of chest CT scan within 1 hour from their practices and 97% were aware of the U.S. Preventive Services Task Force lung cancer screening recommendations. Their age range was 41 and over (55%). Despite the above there were discrepancies when asked about lung cancer screening implementation. Sixteen (16) percent did not perform lung cancer screening at all, and 77% that performed screening, reported limitations to it.</p><p><strong>Conclusion: </strong>This data suggests that although lung cancer screening has shown to reduce mortality and is recommended by the USPTF, it is not been conducted appropriately in PR. The main limitation identified was what the health insurance had to offer rather than lack of health insurance. Other factor to take in consideration is the lack of a comprehensive screening program for Lung Cancer anywhere in the island. We present the case of an adult patient with a medical history of CS who presented with septic shock and was subjected to an emergency laparotomy due to severe abdominal distension. Assessment visits to COVID-19 vaccine providers in Puerto Rico were conducted a month after the availability of COVID-19 vaccines in the island.</p><p><strong>Results: </strong>A total 16 vaccine providers were visited, 12 (75%) administering Pfizer-BioNTech vaccine and 4 (25%) administering Moderna vaccine. All providers adhered to correct handling practices after vaccine thawing. Required resources for managing anaphylaxis on site were available in all sites. Few instances of incorrect use of retractable-needle syringes, unapproved temperature monitoring devices, and lack of recorded temperature data were observed. Corrective actions were taken during the evaluation visit.</p><p><strong>Conclusion: </strong>No major deficiencies that could jeopardize vaccine viability or patient safety were found. Although predominantly asymptomatic, it can become a life-threatening disease in an immunocompromised host. Epidemiologic studies in the Western Hemisphere are scarce, but even more scarce are descriptions of the natural course of this disease. Our objectives were to identify the different manifestations and outcomes of Strongyloides stercoralis infection in the Hispanic veteran population in Puerto Rico. We also discuss the importance of pursuing a timely diagnosis in high-risk patients migrating from or traveling to endemic areas, regardless of their symptomatic status.</p><p><strong>Methods: </strong>This was a single-center, retrospective record review study involving patients diagnosed with S. stercoralis via serology, stool samples, or organ biopsies, from 2008 through 2014.</p><p><strong>Results: </strong>A total of 270 patients tested positive; 210 (77.8%) were asymptomatic. The mean age at diagnosis was 75.4 years old. Symptomatic patients had pulmonary (n = 25), gastrointestinal (n = 21), and dermatologic (n = 5) symptoms; 9 had multiple symptoms. Five had hyperinfection, mostly after treatment with systemic steroids or preexisting immunosuppression. The most common laboratory abnormality was eosinophilia. Reasons for testing were eosinophilia, asthma, diarrhea, screening for parasites, and unexplained skin rash.</p><p><strong>Conclusion: </strong>Our study highlights the importance of being aware of this potentially fatal infection, especially when treating patients traveling from endemic countries. The patient reported experiencing-over the course of the following 3 months-withdrawal symptoms when switched to the Alvogen-produced generic formulation in May of 2019 and then to the Sandoz-produced version in July of that same year, she also was positive for fentanyl during that time. As a result, the buprenorphine dose was increased, and the patient was stable at this new dose using the generic versions. Blood levels pre- and post-change (not reported in previous case reports) showed maximum buprenorphine concentration being reached more quickly when the brand-name drug was used. Additionally, the area under the curve (AUC) values indicate that the generic formulation had higher exposures than the brand-name drug. Based on the clinical impact of the brand-to generic switch in this patient, further research in this area is warranted. The demographic and clinical characteristics of the patients and the diameters of the optic nerve sheaths of both eyes of those patients at the time of admission and at the 6th hour after receiving 100% oxygen therapy and/or hyperbaric oxygen therapy were determined. The ages, genders, arterial blood pressure values, symptoms, carboxyhemoglobin levels, and bilateral ONSD values (measured sonographically before and after the oxygen therapy) of the patients were recorded in a data collection form.