Giacomin","doi":"10.53886/gga.e0220035","DOIUrl":"https://doi.org/10.53886/gga.e0220035","url":null,"abstract":"To catalyze the discussion and implementation of state policies for an integrated continuum of long-term care (LTC), it is imperative to assemble strategic actions involving the public and private sectors, civil society, international agencies, professionals, academia, and the media, considering clear objectives for improving the lives of older adults, their families, and the communities where they live. Care may be provided at home, in the community, or at LTC facilities (LTCFs) for older adults. In this essay, we focused on institutional care. Tensions between advocates of different models of care for older people should include space for dialogue, convergence, and intersectoral actions, regardless of where LTC is provided. Conditions for LTCFs not to be perceived as the “last and undesirable alternative” should exist or be created so that these institutions are seen instead as welcoming, productive, and inclusive environments that are integrated to the community and its social, recreational, and health systems. The aim of this essay was to reflect on the urgency of developing an integrated continuum of LTC for older adults in Brazil that considers care as a right along with its modalities of delivery, flow, services, and activities, as well as sustainable financing alternatives and legal and governance directives. Methods: This is a retrospective cohort study based on a secondary analysis of provincial health administrative data in Quebec, Canada. We included community-dwelling older adults from Quebec who were considered frequent emergency department users (a minimum of 4 visits in the year following an index emergency department visit chosen randomly between January 1, 2012, and December 31, 2013) and who had been diagnosed with at least one chronic condition. We compared characteristics of frequent users living with or without major neurocognitive disorders using chi-square and Kruskal-Wallis tests. Results: The study cohort consisted of 21 393 frequent emergency department users, of which 3051 (14.26%) were identified as having a major neurocognitive disorder. The results highlight a higher burden of chronic conditions, polypharmacy, antipsychotic use, and past use of healthcare services among these individuals. The results also reveal a higher proportion of conditions associated with geriatric syndromes such as trauma and injury, malnutrition, orthostatic hypertension, and gait disorders. Conclusion: Frequent emergency department users living with major neurocognitive disorders represent a complex population. Each trajectory is personal and unique. It’s common to find oneself at a crossroads, with no clear choice or segue to the next step. At such times, we rely on advice from mentors and peers. From this viewpoint, I discuss seven directives I believe to be important for everyone involved in academic life, especially those in biomedicine and young professors or researchers in geriatrics and gerontology: (1) be empathetic, (2) be available, (3) be studious, (4) be different, (5) be communicative, internationally, (6) always be a mentor and a mentee, and (7) be a leader (not a boss) as soon as possible. METHODS: This cross-sectional, descriptive-analytical study was conducted with individuals ≥ 60 years of age based on a secondary analysis of public data from a nationally representative survey called the Family Budget Survey. The data were separated according to the 5 regions of the country and were associated with sociodemographic and economic characteristics. Analyses were performed of categorical and numerical variables and their associations using multinomial logistic regression. RESULTS: Among all participants in the Family Budget Survey, 26 199 (15%) were older adults. It was found that the southeast and south had better living conditions and higher proportions of older adults. Regarding race, the southern region had the highest proportion of self-declared Whites, the southeastern region had the highest mean education level, and the northern and northeastern regions had the lowest levels of education and household income. CONCLUSIONS: The results suggest that the aging process is heterogeneous due to marked regional inequalities, which are related to social issues. Methods: Postural evaluation of 32 older adults was performed using Postural Assessment Software (SAPO). Two raters performed the procedure independently, and one of the raters repeated the process 7 to 10 days later. Reliability was assessed through the intraclass correlation coefficient (ICC) and paired and independent t-tests. SEM and MDC95 values were calculated, and Bland-Altman plots were generated. Results: The inter-rater analysis showed that the raters were not different in any of the angles. Most angles achieved good to excellent inter-rater reliability (ICC = 0.34 – 0.81) and intrarater reliability (ICC = 0.33 – 0.88) and were not different between days. The SEM was lower than 10º in most angles. The MDC95 ranged from 2.88º to 18.49º. Bland-Altman analysis indicated that most angles were within the limits of agreement. Overall, angles generated in lateral view had better reliability properties. Conclusion: Postural Assessment Software (SAPO) showed angles with distinct reliability properties in older adults. Angles that reached good to excellent intra- and inter-reliability and exhibited lower variability could be considered for assessing postural changes in older adults. Social vulnerability refers to the absence of or difficulty obtaining social support from public institutions, situations that hinder the realization of or deny citizens their social rights and affect their social cohesion, and the ability to react to high-risk social situations – associated health and illness. In this study, we used the São Paulo Social Vulnerability Index developed by the SEADE Foundation, which classifies social vulnerability based on socioeconomic and demographic conditions. Methods: Quantitative analytical study of 303 older adults. Fried frailty phenotype assessment was performed and the Mini Mental State Examination, Geriatric Depression Scale, Katz Index of Independence in Activities of Daily Living, and the Lawton Scale of Instrumental Activities of Daily Living were administered. Descriptive statistics and logistic regression were used to analyze data. Results: