This was followed by dyspnoea and refractory type I respiratory failure. The investigations reveal raised C-reactive protein and D-dimer levels concordant with MIS. • The computed tomography revealed bilateral ground glass opacities with an apparent cardiomegaly in Figure 1 which shows dramatic recovery in both these findings in Figure 2. An underlying myocarditis only can explain such a rapid and complete reversal. There is no mention of any electrocardiogram and echocardiograph done in the patient which may have helped to narrow down the diagnosis. The patient was also extubated within 2 days of mechanical ventilation, which is unlikely for any interstitial lung disease. • The diagnosis of MIS-V is difficult and often relies on the existing diagnostic criteria for MIS in adults (MIS-A). Dysregulated hyperimmune response has been postulated as a probable underlying mechanism. Cardiac involvement along with mucocutaneous involvement are most commonly involved organs, besides gastrointestinal, musculo-skeletal, pulmonary and renal. The mainstay of treatment is steroids which leads to a rapid response. The patient had myalgia and respiratory failure for which musculoskeletal and cardiac evaluation is essential. Even if there was isolated myocarditis, which has increasingly been reported with messenger RNA vaccines throughout the world, the treatment essentially remains steroids only.","PeriodicalId":520760,"journal":{"name":"QJM : monthly journal of the Association of Physicians","volume":" ","pages":"697"},"PeriodicalIF":13.3,"publicationDate":"2022-10-25","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC8690108/pdf/hcab292.pdf","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"39751235","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

We compared BMD in participants with and without cancer which was further analyzed according to cancer type. We conducted logistic regression to evaluate adjusted odds ratios of osteopenia and osteoporosis and determine risk factors for their development.

Results: We found that BMD was significantly higher in participants without cancer than cancer patients. Furthermore, the median BMD of patients with breast cancer or skin cancer (including melanoma) was significantly lower than participants without cancer. People with breast, lung, genitourinary and skin cancers were more likely to incur osteopenia/osteoporosis than those without cancer.

Conclusions: BMD differs depending upon type in survivors. They reported that there is no study, so far, demonstrating tangible benefits of steroid use in mild cases, while future well-designed clinical trials can further prove the merit of corticosteroids in mild cases of COVID-19. There was skepticism regarding the potential risk of viral replication increase, the side-effects as hyperglycemia, risk of secondary infections or reactivation of latent infections and possible steroid shortage in pharmacies and hospitals. However, a previous published meta-analysis and systematic review 2 demonstrated that in the absence of contra-indications and/or side-effects, the use of ste-roids should be considered in coronavirus infection including COVID-19. Interestingly, clinical data have shown that asthma and chronic obstructive pulmonary disease exacerbations, often attributed to viral cause, can be reduced with the use of inhaled corticosteroids, while inhaled glucocorticoids can further reduce the severe acute respiratory syndrome coronavirus 2 replication in airway epithelial cells and down regulate the ex-pression of angiotensin converting enzyme 2 and transmem-brane","PeriodicalId":520760,"journal":{"name":"QJM : monthly journal of the Association of Physicians","volume":" ","pages":"701"},"PeriodicalIF":13.3,"publicationDate":"2022-10-25","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC9383071/pdf/hcab312.pdf","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"39577282","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

We continue to contribute to the expanding literature on this important subject and Wellcome the paper by Dr. Li and colleagues from Soochow University, China on identifying factors associated with enhanced societal vulnerability to depression in a world coming to terms with living with COVID-19. It is well recognized historically that major economic upheaval or natural disasters are often accompanied by an enhanced societal vulnerability to significant mental health problems including depression, anxiety, post-traumatic stress disorder, substance abuse and suicide. In the context of living with COVID, the authors recruited just over 2000 residents and found an overall prevalence of clinically significant depression of 27%. Factors associated with vulnerability to depression included rural younger females, higher body mass index within a lower socioeconomic group. We recommend this article to our readers.","PeriodicalId":520760,"journal":{"name":"QJM : monthly journal of the Association of Physicians","volume":" ","pages":"573"},"PeriodicalIF":13.3,"publicationDate":"2022-09-22","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC9619419/pdf/hcac207.pdf","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"33468657","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
