Such analysis has yet to be conducted in Detroit, Michigan. Due to the complexity and unpredictability of orbital fractures, we aim to further understand their epidemiology and association with ocular sequelae within this population. This retrospective study analyzed 204 eyes of 187 patients over a three-year and seven-month period within a Detroit, Michigan hospital system. The incidence of ocular injuries resulting from computed tomography-confirmed orbital bone fractures was quantified and categorized based on patient demographic, fracture location, specific ocular injury, and need for surgical intervention with analysis. It was determined that 84.8% of orbital fracture patients sustained some form of ocular injury. Most fractures occurred within the city (55.1%), and most often occurred in males (63.6%). Fractures of the orbital floor were most often encountered. Commonly injured bones were maxillary, ethmoid, and zygomatic bones, respectively. Approximately one-third of patients with concurrent orbital fracture and ocular trauma sustained severe vision-threatening injury. These traumas are found to peak within summer months. Presence of retinal detachment is significantly related to orbital roof fractures, while maxillary bone fractures are associated with decreased incidences of retinal detachment. Approximately 23% of patients underwent immediate surgery for osseous fractures, while 3.26% of patients underwent immediate ocular surgical repair. Our findings provide insight into the incidence of ocular injury in the setting of traumatic orbital fractures within a Midwest population. Both similarities and differences exist among our own results, and those of distinct geographical regions. These data shed light on Detroit's incidence of ocular sequelae in orbital fracture and contribute to a larger understanding of ocular and orbital trauma epidemiology.","PeriodicalId":515287,"journal":{"name":"Trauma","volume":"9 15","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-07-26","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"141801196","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Over recent years increasing focus has been directed towards ensuring that the physical health needs of older MT patients are met and managed in a timely and appropriate manner during their acute care. This study aims to explore whether the psychological needs of older adults in major trauma centres are also being appropriately recognised and whether this population is appropriately referred for timely psychological support. The paper also briefly outlines the psychological needs identified in this group during their inpatient admission. The implications for major trauma teams and major trauma psychology services are discussed. Retrospective review of referral data captured over a 6 month period as part of the routine service practice for new referrals. Demographic differences were analysed using chi-squared. There is a statistically significant relationship between major trauma patients’ age and their referral status to Clinical Psychology, with older adults receiving fewer referrals for psychological support. The current literature understanding the impact of MT for OAs is sparse. Further exploration is needed with both staff and patients to better understand the service gaps and barriers for OAs accessing psychological support following MT.","PeriodicalId":515287,"journal":{"name":"Trauma","volume":"38 4","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-07-26","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"141798876","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

There are few scalable procedures in place for monitoring patients’ behavioral health recovery and connecting them to needed services. This study piloted an adapted version of an existing technology-facilitated program to monitor substance use and connect patients to care (Trauma Resilience and Recovery Program). Participants aged 12–30 years ( n = 90) were enrolled from Level 1 trauma centers and invited to engage with the adapted Trauma Resilience and Recovery Program. The program involved app-based educational content, daily text message symptom monitoring, periodic online screenings, and referral to behavioral health services. Descriptive analyses were performed to characterize the sample and engagement in the program. Although interaction with app-based educational content was low (15% of participants), engagement in text-based monitoring (participants responded to 65% of prompts) and mental health screens (65% of participants completed all screens) was high. Rates of substance use varied across time points. Elevated depression, anxiety, and post-traumatic stress disorder symptoms were also observed. Most (85%) participants whose screener responses indicated likely presence of behavioral health concerns participated in clinician referral conversations. The enhanced Trauma Resilience and Recovery Program content and procedures were effective in identifying young people experiencing substance use and mental health concerns following traumatic injury and illness. Future work is needed to improve the engagement with educational content and test the effectiveness of the enhanced Trauma Resilience and Recovery Program model in improving substance-related clinical outcomes for trauma-affected young people.","PeriodicalId":515287,"journal":{"name":"Trauma","volume":"60 12","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-07-26","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"141799311","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

