AIM: In this study, our aim is to share with the literature through anticholinergic food poisoning cases that it is possible to prevent delays in examination and treatment and to prevent information pollution when 112 emergency health services. MATERIAL AND METHODS: We retrospectively reviewed 213 mass food poisoning cases which occurred between December 1 and 7, 2019 and which were taken to 14 hospitals with 112 emergency health services. File information of 69 of these cases were reached; in addition to demographic characteristics such as age and gender, physical examination findings such as blood pressure, pulse, skin-eye and consciousness states, laboratory values, hospitalization times, in which units the cases were followed and the duration of admission to the hospital after meals were noted. RESULTS: Of the 69 cases, 42 were found to have skin findings, 37 were found to have mydriasis and 12 were found to have agitation and/or confusion. The duration of admission to hospital after meal was found as 6.5 (4.5–10.0) hours. 60 (87.0%) of the patients were followed in the emergency room, 4 (5.8%) were followed in the internal medicine service and 5 (7.2%) were followed in the intensive care. CONCLUSIONS: When the crisis team of 112 emergency health services is alert, they can categorize the event, prevent the chaos in the emergency service of hospitals with correct triage and thus with the process of understanding the event, examination and treatment process becomes more executable.","PeriodicalId":52339,"journal":{"name":"Disaster and Emergency Medicine Journal","volume":null,"pages":null},"PeriodicalIF":0.0,"publicationDate":"2021-07-14","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"46162485","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Our study was conducted to assess the levels of homophobia among emergency medicine specialists, practitioners, nurses, and other health care professionals working in the emergency department (ED). MATERIAL AND METHODS: This study was carried out as a descriptive survey study in Gaziantep, Turkey, between July 7, 2018 and August 30, 2018. The data was collected using a Google survey form link sent to volunteers via mobile phone. RESULTS: The mean age of the patients was 30.85 ± 7 (20–53) years. While 77.5% (n = 117) of the volunteers who participated in the study worked in state hospitals, 11.3% (n = 17) worked in a private hospital. The Cronbach’s Alpha value was calculated as 92.3 and the scale average score as 85.42 ± 12.33 as a result of the evaluation of the Likert-type scale. While 69 (45.7%) people were lower in homophobia, 82 (54.3%) were higher in homophobia. A difference in attitudes towards homosexuals was only observed between those who had a homosexual friend or acquaintance and those who did not (p = 0.009). Accordingly, those with homosexual friends were determined to be less homophobic. On the other hand, it was observed that those with a homosexual acquaintance and those with no homosexual acquaintances were more homophobic. CONCLUSION: In our study, it was shown that emergency medicine specialists and other health professionals working in the ED exhibit high levels of homophobic attitudes and behaviors. Including lesbian, gay, bisexual, transgender, intersex (LGBT-I) health problems in in-service trainings may contribute to the improvement of homophobic attitudes.","PeriodicalId":52339,"journal":{"name":"Disaster and Emergency Medicine Journal","volume":null,"pages":null},"PeriodicalIF":0.0,"publicationDate":"2021-07-12","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"49564583","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Although the Recognition of Stroke in the Emergency Room (ROSIER) score used for this purpose is indicated better than the other scoring systems, it is expressed that it will be able to have social differentiations. In this study, we targeted to research validation of the ROSIER scale and to compare its efficiency with Face-Arm-Speech Test (FAST), another stroke diagnosis method, for the patients who applied to the emergency department with the stroke or transient ischemic attack symptoms. MATERIAL AND METHODS: The patients who reported to the emergency department with the suggestive symptoms or findings of stroke and were above 18 years of age were included in the study. The study forms were filled out by the emergency medicine specialist or the senior emergency medical assistant after the patients were evaluated, and then they were consulted by the neurology specialist. The final diagnosis, which was established after the clinical evaluation and necessary imaging done by the neurology specialists, was accepted as a standard reference. RESULTS: A total of 335 patients, including 168 (50.1%) females, were included in the study. The sensitivity was 68.5%, specificity was 79.0%, Positive Predictive Value (PPV) was 78.7%, NPV (Negative Predictive Value) was 68.9%, and test validity was 73.4% for the ROSIER scale. For the FAST scale the sensitivity was 63.5%, specificity was 88.5%, PPV was 86.3%, NPV was 68.1%, and test validity was 75.2%. CONCLUSION: In the present study, it was seen that the ROSIER scale could be used in separating the patients with CVO (cerebrovascular accident) from the patients who applied with the similar clinical findings. However, FAST was superior because its specificity and PPV were higher and its practicability was easier than the ROSIER.","PeriodicalId":52339,"journal":{"name":"Disaster and Emergency Medicine Journal","volume":null,"pages":null},"PeriodicalIF":0.0,"publicationDate":"2021-07-12","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"48101672","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