</p><p><strong>Results: </strong>Measurements of ONSD before and after treatment were observed significantly (P = .01). Significant decreases were observed in ONSDs after treatment (P < .05). Although insulin is the treatment of choice in pregnant patients with DM, the prescription of oral hypoglycemic agents (OHA) has been increasing among practitioners. This study aimed to evaluate the maternal and neonatal outcomes when oral hypoglycemic agents were used in diabetic pregnant women.</p><p><strong>Methods: </strong>Medical records from the Maternal-Infant Care Unit Clinics SoM-UPR (n=149) were reviewed. Patients that were treated with metformin, sulfonylurea or insulin were included. Maternal and neonatal outcomes were compared between groups.</p><p><strong>Results: </strong>Patient's mean age was 28 ± 6 years. The majority had GDM (91%). The most common comorbidity was hypertension (9.9%). Lifestyle modification was used as treatment in 77% of patients during the second trimester, but its use decreased to 33% during the third trimester. Insulin was the treatment of choice. Among the OHA, sulfonylurea was preferred. Postprandial glucose levels were lower in patients who used insulin as compared to those without medications.</p><p><strong>Conclusion: </strong>No significant differences were found in maternal outcomes such as C-section, induction of labor, episiotomy or preterm labor, or neonatal outcomes such as macrosomia, neonatal hypoglycemia or congenital abnormalities among treatment groups. The 25-item Tinnitus Handicap Inventory (THI) and the Revised Fibromyalgia Impact Questionnaire (FIQR) were administered to 27 patients who fulfilled the revised diagnostic criteria for fibromyalgia of the American College of Rheumatology and to 26 healthy controls who had no history of suffering from fibromyalgia or any other rheumatological or neurological illness.</p><p><strong>Results: </strong>The two groups were matched for age, sex and ethnicity. The fibromyalgia patients were significantly more likely to suffer from tinnitus than the controls: 16 (59.3%) of the patients suffered from tinnitus, compared with two (7.7%) of the controls (p < 0.001). The median (interquartile range) total THI score for the fibromyalgia group was 6 (44), which was significantly higher than that of 0 (0) for the control group (p < 0.001). The fibromyalgia group scored significantly higher than the control group for 24 of the 25 THI items. {"title":"Assessment of Academic-Community Partnerships in Translational Research.","authors":"Irene Lafarga-Previdi,&nbsp;Carmen M Velez-Vega,&nbsp;Edda I Santiago-Rodriguez,&nbsp;Yolanda Lasalle","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>The Community Engagement Core (CEC) of the Center of Collaborative Research in Health Disparities focuses on developing and implementing strategies to increase academic-community collaborations and partnerships, enhance the recruitment and retention of study participants, disseminate research findings to a broader audience, and mitigate health disparities in Puerto Rico.</p><p><strong>Methods: </strong>In order to assess the current state of academic-community relationships and also collect ideas for their improvement, a strengths, weaknesses, opportunities, and threats (SWOT) analysis was conducted. Participants for the SWOT analysis activity were drawn from a pool of stakeholders at the University of Puerto Rico Medical Sciences Campus and a group of community representatives.</p><p><strong>Results: </strong>The areas identified by the group for the CEC to focus on were weaknesses such as 1) a lack of interaction and involvement with community leaders, 2) limited numbers of advocacy groups in several health areas, and 3) few research consortia. Opportunities identified included the possibilities of 1) creating alliances between academia and industry, municipalities, and community-based organizations, 2) advocating integration in research proposals, and 3) establishing a network of researchers and community leaders.</p><p><strong>Conclusion: </strong>The SWOT analysis activity served to foster relationships with diverse community stakeholders and select members for a community advisory board to collaborate in developing educational activities for our researchers and communities. The demographic and clinical characteristics were analyzed.</p><p><strong>Results: </strong>Twenty-two patients who met the diagnostic criteria for VKH were identified and included in the analysis. The median age at presentation was 41 years; 68.2% were female, and all patients were Hispanics. Bilateral disease was present in 90.9% of patients, and 59.1% of patients were categorized as having probable VKH. A headache was reported in 54.5% of patients and was the most common complaint at the time of presentation; the second most common complaint was tinnitus, which was present in 22.7% of patients. Vitiligo, alopecia, and meningismus were each preset in 9.1% of patients while hearing loss and aseptic meningitis were each reported in 4.5% of patients.
