The BALM (brilliance, attitude, leadership, and materials) framework is a novel model for adjudicating clinical excellence. There are a few models and frameworks for evaluating clinical excellence, but each has its flaws and does not consider the peculiarities of economically-challenged regions such as sub-Saharan Africa. Coincidentally, such low-resource areas have an urgent need for a framework to assess clinical excellence because of their low health-related indices. The five-step model of professional excellence is a well-recognised tool and was the model upon which the BALM framework was built. So, the BALM framework determines to what extent, using the five-step stages of novice, advanced beginner, competent, proficient, and expert, a clinician has achieved in each of the four critical domains, namely brilliance, attitude, leadership, and materials. Therefore, a clinician, who strives for excellence, aims to be an “expert” in each of the domains of the BALM framework. They aimed to determine whether the SI and MSI correlated with hyperlactatemia, which is considered a marker of sepsis severity and a predictor of poor prognosis. This was a prospective cohort study including all adult patients transported by our county EMS system as per our system’s prehospital sepsis protocol. The study was approved by our medical school’s institutional review board. The study cohort included 1,426 patients. Of these, 969 had an elevated SI. The initial SI ranged from 0.260 to 2.840, with a median of 1.11, and 68% had a “positive” SI (>1.0). The initial MSI ranged from 0.336 to 3.727, with a median of 1.465, and 74% of the cohort had a “positive” MSI (<0.7, or >1.3). Of the patients, 92% had repeat vitals during transport, and the second SI was elevated in 41%. The study found that patients with an elevated SI were significantly more likely to have an elevated lactate (P=0.0309) and be admitted for sepsis (P<0.0001, 95% CI 0.1038 to 0.2714). Patients with an elevated serum lactate were significantly more likely to die during their hospital stay (P<0.0001, 95% CI 0.1503 to 0.3210) with an odds ratio of 2.38 (95% CI 1.59 to 3.56). We aimed to examine the effect of a dedicated emergency department (ED) nerve block team (NBT) on the number and types of ultrasound-guided nerve blocks administered in the ED. We performed a retrospective chart review comparing the nine-month period before and after the creation of a dedicated NBT. This was performed at a tertiary care urban hospital that is a level 1 trauma center. We assessed and compared the number and types of nerve blocks performed, level of learners who performed nerve blocks, and time of ED arrival to disposition, before and after implementation of the NBT. The utilization of nerve blocks rose from 79 prior to the establishment of the NBT to 108 following its creation, representing a 36.7% increase. The average duration of stay in the emergency department decreased by 51 minutes, though this change did not demonstrate statistical significance (P=0.1237). There was an increased variety in the types of nerve blocks performed.