We describe a unique presentation with haemoptysis due to complications of concurrent splenic and diaphragmatic injuries resulting in a spleno-pulmonary fistula. A healthy 37-year-old man presented with severe abdominal pain 3 days after blunt abdominal trauma. Contrast-enhanced computed tomography (CT) of the thorax and abdomen showed an American Association for the Surgery of Trauma grade 3 splenic injury with a large subcapsular haematoma, intraperitoneal blood and ongoing intraparenchymal extravasation managed by successful angioembolization of the proximal splenic artery. The patient returned to the emergency department 5 weeks later with a three–day history of progressively worsening haemoptysis. A new CT showed an increase in the splenic haematoma with protrusion into the thorax and a diaphragmatic rupture. Thoracotomy identified a spleno-pulmonary fistula – the splenic haematoma was evacuated, a non-anatomic lung resection was performed and the diaphragmatic defect closed. Cultures from the splenic haematoma grew Cutibacterium acnes which was treated with intravenous antibiotics. We report a unique complication of concurrent splenic and diaphragmatic injury with the development of a spleno-pulmonary fistula possibly due to infection of the splenic haematoma at the time of angio-embolization of the spleen. This case shows the importance of diagnosing diaphragmatic injuries promptly, adhering to sterile technique and considering giving prophylactic antibiotics before angio-embolization.","PeriodicalId":515287,"journal":{"name":"Trauma","volume":"53 14","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-07-26","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"141798690","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Research suggests a potential link between neutrophil-to-lymphocyte ratio (NLR) and mortality. This study aims to assess the prognostic value of NLR in intensive care unit (ICU)-admitted individuals with multiple trauma. This study reviewed medical records of multiple trauma patients admitted to the ICU of Shahid Mohammadi Hospital in Bandar Abbas, Iran, from 2020 to 2021. Variables assessed included age, sex, blood pressure, Glasgow Coma Scale, Injury Severity Score, NLR, trauma details, surgical procedures, hospital and ICU length of stay, duration of mechanical ventilation, and mortality. In the study, 160 multiple trauma patients were analyzed, with an average age of 34.13 ± 14.74 years. Among them, 130 (81.3%) were male and 75 (46.9%) died. The mean NLR did not show a significant difference between survivors and non-survivors ( p = 0.853). The predictive ability of NLR for mortality, as measured by the area under the receiver operating characteristic curve, was 0.530 (95% confidence interval [CI] 0.440; 0.620, p = 0.517). Patients who died had higher rates of ICU admission, mechanical ventilation requirement, longer mechanical ventilation duration, and longer hospital and ICU stays (all p < 0.001). Multivariable logistic regression analysis found that a non-sinus rhythm in the admission ECG was associated with approximately eight times higher odds of mortality compared to a sinus rhythm (adjusted odds ratio = 7.961, 95% CI 1.724; 36.759, p = 0.008). No significant association was found between NLR and mortality in univariable binary logistic regression, even after adjusting for various factors (adjusted OR = 0.967, 95% CI 0.989; 1.042, p = 0.375). Our findings indicate that admission NLR did not demonstrate efficacy as a prognostic indicator of mortality among patients with multiple trauma admitted to the ICU.","PeriodicalId":515287,"journal":{"name":"Trauma","volume":" 6","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-06-07","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"141372185","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Dislocations of the acromial end of the clavicle are quite common traumatic injuries of the locomotor apparatus and, according to various authors, account for 3 to 26.1 % of dislocations of other locations and about 10 % in the structure of acute injuries of the shoulder girdle, ranking third after dislocated shoulders and forearms. They occur mainly in young men of the most working age, which determines the social significance of this injury. Stabilization of the acromial end of the clavicle, as a rule, was ensured by fixation according to Weber and the use of a hook plate. The purpose of the study: to determine the shortcomings and strength characteristics of the fastening elements in the fixation of the acromial end of the clavicle according to Weber. Materials and methods. From 2015 to 2020, a retrospective analysis was performed of treatment outcomes in 57 patients who underwent surgery for dislocation of the acromial end of the clavicle and Weber fixation. Calculations of the strength of fixation of the acromial end of the clavicle with a cerclage wire and spikes in case of its dislocation were carried out in the biomechanics laboratory of the State Institution “Sytenko Institute of Spine and Joint Pathology of the National Academy of Medical Sciences of Ukraine”. Results. According to the results of clinical studies, a violation of the integrity of the wire and spikes was observed in 10.5 % of cases, destruction of the acromion process and acromial end of the clavicle — in 19.3 %, migration of the spikes — in 12.3 %. Results of the experimental study show that these shortcomings are clearly correlated with the mechanical properties of the wire and spikes. Conclusions. The factors for unsatisfactory results of surgical treatment for dislocations of the acromial end of the clavicle when using Weber’s method are the violation of the integrity of the wire and spikes, the destruction of the acromion process and the acromial end of the clavicle, the migration of the spikes, which are caused by the mechanical properties of these structures.","PeriodicalId":515287,"journal":{"name":"Trauma","volume":"17 10","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-05-16","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"140967821","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Knee osteoarthritis is a progressive incurable disease that in severe cases leads to total joint replacement, which requires significant economic costs and medical and social adaptation, has a number of postoperative complications and adverse outcomes. Therefore, special attention has recently been paid to the prevention and treatment of the early stages of the disease. The purpose of the study was to conduct a systematic analysis of scientific literature on the prospects and possibilities of using arthroscopy in the early diagnosis of monoarthrosis. Material and methods. A literature search was conducted in the PubMed and MEDLINE databases for 2010–2024 using the following medical subject headings and keywords: “deforming osteoarthritis of the knee joint”, “osteoarthritis of the knee joint”, “unilateral deforming osteoarthritis of the knee joint”, “unilateral osteoarthritis of the knee joint”, “gonarthrosis”, “monoarthrosis”, “arthroscopy”, “diagnosis”, “treatment”. If necessary, literature sources beyond the search period were used in some cases. A general search revealed 48 references on the use of arthroscopy as a diagnostic and therapeutic method in the early stages of monoarthrosis. The primary exclusion concerned the literature in which arthroscopy was used for the diagnosis and treatment of late-stage knee osteoarthritis (n = 38). The secondary exclusion included literature sources