{"title":"The impact of genital lichen sclerosus and lichen planus on quality of life: A review.","authors":"Alison Ranum, David R Pearson","doi":"10.1097/JW9.0000000000000042","DOIUrl":"https://doi.org/10.1097/JW9.0000000000000042","url":null,"abstract":"<p><strong>Background: </strong>Lichen sclerosus (LS) and lichen planus (LP) are inflammatory diseases that demonstrate genital and extra-genital manifestations. Genital involvement may result in pruritus, sexual dysfunction, pain, and irritation. It is intuitive that the severity of symptoms may result in functional impairment and emotional distress, leading to a reduction in quality of life (QoL).</p><p><strong>Objective: </strong>Investigate the current literature on the impact of genital LS and LP on QoL.</p><p><strong>Methods: </strong>A literature review was performed using PubMed and results were summarized. Articles published between 1994 and 2020 were screened and reviewed by both the authors.</p><p><strong>Results: </strong>These diseases have been assessed with dermatological assessments, such as the Skindex-29 and the Dermatology Life Quality Index, general health surveys such as the Short Form-12 questionnaire, and mental health instruments including the Beck Depression Inventory, and others; these instruments consistently demonstrate impaired QoL. It follows that treatment may reduce these symptoms. Interventions, such as photodynamic therapy, have demonstrated improvement in the depressive symptoms that impact QoL. Although both females and males experience a reduction in QoL related to these diseases, they experience the symptomatology differently. Through interviewing, male patients were more likely to report no symptoms compared with female patients, while female patients were also more likely to report worse QoL in the work-school domain of the Dermatology Life Quality Index as compared with male patients.</p><p><strong>Limitations: </strong>Few studies have addressed the effect of disease severity or treatment on QoL.</p><p><strong>Conclusions: </strong>Understanding how genital LS and LP contribute to reduced QoL for patients is critical for health care providers to better prioritize treatment strategies. {"title":"Refractory pruritic Fox-Fordyce disease successfully treated with botulinum toxin type A.","authors":"Meshal Mohammad Alhameedy, Muhammad Usman Tariq","doi":"10.1097/JW9.0000000000000039","DOIUrl":"https://doi.org/10.1097/JW9.0000000000000039","url":null,"abstract":"Editors, Fox-Fordyce disease (FFD) was first described in 1902 by Fox and Fordyce 1 in 2 patients with axillary disease. FFD is a chronic, pruritic, papular eruption involving apocrine-gland– rich areas. It most commonly affects women between the ages of 15 and 35 years with a female-to-male ratio up to 13:1. 2 A 30-year-old Saudi female presented to our dermatology clinic complaining of a 7-year history of intensely pruritic papules over both axillae. There was no hyperhidrosis or axillary chromhidrosis. There was neither fluctuation in pruritus during the day or before menstruation nor a his- tory of laser therapy in the affected areas. Over the last 7 years, multiple treatment modalities were tried, including topical and intralesional corticosteroid, topical tacrolimus 0.1% ointment, 1% clindamycin solution, topical tretinoin 0.05%, an oral antihistamine, topical aluminum chloride hexahydrate 20%, oral isotretinoin up to 1 mg/kg for 6 months, and multiple sessions of electrosurgery. There was no significant reduction either in pruritus intensity or in the number of papules. Her main complaint was intense pruritus, rated using the Visual Analogue Scale as being 10/10. The severity the patient’s pruritus significantly her daily quality of life.
