Method: This is a qualitative document analysis study. Data were collected between August and September 2021 from government websites and the Virtual Health Library database. Results: We analyzed resolutions 216/2004 up to the most recently published resolutions 502/2021, technical notes, and public policies for the older population published between 1994 and 2021, including people residing in long-term care facilities in Brazil. Our analysis indicated that, in order to operate the apparatus of protection and care, councils and Society need to fulfill their roles, aiming at the dignity and quality of life of residents of long-term care facilities. The discussion was conducted in light of the political framework and technical foundations that guide the operation of these facilities. Conclusion: This study highlights the relevance, value, and quality of the area of gerontology, specifically for reflecting on public policies and resolutions so that we have the necessary guidelines for elaborating policies for the long-term care and protection of the health of older adults, which includes a wide discussion on collective housing/lon-term care institutions.","PeriodicalId":52782,"journal":{"name":"Geriatrics Gerontology and Aging","volume":"1 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"71148779","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Methods: This is a cross-sectional study performed with older caregivers recruited in contexts of outpatient and home care. We collected information on sociodemographic characteristics, duration of caregiving, physical and cognitive function indicators of the older care recipients, perceived burden, family functioning, and psychological health measures (psychological need satisfaction and depressive symptoms). Results: We evaluated 133 caregivers (76% female, 69.5 ± 6.98 years). Variables that were significantly correlated with psychological health were selected to form an association model to be tested by structural equation modeling via path analysis. Depressive symptom variability was best explained by this model. Caregiver burden remained in the model as a mediator of indirect associations between physical function for instrumental activities of daily living and indicators of family functioning and psychological health. Three associative paths between caregiver burden and depressive symptoms were found — one of them was direct and the other two were mediated whether by family functioning or by the level of psychological need satisfaction. Conclusion: Depressive symptoms were the psychological health indicator best explained by the model involving instrumental functional demands that generate burden. Clinical consequences suggested by the model indicate interventions aimed at family functioning and opportunities of psychological need satisfaction as strategies for promoting caregivers’ psychological health.","PeriodicalId":52782,"journal":{"name":"Geriatrics Gerontology and Aging","volume":"1 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"71149218","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The Brazilian population is expected to increase until 2030, when it will reach its maximum, with a total of approximately 215 million inhabitants. A trend of population decline was already in progress and had already been documented, but the pandemic accelerated this process. This study describes a set of projections for the older Brazilian population. The projections were elaborated using the main components method, whose advantages are the possibility of separately projecting the behavior of the three demographic variables (fertility, mortality, and migrations) and obtaining results disaggregated by sex and age groups. Birth data for 2018, 2019, and 2020 suggest a 3.51 and 5.28% decrease in total births between 2018 and 2019 and 2019 and 2020, respectively. Preliminary data for 2021, which indicate the continuation of this trend between 2020–2021, show a 2.32% reduction in the number of births. The hypotheses raised for the mortality patterns, if proven to be accurate, suggest a life expectancy of 72.8 years for men and 76.2 years for women at the final period of the projection, resulting in gains of 4.6 and 2.0 years, respectively. Despite these gains, the levels obtained in 2019, pre-pandemic, would be reached by the male population only between 2035 and 2040.","PeriodicalId":52782,"journal":{"name":"Geriatrics Gerontology and Aging","volume":"1 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"71150729","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Motor impairment is mild, but behavioral and linguistic alterations stand out, in what is known as the cerebellar cognitive affective syndrome secondary to cerebellar atrophy. We report the case of an older woman with early-onset (age 45) signs and symptoms of this syndrome, including impairment of executive functions and visuospatial cognition, personality changes, and language deficits, who was followed at a geriatric medical center for 14 years. Neuropsychological, imaging, and behavioral aspects during this period are discussed in light of scientific evidence. This case report contributes to the scientific literature by describing the progression of the signs and symptoms of cerebellar atrophy over the years, which can help guide medical management and support advice for patients and their families. Methods: This is a retrospective, descriptive study performed using secondary data from the Brazilian Ministry of