Intraductal protrusion is uncommon and can mimic cystic intraductal papillary neoplasm of the bile duct (IPNB). A 39-year-old woman undergoing fertility treatment presented with recurrent hyperbilirubinemia and predominantly cholestatic liver test abnormalities. MRI demonstrated a 10.5-cm multilocular cystic lesion in segment 4 of the liver with downstream biliary dilatation, and internal septa were visible within the dilated bile duct on T2-weighted single-shot fast spin-echo (SSFSE) images. Contrast-enhanced computed tomography (CT) revealed enhanced cyst wall and septa, while endoscopic retrograde cholangiography revealed a smooth, oval-shaped filling defect. Endoscopic and intraductal ultrasonography indicated septate cystic components of the bile duct, and biopsy revealed mucin-producing columnar epithelium overlying the ovarian-like stroma, confirming MCN. The patient was treated via extended left hepatectomy with extrahepatic bile duct resection and reconstruction. Pathological examination revealed low-grade intraepithelial neoplasia. The patient remained recurrence-free for 20 months. This case suggests that, although biliary communication usually favors cystic IPNB over MCN-L, identification of intraductal extension of the cyst wall or septa within the dilated bile duct may provide an important clue to the diagnosis of MCN-L.
","PeriodicalId":520541,"journal":{"name":"Abdominal radiology (New York)","volume":" ","pages":"5989-5992"},"PeriodicalIF":2.2,"publicationDate":"2026-11-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"147794777","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Current risk stratification relies on invasive endoscopy or systemic surrogates of liver cirrhosis. This study aimed to develop a machine learning model utilizing EV-specific CT radiomics to identify high-risk esophageal varices, as defined by endoscopic criteria, in patients with HCC.

Methods: 151 HCC patients were enrolled and divided into a training cohort (n = 101; 75 high-risk; January 2017 - December 2022) and an independent temporal validation cohort (n = 50; 29 high-risk; January 2023 - December 2024). Endoscopic stigmata were used as an endpoint, including active bleeding or varices with high-risk features. Radiomic features were extracted from contrast-enhanced CT images acquired within six months prior to endoscopy. Model performance was evaluated using the area under the curve (AUC) of receiver operating characteristic (ROC), Brier score, calibration curves, and decision curve analysis (DCA).

Results: In the training cohort, the varix-specific radiomics signature developed via the least absolute shrinkage and selection operator (LASSO) demonstrated excellent discriminative power (cross-validated AUC: 0.956; 95% CI 0.872-1.000). In the temporal validation cohort, this model maintained high generalizability, yielding an AUC of 0.926 (95% CI 0.846-0.984, sensitivity: 93.1%, specificity: 71.4%) as well as a Brier score of 0.1186 and good calibration (intercept = -0.157, slope = 1.354). Surface-to-Volume Ratio (SVR) and NGTDM Coarseness are feasible radiomics.

Conclusion: The varix-specific radiomics model provides highly sensitive and mathematically stable non-invasive risk stratification for EV bleeding in patients with HCC.
","PeriodicalId":520541,"journal":{"name":"Abdominal radiology (New York)","volume":" ","pages":""},"PeriodicalIF":2.2,"publicationDate":"2026-10-20","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"149229795","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Five residents became unavailable before report generation and provided no outcome data. The remaining 15 readers completed the same 22 pelvic MRI examinations, generating 330 reports: 154 free-text and 176 structured reports. Reports were compared with an imaging reference standard established by two pelvic radiologists through independent review and consensus adjudication. The primary endpoint was clinically weighted completeness. Secondary endpoints included unweighted completeness, explicit negative discrepancies relative to the expert imaging reference, positive findings unsupported by the expert imaging reference, and reporting time. Objective outcomes were analyzed using mixed-effects models or Poisson generalized estimating equations. In an exploratory paired assessment, two expert gynecologic surgeons evaluated 44 reports, comprising one free-text and one structured report for each MRI examination.

Results: Structured reporting was associated with an 8.1%-point increase in completeness compared with free-text reporting (95% CI, 4.1-12.1; p < 0.001) and a 5.6%-point increase in clinically weighted completeness (95% CI, 1.3-10.0; p = 0.011). Reporting time was 4.52 min longer with structured reporting (95% CI, 3.05-5.99; p < 0.001). Structured reporting was also associated with more explicit negative discrepancies relative to the expert imaging reference (rate ratio, 2.94; 95% CI, 1.73-4.98; p < 0.001). These discrepancies were mainly explicit compartment-based negative statements. In the exploratory surgeon assessment, structured reports received higher ratings for perceived completeness, usefulness for clinical decision-making, and adequacy to clinical needs, whereas clarity ratings were similar between formats.

Conclusion: Structured MRI reporting was associated with higher objective completeness and, in an exploratory assessment, higher surgeon ratings for several dimensions of perceived clinical utility. These findings should be balanced against longer reporting time and more explicit negative discrepancies relative to the expert imaging reference.
","PeriodicalId":520541,"journal":{"name":"Abdominal radiology (New York)","volume":" ","pages":""},"PeriodicalIF":2.2,"publicationDate":"2026-10-17","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"149217952","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
