To connect the dots, it is interesting that in this issue of Polish Archives of Internal Medicine (Pol Arch Intern Med), Ruszkowski et al8 looked at the associ‐ ation between constipation symptoms and sleep quality among patients with nondialysis CKD. In this cross ‐sectional study, 100 patients with CKD in the outpatient setting filled question‐ naires addressing sleep quality and disorders. Pa‐ tients were divided into 3 groups according to es‐ timated glomerular filtration rate (eGFR) terciles: with low eGFR (≤32 ml/min/1.73 m2), medium eGFR (33–43 ml/min/1.73 m2), and high eGFR (≥44 ml/min/1.73 m2). Patients were asked to re‐ call and fill the Medical Outcomes Study (MOS) 12 ‐Item Sleep Scale ‐Revised (MOS ‐Sleep ‐R). The MOS ‐Sleep ‐R questionnaire consists of 12 items that measure 6 dimensions of sleep with a 4 ‐week recall period: sleep disturbance (4 items), daytime somnolence (3 items), sleep adequacy (2 items), snoring (1 item), awakening due to shortness of breath / headache (1 item), and sleep quantity (1 item). For the assessment of the con‐ stipation symptoms, the scores of the Patient As‐ sessment of Constipation‐Symptoms (PAC ‐SYM) It is estimated that 14% to 57% of patients with chronic kidney disease (CKD) experience poor sleep quality.1 Poor sleep quality among patients with CKD has been associated with various health‐ ‐related outcomes including increased risk of obe‐ sity, diabetes, cardiovascular disease, and mortal‐ ity.2 Also, poor sleep quality and excessive or in‐ adequate sleep have been shown to be associated with rapid progression of CKD to end ‐stage kid‐ ney disease.2 The exact mechanism of poor sleep quality among patients with CKD is not known; however, the one proposed mechanism is sym‐ pathovagal imbalance due to the impairment of the baroreceptor reflex function in which the sym‐ pathetic nervous system is hyperactive, leading to a decreased vagal tone.3 Different studies have looked at the intervention to improve sleep quali‐ ty in this population. Poland introduced extensive antiepidemic measures relatively early in order to slow down the spread of the disease. OBJECTIVES The study aims to present recent data on COVID‑19 hospitalizations during the first months of the disease outbreak in Poland. PATIENTS AND METHODS This is a retrospective, population‑based study conducted using hospital discharge records that included a diagnosis of COVID-19. Data were obtained from the National Institute of Public Health, where they had been originally collected for a Polish hospital morbidity study. They included 8840 hospitalization records from the period between February and September 2020. RESULTS Overall, there were 8252 records of patients hospitalized for COVID‑19 for the first time, which accounted for 93% of all hospitalizations. The study group consisted of 4161 men (50.4%) and 4091 women (49.6%). Significantly more patients from urban than rural areas were hospitalized (21.8 per 100 000 vs 20.5 per 100 000; P <0.02). In the period subject to analysis, 1073 in‑hospital deaths were observed (13% of all patients), 965 of which occurred during first‑time hospitalizations (11.7% of all patients). In the study group, patients who died during hospitalization compared with those who survived hospitalization were significantly older, more frequently lived in urban areas, and had more comorbidities. In the Polish arm of the HAPIEE (Health, Alco‐ hol and Psychosocial factors In Eastern Europe) study8 conducted in the urban area of Kraków be‐ tween 2002 and 2005, 8821 participants aged 45 to 69 years were included. According to the Inter‐ national Diabetes Federation (IDF) definition, the overall crude prevalence of MS was 27.9% (29.5% in women and 26.2% in men). A study conducted in primary health care units of Kraków9 between 2004 and 2005, which in‐ cluded 40 989 participants aged over 25 years, found a crude MS prevalence of 19.4% (20.9% in women and 16.2% in men), according to the ATP III criteria. In the PURE Poland study10 conducted between 2007 and 2009, which included 1634 inhabitants of Lower Silesia aged 35 to 70 years, a crude MS prevalence of 44.1% (41.8% in women and 48.2% in men) was found, based on the criteria close to those used in the WOBASZ II study. According to a study published in 2010,11 in‐ volving 1648 participants aged 25 to 85 years from a local community in Lower Silesia, MS prev‐ alence was 43.9% and 49.9%, in women and men, respectively, based on the IDF criteria (39% and 