that contained only background information (summary, figures, references) (n = 12). As a result, only relevant full-text articles in professional journals remained (n = 15). Results. The lack of correlation between clinical symptoms and radiological signs of knee osteoarthritis causes low availability of orthopaedic care: more than 30 % of newly diagnosed patients have a severe stage of the disease, and in some cases the pathology is detected only in connection with the manifestation of complications; the diagnosis of osteoarthritis due to a large percentage of painless development of the disease (40 %) is often established at terminal stages. All of this suggests the need for further research into the various factors that influence the frequency, prevalence, economic and social burden of knee osteoarthritis. Arthroscopy is potentially the gold standard for validating non-invasive assessment methods such as magnetic resonance imaging, as it provides high magnification and direct view of articular cartilage with non-destructive interactive assessment of its structure and functional properties. Arthroscopy allows for a more detailed description of the depth and extent of lesions, as well as the detection of subtle changes such as cartilage softening, fibrillations, and tangential peeling. Clinical symptoms and structural changes in the knee joint elements visualised during arthroscopy in patients with monoarthrosis are covered in a few studies, some of which were published 10 years ago. The results of modern arthroscopic studies can be an important co","PeriodicalId":515287,"journal":{"name":"Trauma","volume":"41 13","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-05-16","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"140971529","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

In the structure of all traumatic spine injuries, the thoracolumbar junction is predominant, accounting for over 53 % of all vertebral fractures. One of the most clinically significant types of injuries in this area are burst fractures. The purpose was to study the stress-strain state of the thoracolumbar spine model with a burst fracture of the Th12 vertebra in various transpedicular fixation options under compression load. Materials and methods. The study developed and investigated a finite element model of the thoracolumbar spine with a burst fracture of the Th12 vertebra. The burst fracture was modeled by dividing the vertebral body of the Th12 into several planes, transforming it into separate fragments. The gaps between these fragments were filled with a material that simulated the interfragmentary regenerate. Variants of transpedicular stabilization using different types of screws, mono- or bicortical, and with or without cross-links, were examined. The model was analyzed under compression load. Results. The maximum level of stress among the bone structures directly involved in fixation was registered in the L2 vertebral body. It amounted to 19.9, 15.6, 19.4, and 15.1 MPa, respectively, for models with monocortical screws without cross-links, bicortical screws without cross-links, monocortical screws with cross-links, bicortical screws with cross-links. Simultaneously, the zone of screw entry into the arch of this vertebra shows values of 10.1, 15, 10.2, and 14.3 MPa for these models, respectively. Peak loads on the metal structure elements are observed on the rods, amounting to 212.5, 159.6, 203.7, 142.8 MPa, respectively, for the considered models. Conclusions. The results of the study showed that under the influence of compression load when modeling a burst fracture of the thoracolumbar junction, the use of long screws leads to a reduction in stress levels, both in the elements of the metal structure and in the bone elements of the model, while the use of cross-links has a negligible effect.","PeriodicalId":515287,"journal":{"name":"Trauma","volume":"37 13","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-05-16","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"140969732","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Surgical methods of treatment for dislocated clavicle fractures, such as osteosynthesis with a plate or an elastic titanium rod, are mostly used in practice. The disadvantage of the latter is the possible migration of the implant with perforation of the skin. The purpose was to study the effectiveness of an intramedullary rod, improved by us, for osteosynthesis of dislocated clavicle fractures type OTA 15A and OTA 15B according to the AO/ASIF classification. The essence of the improvement was the possibility of closed minimally invasive intramedullary osteosynthesis of the clavicle body, with a simplified technique of the surgical intervention itself, prevention of implant migration. The latter was achieved by the fact that one of the ends of the rod had a drill, which eliminated the need for predrilling the medullary cavity of the clavicle to insert the rod, and its opposite end had a thread onto which a nut is screwed to achieve compression between the clavicular fragments. Forty-two patients with closed dislocated clavicle fractures were under supervision in the clinic of traumatology and orthopedics of Kyiv City Clinical Hospital 6. They underwent intramedullary osteosynthesis for clavicle fractures with an improved compressing rod (closed in 28 cases, open in 14) under regional anesthesia. In dyna-mics of the postoperative period, a clinical and radiological examination was performed, the severity of the pain syndrome and the function of the upper limb were assessed according to the visual analog scale (VAS), Disability of Arm, Shoulder and Hand (DASH). As shown by the results of X-ray examinations, the average period of clavicle fracture union was 10.1 ± 1.4 weeks (range 8–12 weeks). During the observation of the patients, favorable dynamics on the VAS and DASH were noted; by the end of the study (12 months after the operation), the functional capabilities of the upper limb on the DASH corresponded to 1.54 ± 0.84 points, VAS score was 9.60 ± 2.11 points. The improved construction of the intramedullary rod for osteosynthesis of dislocated clavicle fractures of the OTA 15A and OTA 15B type can be recommended for use in practice, as it has demonstrated favorable clinical, radiological, and functional outcomes.","PeriodicalId":515287,"journal":{"name":"Trauma","volume":"30 5","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-05-16","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"140967617","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
