Higher initial doses than the standard starting dose of 1 μg/kg may have the potential to lead to faster platelet responses. We conducted a single-center, retrospective, observational study to evaluate the efficacy of romiplostim dosing strategies in adult patients with primary ITP who initiated romiplostim in the acute care setting. Patients were classified into low-dose (<3 μg/kg) or high-dose (≥3 μg/kg) based on the initial weight-based dose administered. The primary outcome was time to achieve a platelet count greater than 30 × 10 9 /l. Secondary objectives included time to platelet count greater than 50 × 10 9 /l, healthcare resource utilization, thrombotic events, and bleeding incidents. Sixty-five patients were included in the study. Patients in both dosing cohorts achieved a platelet response of greater than 30 × 10 9 /l at a median of 7 days ( P = 0.871). There were no significant differences in healthcare resource utilization or safety events between dosing cohorts. This study aimed to evaluate in vitro the antioxidant and antithrombotic properties of Tetraclinis articulata aqueous extract. Leaves were collected in Oujda, dried, crushed, and extracted by aqueous infusion. Total phenolic content was measured using the Folin-Ciocalteu method. Antioxidant activity was assessed via DPPH and ABTS assays. Antithrombotic activity was evaluated using blood from healthy donors. Platelet aggregation was measured with optical aggregometry using thrombin as the agonist. Anticoagulant effects were assessed through prothrombin time (PT), activated partial thromboplastin time (aPTT), and fibrinogen concentration. Fibrinolytic activity was determined by a clot-lysis assay. The extract contained 70.55 μg GAE/mg of phenolics and showed strong antioxidant activity (IC50 = 20.26 ± 0.54 μg/ml for DPPH and 8.56 ± 0.93 μg/ml for ABTS). It inhibited platelet aggregation in a dose-dependent manner: 35.2% at 0.5 mg/mL, 64.1% at 1 mg/ml, and 92.18% at 2 mg/ml. The extract significantly prolonged PT and aPTT and reduced fibrinogen levels. Clot lysis reached 34.6% at 1 mg/ml. Tetraclinis articulata exhibited potent antioxidant, antiaggregant, anticoagulant, and fibrinolytic activities in vitro . These findings support its potential as a natural antithrombotic agent. However, its strong antiaggregant effect warrants caution regarding potential adverse interactions when used alongside conventional antithrombotic treatments.</p>","PeriodicalId":520571,"journal":{"name":"Blood coagulation & fibrinolysis : an international journal in haemostasis and thrombosis","volume":" ","pages":"7-14"},"PeriodicalIF":1.1,"publicationDate":"2026-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"145484522","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

In clinical settings, assessing numerous genetic factors can lead to diagnostic errors and unnecessary treatments. This meta-analysis examines gene variants associated with thrombosis in the Iranian population, where their role in thrombotic disorders remains underexplored. A systematic literature search was performed across PubMed, Scopus, and Web of Science, targeting case-control studies published up to July 2025. Studies were included if they evaluated thrombophilia-related polymorphisms in Iranian patients with various thrombotic conditions, such as recurrent pregnancy loss (RPL), venous thromboembolism (VTE), or deep vein thrombosis (DVT). Advanced statistical analyses, including random-effects models, fixed-effects models, and Bayesian meta-analysis, were used to compute odds ratios (ORs) and 95% confidence intervals (CIs). From 36 studies encompassing over 14 000 participants, significant associations emerged. For RPL, FVL G1691A heterozygote (OR: 1.998, 95% CI: 1.02-3.88), methylenetetrahydrofolate reductase (MTHFR) C677T heterozygote (OR: 1.77, 95% CI: 1.31-2.39), MTHFR A1298C heterozygote (OR: 3.10, 95% CI: 1.33-7.20) and homozygote (OR: 1.69, 95% CI: 1.05-2.70), prothrombin G20210A heterozygote (OR: 2.435, 95% CI: 1.09-5.39) and homozygote (OR: 0.487, 95% CI: 0.40-0.58), plasminogen activator inhibitor-1 (PAI-1) polymorphisms, factor V (FV) A4070G, FV 5279A/G, factor XIII (FXIII) Val34Leu, and integrin subunit beta-3 (ITGB3)1565T/C were linked to elevated RPL risk. Additionally, FVL G1691A heterozygote (OR: 5.25, 95% CI: 2.39-11.54) was associated with higher VTE risk, while MTHFR C677T heterozygote (OR: 1.404, 95% CI: 1.030-1.914) increased DVT risk. Bacterial sepsis is a well established risk factor for venous thrombosis, and numerous experimental studies have shown that sepsis indeed enhances thrombotic responses. In this study, we aimed to investigate the impact of endotoxemia - induced by either lipopolysaccharides or α-toxin - on venous thrombosis development in mice. Venous thrombosis was induced using a model based on siRNA-mediated transient inhibition of the natural anticoagulants - protein C (PC) and anti-thrombin (AT). Unexpectedly, endotoxemia attenuated rather than promoted venous thrombus formation. This counterintuitive finding appears to be explained by a transient increase in circulating protein C levels following endotoxemia. As our venous thrombosis mouse model strongly depends on the level of reduced protein C activity, this endotoxemia-induced elevation interfered with the intended prothrombotic conditions and compromised comparability between experimental groups. These results highlight the context-dependent effects of bacterial sepsis on venous thrombosis and underscore the importance of rigorous model validation in (immuno)thrombosis research.</p>","PeriodicalId":520571,"journal":{"name":"Blood coagulation & fibrinolysis : an international journal in haemostasis and thrombosis","volume":" ","pages":"381-385"},"PeriodicalIF":1.1,"publicationDate":"2025-12-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"145351251","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