38.4%, respectively, by the ATP III criteria). In a preliminary report of the PONS study (Polish Norwegian Study),12 which took place in the Kieleckie region between 2010 and 2012 and included 3845 participants aged between 45 and 64 years, a crude MS prevalence of 39.5% (34.3% in women and 49.9% in men) was found by the JIS criteria. In another sample of 7997 participants of the same study, aged between 37 and 66 years, an overall crude MS prevalence of 39.9% was found.13 According to the WOBASZ II study, the median values of WC and blood pressure (BP) as well as The article by Rajca et al1 published in the current issue of Polish Archives of Internal Medicine (Pol Arch Intern Med) estimated the prevalence of met‐ abolic syndrome (MS) in Poland in the years 2013 to 2014, based on the results of the WOBASZ II study. It also compared the results of this study with those of the WOBASZ study2 which used a similar methodology and was carried out a de‐ cade earlier (2003–2005). The WOBASZ II study included 6170 Polish participants (3224 women and 2946 men) aged 20 years and over. Metabolic syndrome was de‐ fined according to the 2009 Joint Interim State‐ ment (JIS) criteria.3 The adopted definition al‐ lowed for some options in identifying the indi‐ viduals meeting the criteria, for example, drug treatment for reduced HDL cholesterol (HDL ‐C) and elevated triglycerides levels was not consid‐ ered. It was also decided to use the “Europid” waist circumference (WC) thresholds. The participation rate in the WOBASZ II study was lower than in the WOBASZ study (45.5% vs 77%, respectively). OBJECTIVES The aim of the study was to investigate sleep problems in conservatively treated patients with CKD and to assess association between sleep quality and constipation in that population. PATIENTS AND METHODS In this cross‑sectional study, 100 conservatively treated outpatients with CKD filled questionnaires addressing sleep quality (The Medical Outcomes Study 12‑item Sleep Scale-Revised [MOS‑Sleep‑R]) and constipation‑related symptoms (PAC‑SYM, Rome III criteria). RESULTS The T scores of none of the assessed sleep domains differed across the estimated glomerular filtration rate terciles (all P >0.05). The scores from the PAC-SYM abdominal and stool subscales correlated with all assessed sleep quality domains. In both univariable and multivariable regression models adjusted for key clinical data, functional constipation, less than 7 bowel movements a week, abdominal discomfort, and pain as well as too small bowel movements were independently associated with increased prevalence ratio of decreased sleep quality. However, most of the studies excluded patients with STEMI and cardiogenic shock (CS).</p><p><strong>Objectives: </strong>We sought to evaluate periprocedural outcomes of percutaneous coronary intervention (PCI) with RA and FA in patients with STEMI complicated by CS using data from the Polish National PCI Registry (ORPKI).</p><p><strong>Patients and methods: </strong>A total of 3,565 consecutive patients with STEMI and CS treated with emergent PCI and stent implantation were included. Data was collected prospectively between 2014 and 2018 from 151 tertiary primary-PCI centers in Poland. To avoid possible selection bias, a propensity score matching (PSM) was used to create 945 matched pairs treated via RA or FA.</p><p><strong>Results: </strong>No differences were reported in baseline characteristics, clinical presentation and delays in treatment between RA and FA after the PSM. Similar radiation doses and the total amount of contrast were used in both groups. A similar rate of periprocedural complications was observed in both RA and FA. However, RA was associated with reduced periprocedural mortality (9.4% (89) vs. 18.6% (176); P=0.001) and lower incidence of cardiac arrest (9.7% (92) vs. 16.1% (152); P=0.001). In multivariable analysis, FA was the strongest independent predictor for increased periprocedural mortality (OR 2.087, 95% CI 1.629-2.674; P=0.001).</p><p><strong>Conclusions: </strong>The radial approach was associated with lower periprocedural mortality compared with FA in patients with STEMI complicated by CS. It associates with endothelial dysfunction and increased endothelial permeability, which may lead to severe cardiovascular events.</p><p><strong>Objectives: </strong>We aimed to evaluate the relationship between polymorphisms in the cytochrome b-245 alpha chain (CYBA) gene encoding p22phox, a key subunit of nicotinamide adenine dinucleotide phosphate (NADPH) oxidase, with endothelial function, atherosclerosis and systemic oxidative stress in type 2 diabetes mellitus (T2DM).