Calf stretch effectiveness in plantar fasciitis cases: A case series 足底筋膜炎病例中的小腿拉伸效果:病例系列

There are risk factors for plantar fasciitis include structural abnormalities, being overweight, age-related degenerative changes, activities that require prolonged standing and/or ambulation, and errors in training, especially in athletes. We present 20 case series of all patients diagnosed with plantar fasciitis who went to the Orthopedic Polyclinic in 2018 and 2019, they received conservative management with Calf Stretch workouts for 30 days and were evaluated with AOFAS Score. Calf stretch exercises in which the patient stands with the sore leg behind and both hands pushing against a wall, also known as the wall stretch. This exercise is performed 2 times a day with a duration of 10-15 minutes each session. This exercise is done routinely for 30 days. A simple, progressive training protocol consisting of strength training with heavy weights will produce better results in the first three months when compared to plantar-focused stretching and may aid in more rapid relief of symptoms on pain. Cutaneous manifestations are common in patients with end-stage renal disease (ESRD). It can be severe and negatively impact a patient's quality of life. Clinicians can effectively manage ESRD by closely examining the patient's skin and nails, improving the patient's quality of life, and reducing mortality and morbidity. Clinicians can be more aware of ESRD by examining the patient's skin and nails closely so proper management can be performed and the patient's quality of life can be improved. This review aims to increase understanding of common cutaneous manifestations in ESRD for early recognition and better management.  Cutaneous manifestations in ESRD are divided into specific and nonspecific manifestations. Specific manifestations include acquired perforating dermatosis (APD), bullous disease (porphyria cutanea tarda and pseudoporphyria), metastatic calcification (calcinosis cutis and calcific uremic arteriolopathy), and nephrogenic systemic fibrosis. Nonspecific manifestations include pruritus, xerosis, skin pigmentation changes, nail disorders, purpura, hair disorders, oral mucosal changes, skin infections, and other skin manifestations. These manifestations range from benign and asymptomatic to serious conditions that negatively impact life quality. It causes the accumulation of beta-amyloid (Aβ) plaques in the hippocampus and entorhinal cortex, resulting in decreased mass from the brain. Recent studies have shown that the manifestation of this disease is due to an overaccumulation of abnormal Aβ protein due to abnormalities in the APP and APOE4 genes. Point mutations in the APP gene will create the toxic form of Aβ protein, namely Aβ42, and the toxic APOE4 gene will accelerate the onset of Aβ42 deposition and pro-inflammatory activity that exacerbates the degenerative process of the brain. Gene editing as a potential definitive therapy was recently a concern by researchers. CRISPR-Cas9 repairs the APP gene and substitutes the APOE4 gene with APOE3 by modifying the gene's DNA sequence. Nano complex CRISPR-Cas9 tandem peptide lipid is a model for clinicians to target brain nerve cells. In vivo research on an Alzheimer's mouse model proved the potential of nano-complex-based peptides as carriers of CRISPR-Cas9 in brain nerve cells. Disability develops due to the consequences of neurological deficiencies and the failure of the medical rehabilitation process. Post-stroke pain is one of the causes of this failure. In a post-stroke patient, pain is an unpleasant physical and emotional experience. In contrast, range of motion barriers might cause the medical rehabilitation procedure to fail. This paper aims to review the various methods of post-stroke pain management that can be used as an alternative therapy that helps post-stroke patients' rehabilitation. The PubMed database was used to search for different kinds of literature. The study includes clinical studies, pilot studies, and randomized control trials published between January 2015 and June 2023. The authors omit several publications to ensure that the final selection of papers includes only the most relevant and reputable sources of information on post-stroke pain, post-stroke pain management, and pain as a measure of outcome. There are 28 publications to be reviewed. The most prevalent cause of post-stroke pain was hemiplegic shoulder pain in numerous studies that have an impact on the post-stroke recovery process. In conclusion, the options for post-stroke therapy range from conservative rehabilitation to interventional therapy. Several innovative experimental rehabilitation treatment approaches have been studied. The arduous medical education journey exposes students to prolonged stress, potentially exacerbating anxiety and sleep disturbances. This study investigates the correlation between anxiety and sleep quality in Hang Tuah Faculty of Medicine students, bridging the pre-clinical and clinical phases. Employing an analytical observational approach with a cross-sectional design, this study enrolled students from pre-clinical (batches 2020, 2021, 2022) and clinical years (batches DM45, DM46, DM47). Among the 409 approached participants, 296 met the inclusion criteria after excluding 113 samples. Data collection utilized online questionnaires with acquired informed consent. Sleep quality was evaluated via the Pittsburgh Sleep Quality Index, generating scores from 0 to 21. This study showed significantly robust evidence of an association between anxiety and sleep quality (p-value: 0.0001; r: 0.342) across both pre-clinical and clinical students. This study underscores a direct relationship: elevated anxiety levels correspond to diminished sleep quality. Such findings resonate with the prevailing challenges in medical education and their impact on students' mental and physical well-being. Implementing targeted interventions is crucial to address these concerns. In conclusion, this study underscores the undeniable link between anxiety and sleep quality among Hang Tuah Faculty of Medicine students, encompassing the years 2017 to 2022. The implications are far-reaching, emphasizing the necessity of interventions that enhance both psychological well-being and sleep quality. One of which is Ebstein Anomaly (EA), with a prevalence of 1 in 200,0000 births with a clinical characteristic of displacement of the tricuspid valve that causes atrialization of the right-side heart. Women of childbearing age are at risk because they especially have asymptomatic EA, compromised to hemodynamic consequences, and risk of maternal complications. A 25-year-old woman visited the emergency department with worsening shortness of breath since last week, during rest, accompanied by decreased physical activity tolerance, bilateral ankle edema, cyanosis to the lips, and nail bed. She had echocardiography and cardiac multi-slice computed tomography to identify EA before and after her first child's birth. She was well controlled with a loop diuretic, phosphodiesterase type 5 inhibitor, and beta-blocker but could not care for her child due to her condition. The patient was discharged after symptoms subsided, awaiting further interventional evaluation. Although EA is rare, it has high morbidity and mortality, especially in women of childbearing age since it may be asymptomatic during childhood.
