Akçiçek","doi":"10.31086/TJGERI.2020.125","DOIUrl":"https://doi.org/10.31086/TJGERI.2020.125","url":null,"abstract":"Giriş: Bu çalışma, acil servise başvuran yaşlı hastalarda düşme korkusu ile düşme nedenli başvuruları ve ilişkili faktörleri araştırmak amacıyla planlanmıştır. Gereç ve Yöntem: Bu araştırmanın evreni, 2015 Ağustos ayı içerisinde acil servise başvuran 65 yaş ve üstü hastalardan oluşmuştur. Başvuru sonuçları, fonksiyonel durum, son bir yılda denge ve yürüme sorunu ile düşme nedenli acil servise başvuru ve sosyodemografik verileri içeren ölçme aracı eğitim alan anketör tarafından acil serviste hasta ve/veya hasta yakınları ile yarı yapılandırılmış görüşme yolu ile doldurulmuştur. Lojistik regresyon analizi uygulanmıştır. Bulgular: Acil servise başvurarak çalışmaya alınan 555 hastanın %12.6’sı düşme şikayeti ile gelmiştir. Huzurevinde yaşama, düşme korkusu ve son bir yıl içinde düşme nedeni ile acil servise başvurmuş olma düşme ile başvuru ile ilişkili bulunmuştur. Düşme korkusu, çalışmaya alınan yaşlıların %22.2’sinde tespit edilmiş olup; kadın cinsiyet, medeni durum, düşme durumu ve daha iyi fonksiyonel durum ile ilişkili bulunmuştur. Sonuç: Acil servise başvuran yaşlılarda; düşme ve düşme korkusu arasında anlamlı bir ilişki bulunmuştur. Düşme korkusunun düşme üzerine etkisini daha iyi anlamak için prospektif çalışmalara ihtiyaç vardır. Anahtar Sözcükler: Düşme; Acil Servis; Düşme Korkusu; Yaşlı. Introduction: Parkinson’s disease is the second leading neurodegenerative disease worldwide; however, its pathogenesis remains unclear. Recently, the neuroinflammation theory, one of the theories explaining the pathogenesis of Parkinson’s disease, has become prominent. We investigated the relationship between Parkinson’s disease and inflammatory markers including epicardial adipose tissue thickness, neutrophil-lymphocyte ratio, and thrombocyte-lymphocyte ratio. Materials and Method: Seventeen patients with Parkinson’s disease and 21 healthy individuals (control group) were enrolled. All the patients were evaluated by a neurologist using the Unified Parkinson’s Disease Rating Scale and Hoehn and Yahr staging scale. A cardiologist measured epicardial adipose tissue thickness using echocardiography. After routine laboratory analyses, neutrophil-lymphocyte and thrombocyte-lymphocyte ratios were calculated for each patient. Results: Both epicardial adipose tissue thickness and thrombocyte-lymphocyte ratio were higher in the patient group than in the control group, but the difference was significant only for the former. Epicardial adipose tissue thickness was significantly correlated with the scores of 2nd and 3rd sections of the Unified Parkinson’s Disease Rating Scale, which evaluate activities of daily living and motor functions, respectively, and with the total Unified Parkinson’s Disease Rating Scale score. Inflammatory marker evaluation according to the disease stage based on the Hoehn and Yahr staging scale revealed a significant difference in epicardial adipose tissue thickness between stage 1 and stage 2 patients. Moreover, there was a significant correlation between disease duration and thrombocyte-lymphocyte ratio. Although the gap between the two sexes has begun to close, women still have higher all-cause mortality. However, there is little data with respect to elderly women, particularly for those over 80 years. Therefore, we sought to determine all-cause mortality of elderly women with acute myocardial infarction and to compare it to men. Materials and Method: This retrospective study evaluated patients 80 years or older who were admitted to the cardiology clinic due to acute myocardial infarction between May 2015 and November 2017. The primary outcomes of the study were in-hospital, 30-day, and longterm all-cause mortality. Results: Two hundred twenty patients (126 women and 94 men) were included in the final analysis. Median follow-up was eight months. Long-term all-cause mortality of women was significantly higher compared to men [n=52 (41.3%) vs. n=26 (27.7%), respectively; p=0.03]. There was no significant sex difference for both in-hospital and 30-day mortality, although women had more events. Kaplan-Meier analysis revealed lower cumulative survival for women (log-rank: p=0.02), and Cox regression analysis found that female sex was as an independent risk factor for mortality [hazard ratio=1.7 (95% confidence interval=1.06-2.72); p=0.02]. Conclusion: Sex-related differences on mortality continue to be a critical issue, even for elderly patients with acute myocardial infarction.