</p><p><strong>Patients and methods: </strong>Intima-media thickness (IMT), flow- and nitroglycerin-mediated dilatation (FMD and NMD) were measured in 182 patients with T2DM. Assessment of plasma levels of von Willebrand factor (vWF) and malondialdehyde (MDA) levels as well as genotyping of coding sequence C242T (rs4673) and promoter region A-930G (rs9932581) polymorphisms of CYBA were performed using standardized protocols.</p><p><strong>Results: </strong>We observed a significant association of the impaired endothelial function, as measured by FMD, with the C allele of C242T, but not with the A-930G polymorphism. Functional relationship of the C242T polymorphism with endothelial dysfunction remained significant following a multivariable adjustment for major risk factors for atherosclerosis. Mean IMT, NMD, concentrations of MDA or vWF were not related to the specific genotypes of the investigated polymorphisms.</p><p><strong>Conclusions: </strong>C242T, but not A-930G, polymorphism of CYBA significantly affects endothelial function in T2DM. Several nutritional and behavioural aspects have been associated with an increased inflammatory state, or oxidative stress, which could negatively affect the course of COVID-19. As research has shown, metabolic diseases, including obesity, or type 2 diabetes mellitus (T2DM), in which proper diet is especially important, increase the risk of the COVID-19 infection and can exacerbate its course. This should be taken into account particularly in the areas with high prevalence of obesity and T2DM, such as the Western countries. Bone scintigraphy is the first ‐choice method in case of a suspicion of bone metastasis caused by PCa as well as other types of cancer. The in‐ creased uptake (”hot” lesions) on technetium 99m methylene diphosphonate scintigraphy (99mTc ‐MDP) could be seen in all processes (be‐ nign and malignant) which activate osteoclasts. Our case was a false ‐positive finding owing to the fact that the activated osteoblast in the cal‐ cification of soft tissue resembled a metastat‐ ic lesion in the whole ‐body image. In the liter‐ ature, there are a few cases describing unusual calcifications on 99mTc ‐MDP, for example, in vas‐ cular disease, physical exertion, cold exposure, or specific lesions after liver transplantation.3,4 The majority of them are associated with hyper‐ calcemia after kidney transplantation or calcifi‐ cation after bone fracture. Our patient had nei‐ ther a history of bone trauma nor an elevated calcium level. Based on the recent European Associa‐ tion of Urology recommendations, a patient with biochemical recurrence with PSA exceed‐ ing 0.2 ng/ml could be offered 68Ga ‐PSMA ‐11 PET / CT.5 According to the reimbursement rules in Poland, it could be offered only to patients with a suspicion of bone metastasis and uncer‐ tain findings on bone scintigraphy. The false‐ ‐positive findings on bone scintigraphy could have strong clinical implications for treatment recommendation. This case showed that nowa‐ days, hybrid images combining functional and anatomical images are required in case of any unclear picture. Prostate cancer (PCa) is the second most com‐ mon cause of cancer deaths among men in Po‐ land. It is usually a slowly growing tumor, but sometimes it can metastasize early, most often to lymph nodes and bones. Despite various imaging methods used in sus‐ pected cancer recurrence or bone metastases, such as scintigraphy, contrast enhanced computed to‐ mography, or whole ‐body magnetic resonance, positron emission tomography / computed tomog‐ raphy (PET / CT) targeted at the prostate ‐specific membrane antigen (PSMA) expressed on PCa cells is the best solution for PCa imaging. It showed a higher detection rate than other imaging modal‐ ities for disease recurrence with detection rates of 33%, 45%, 59%, 75%, and 95% for prostate ‐specific antigen (PSA) levels 0 to 0.19, 0.2 to 0.49, 0.5 to 0.99, 1 to 1.99, and ≥2 ng/ml, respectively.1,2 A 76 ‐year ‐old man with prostate adenocarci‐ noma, the initial PSA level of 8.5 ng/ml, 1 year after robotic assisted laparoscopic prostatectomy with Gleason score 3 + 3 = 6, and with biochemi‐ cal recurrence with the PSA level of 0.446 ng/ml (PSA doubling time of 3 months) underwent bone scintigraphy. It showed an intense diffuse “hot” lesion in the left femur (FIGurE 1A and 1B).