In addition, a comparison of the efficiency of the brain CT examinations performed for trauma and non-traumatic reasons was made. MATERIAL AND METHODS: CT’s were examined by dividing into 2 groups according to the indications as trauma (group 1) and non-trauma related (group 2). The 2 groups were compared statistically according to the number of pathologies, distribution of pathologies by gender and age. RESULTS: Pathologies were detected in 9.3% (n = 30) of the patients in the first group and 21.2% (n = 14) of the patients in the second group. A statistically significant difference was found between the groups in terms of whether pathology was detected (p = 0.023). The rate of pathology detection in the group that underwent CT for non-traumatic reasons was statistically significantly higher than the other group. CONCLUSION: Precautions should be taken especially with trauma patients to prevent unnecessary CT scans in the paediatric emergency department. In addition, if a CT scan is planned in the paediatric emergency department with the approval of the paediatrician and radiologist, the CT examinations can be made with more accurate indications.","PeriodicalId":52339,"journal":{"name":"Disaster and Emergency Medicine Journal","volume":null,"pages":null},"PeriodicalIF":0.0,"publicationDate":"2021-06-28","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"48819322","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

However, the high cost is the biggest constraint especially in countries with limited resources. To lower the cost, a custom-made VL obtained by attaching a USB-endoscope camera ($8.5) to the Macintosh laryngoscope (USB-L) can be used. MATERIAL AND METHODS: All intubations were performed in a difficult intubation model. Intubations were carried out by two emergency physicians. A Glidescope as a VL and a custom-made USB-L were used. In addition to these devices, one bougie to facilitate the advancement of the tube was used. The total intubation time was evaluated. RESULTS: Correct tube placement for both operators was 100% for both devices. A difference between the operators in the duration of intubations could not be found. Also, there is no difference in the duration of intubations between the devices for both operators. CONCLUSION: It was concluded that the USB-L and VL are not statistically different in terms of intubation time in the difficult intubation model using bougie. For countries with limited resources, the low-cost USB-L has come to the forefront due to the high cost of VL and difficulty of access.","PeriodicalId":52339,"journal":{"name":"Disaster and Emergency Medicine Journal","volume":null,"pages":null},"PeriodicalIF":0.0,"publicationDate":"2021-06-08","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"41928874","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

While hemorrhagic stroke often necessitates immediate neurosurgical intervention, ischemic stroke is treated with reperfusion therapies such as thrombolysis with intravenous recombinant tissue plasminogen activator (IV rtPA) and early endovascular thrombectomy for broad vessel occlusions. OBJECTIVES: Early diagnoses, accurate emergency medical services (EMS) dispatch, rapid EMS transfer, and stroke team activation have helped reduce door-to-IV tPA time and continue to be critical in saving time for stroke patients’ treatment. MATERIAL AND METHODS: One reason for prehospital delays may be incorrect qualification by emergency team members due to incomplete medical records and incorrect evaluation of symptoms by dispatchers or paramedics. The dispatcher’s precise identification of the report helps them decide on the patient’s priority disposal of the ambulance. In comparison, a correct initial diagnosis by paramedics allows the patient to be transported immediately to the destination hospital, i.e., the unit with a stroke unit. Extending the time it takes for the patient to enter the stroke facility due to the patient being moved through stages reduces the probability of successful treatment being introduced significantly. RESULTS: We hypothesized that paramedics’ knowledge of prehospital stroke management protocols would be linked to their clinical experience as well as their stroke preparation. CONCLUSION: A secondary goal of this study was to evaluate and compare the theoretical knowledge on stroke management among paramedics and identify factors associated with high knowledge. 468 EMS providers agreed to complete a questionnaire that included demographic questions, practical experience questions, and 14 theoretical information questions. Our research found that paramedics in Poland have significant awareness gaps in existing stroke treatment guidelines.","PeriodicalId":52339,"journal":{"name":"Disaster and Emergency Medicine Journal","volume":null,"pages":null},"PeriodicalIF":0.0,"publicationDate":"2021-05-31","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"47230808","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

An ACS diagnosis can be made with electrocardiogram (ECG) and cardiac markers. However, despite medical advances, 2–5% of ACS patients are undiagnosed and discharged from emergency departments (EDs) because clinicians often find it difficult not only to diagnose and treat high-risk patients but also to define nonemergency diseases or safely discharge healthy patients. Risk stratification can be prevented, and inappropriate diagnosis and treatment protocols can be identified. The ED Assessment of Chest Pain Score-Accelerated Diagnostic Protocol (EDACS-ADP) scoring system, developed to identify patients with chest pain but at low risk for a major adverse cardiac event (MACE), is the first score based on clinical data from emergency medicine. OBJECTIVES: This study investigates the usability of EDACS-ADP in Turkey. MATERIALS AND METHODS: This is a prospective observational study of 392 patients. The primary outcome was a major adverse cardiovascular event (MACE) within thirty days. RESULTS: A total of 116 MACEs occurred in 65 (16,6%) patients during a one-month follow-up. The sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), positive likelihood ratio (+ LR), and negative likelihood ratio (–LR) values of the EDACS-ADP score for the evaluation of 30-day MACE rate in patients who admitted with chest pain for two months were as follows: 96.9%, 64.5%, 35.2%, 99.1%, + LR: 2.73, and –LR: 0.05. CONCLUSION: Most of these patients were classified by the EDACS-ADP as low risk and suitable for discharge.