Of the older adults, 12.21% were nonfrail, 60.72% were prefrail, and 27.06% were frail. The single factor most associated with frailty was depressive symptoms (OR = 2.65; 95%CI 1.38 – 5.08) in the younger-old and illiteracy (OR = 14.64; 95%CI 1.82 – 116.51) in the oldest old. Conclusion: The factor most associated with frailty in younger-old adults (aged 60 to 74 years) was depressive symptoms, whereas in the oldest old (aged 75 or older), the factor most associated with frailty was being illiterate. Methods: This is a non-randomized controlled clinical trial. Forty-three community-dwelling older women, who were allocated to the Control Group (CG; n = 20) or the Nutrition Group (NG; n = 23). The NG received individualized nutrition assistance and participated in group activities that focused on dietary protein intake once a week for 12 weeks. Main outcome measures: protein and energy intake; handgrip strength (HGS); gait speed (GS); quadriceps cross-sectional area (CSA); and intramuscular non-contractile tissue (IMNCT). The Student’s t-test for independent samples, the Mann-Whitney U test, and a mixed model ANOVA with two factors (group and time) were adopted, followed by a post hoc Bonferroni test. A Spearman’s correlation test was performed on tests for HGS; GS; CSA; IMNCT; age; and protein intake adjusted for weight (g/kg) (p ≤ 0.050). Results: Participants in the NG showed higher CSA values than those in the CG (p < 0.001). NG participants also had higher HGS (p < 0.001) and GS (p = 0.037) when compared to pre-intervention. Correlations were observed between IMNCT and protein intake adjusted for weight (g/kg) (r = -0.517; p = 0.020); HGS, and CSA (r = 0.827; p = 0.000); and CSA and age (r = -0.520, p = 0.009). Methods: The sample included 50 older adults and a control group of 50 younger adults. The variables were initially analyzed for both feet together, and were later analyzed separately, comparing each foot (right and left) between groups. Range of motion was assessed by validated goniometric procedures: the position of subtalar axis was evaluated by the palpation technique, while the Foot Posture Index was used to assess behavior in a closed kinetic chain. Student’s t-test / Mann-Whitney test compared the main variables according to sample distribution, while Student’s t-test / Wilcoxon test was used for paired samples. A standardized Haberman residuals test was also used to determine the connection between the position of subtalar joint axis and the Foot Posture Index. Results: Data from the right and left feet were similar for all variables. The older group had reduced mobility in the ankle and first metatarsophalangeal joint (5.42º [SD (Standard Deviation), 4.49] and 76.12º [SD, 19.24], respectively) with statistically significant values, (p < 0.001). The difference in subtalar axis position was not significant (p = 0.788), with more internal deviations in both groups. There was a significant difference in Foot Posture Index (p = 0.006, by applying the chi-square test), with the normal position more prevalent in the older group and the prone position more prevalent in the younger group. Conclusions: Regarding internal deviations in the subtalar joint axis, the older group had a higher frequency of feet in the normal position, while the younger group had a higher frequency of feet in the prone position which, in this case, agrees with the rotational balance theory. For the normal axis position, a higher frequency of normal position was found in both groups. Regarding external deviations of the subtalar joint axis, neither group followed the pattern expected in rotational balance theory. Bauer","doi":"10.53886/gga.e0220004","DOIUrl":"https://doi.org/10.53886/gga.e0220004","url":null,"abstract":"Objective: The goal of this study is to evaluate the benefits of an increase in water intake guided by a mathematical formula (per kg of body weight) on kidney function in older adults. Methods: Older adults (≥ 65 years old) cared for at the Internal Medicine Unit of a tertiary hospital will be randomized to receive or not guidance on water intake (30 mL/kg per day) after initial assessment of kidney function. After 14 days, participants will be reevaluated through clinical and laboratory examinations. Patients with uncompensated disease will be excluded. The main outcomes will be glomerular filtration rate and laboratory measures such as serum and urinary osmolality, sodium, urea, 24-h urine volume and serum creatinine, uric acid, and copeptin. The Mini Nutritional Assessment (MNA) questionnaire will be applied to participants at each visit. Categorical variables will be described as numbers of cases (%) and compared using the χ2 test whereas continuous variables will be analyzed with Student’s t-test in relation to baseline measures. The Generalized Estimating Equations (GEE) method will be performed to assess differences over time and between groups. This study was approved by the Institution’s Research Ethics Committee (grant number 16-0153) and is in accordance with the Declaration of Helsinki. Expected Results:By increasing water intake (ml/Kg) we expect to provide an improvement in kidney function in older population assessed by serum creatinine and cystatin-c applied to eGFR formulas. Relevance:Many conditions, both organic and behavioral, can contribute to chronic dehydration states in older adults. To mention, decreased ability to concentrate urine, reduced kidney mass, blood flow, and glomerular filtration rate (GFR) along with changes in sensitivity to hormones such as renin, vasopressin and natriuretic peptide can generate water imbalance, leading to dehydration. Methods: This descriptive study analyzed secondary data from older adults interviewed for the 2018 Surveillance System for Risk and Protective Factors for Chronic Diseases by Telephone Survey. Results: Of 53395 adults who completed the survey, 20981 were aged ≥60 years (6468 men and 14 513 women). Walking was the most prevalent physical activity. Most older adults engaged in physical activity 3-4 days per week for 30 minutes or longer. Sessions lasting more than 60 minutes were reported by 54.33% of participants. The rate of older people engaging in leisure time physical activity decreased with age and low education. Older adults engaging in physical activity were more optimistic about perceived health status and were less likely to be smokers than their inactive peers.