Health. Results: During the analyzed period, 14 145 686 older adults died in the country, of which 40.42% were over 80 years old. The main cause of death was circulatory system disease (21.50%), and the most frequent place of death was a hospital environment (68%). The Southeast region accounted for 52.83% of the country’s hospital deaths and 73.33% of those occurring in other health facilities, whereas 38.56% of the deaths that happened at home took place in the Northeast region. Conclusions: The hospital environment was the predominant place of death in all regions of the country, and the main causes of death were chronic noncommunicable diseases. Alternative care modalities emerge as a possibility of establishing accessible end-of-life care in scenarios other than the hospital.","PeriodicalId":52782,"journal":{"name":"Geriatrics Gerontology and Aging","volume":"1 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2022-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"71149458","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It focuses on issues that are of specific relevance to Brazilian policy-settings, including examples of its local implementation. The CIAT Framework combines and summarizes broad elements for an emergency strategy to address the potential effects of COVID-19. The 4 steps of the Framework entail policies to coordinate, identify, assess, and target support. Those policies can be applied immediately to mitigate the impact of the COVID-19 pandemic in LTCFs. It is, however, essential to situate these responses within a more comprehensive and permanent strategy. Intersectoral collaboration must evolve into a fully institutionalized system.","PeriodicalId":52782,"journal":{"name":"Geriatrics Gerontology and Aging","volume":"17 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2021-04-08","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"89589242","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

METHODS: This cross-sectional quantitative study was conducted in 2018/2019 with 492 older adults registered in Family Health Strategy units in the city of Três Lagoas, MS, Brazil. Sociodemographic data were collected and the Clinical-Functional Vulnerability Index (CFVI-20) was applied (possible score 0-40; higher scores indicate greater vulnerability). A multinomial logistic regression was performed to identify the risk factors for clinical-functional vulnerability. RESULTS: The sample’s mean age was 70.80 years (SD, 7.82) and the mean CFVI-20 score was 9.25 (SD, 7.09), with 17.07% at high risk of clinical-functional vulnerability, 38.82% at moderate risk, and 44.11% at low risk. Low education, social isolation, difficulty sleeping, and being female were risk factors for moderate vulnerability. On the other hand, low education, social isolation, difficulty sleeping, physical inactivity, being female, not using alcohol, and not participating in social groups were risk factors for high vulnerability. METHODS: This was a cross-sectional study of 608 older adults diagnosed with cancer. Age groups were considered an independent variable. For the analysis of HRQoL, the mean scores of age groups were compared by analysis of variance and the Scheffé comparison test. For measuring the association between age and HRQoL, we used simple and multiple linear regression analyses. RESULTS: Cognitive function showed the highest scores (average 87.94 ± 26.87), while physical function showed the lowest ones (68.04 ± 28.63). The highest symptom score was observed for financial difficulties (34.21 ± 39.06), followed by pain (29.47 ± 33.92) and insomnia (28.51 ± 37.03). METHODS: This exploratory research was conducted for 12 months, between August 2020 and July 2021, based on primarily publicly accessible data. First, the Unified Social Assistance System (Sistema Único de Assistência Social [SUAS]) database from 2019 was adopted as the primary source of information. In addition, public agencies and managers were consulted and invited to share their databases, while researchers and private entities collaborated by making their spreadsheets available. Data were organized in spreadsheets for each Brazilian state. LTCFs not catering to older adults (aged 60 years and over) were excluded. Duplicate data were excluded when overlaps were identified. RESULTS: This brief communication describes the methodology adopted for mapping the current status of Brazilian LTCFs. Despite its caveats, this study represents an important advance in the identification, characterization, and monitoring of these services nationwide. A total of 5769 facilities were found in the 2019 SUAS census. After excluding facilities not caring for residents aged 60 years or over, this number decreased to 2381 LTCFs. The consolidation and filtering of information from multiple data sources led to the identification of 7029 LTCFs throughout the country. CONCLUSION: Building a solid database was paramount to devising a national policy on long-term care. By including multiple sources, the scope of this survey was wider than all previous efforts and constituted an unprecedented collaborative experience in the country, including the potential to become the first national dataset for the Brazilian LTC sector.","PeriodicalId":52782,"journal":{"name":"Geriatrics Gerontology and Aging","volume":"1 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2021-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"71147738","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
