We aimed to investigate the career and season prevalence of sport-related analgesics use among elite youth and elite senior handball players. We explored the frequency of analgesic use on practice and competition days within one season. The sample consisted of 459 elite handball players (233 female, 226 male). Sport-related analgesic use was assessed with two replicable measures of prevalence complemented by two frequency measures. χ2 analyses were performed to determine group differences in prevalences for gender, age group, contract situation, and role within the team. We conducted hierarchical binomial linear regression analyses for the analgesic prevalences to evaluate the prediction of the prevalences from the independent variables. The sport-related career prevalence of analgesic use was 70.7% (95% CI[66.6, 74.9]). Throughout one season, 55.6% (95% CI[51.0-60.1]) of handball players used analgesics. Gender, age, contract status, and role within the team seemed to impact the use of analgesics. Further, analgesics were used significantly more often on competition than on practice days. Our findings indicate that the sport-related use of analgesics is relatively common among elite handball players, with female players, senior players, players with a contract, and perceived rotation players reporting significantly higher career and season prevalences. Shortness of breath is a common symptom reported by individuals with PASC. Respiratory muscle training (RMT) to strengthen respiratory muscles (e.g diaphragm, intercostal muscles) can improve symptoms in various diseases. The aim of the study therefore was to investigate the effectiveness of RMT for the reduction of PASC-related shortness of breath by a daily home-based app-guided training intervention. Methods: Sixteen patients (age 32.0±16.1 years (Mean±SD), female N=9) suffering from persistent shortness of breath after SARS-CoV-2 infection were randomized to either an Intervention-first-group (IfG) starting with RMT immediately or an Intervention-second-group (IsG) receiving RMT after a control period of one month. RMT was conducted using a commercially available training device and daily app-based RMT instructions. Severity of respiratory symptoms was measured using the St. Georges Respiratory questionnaire (SGRQ). Ventilation parameters and peak oxygen consumption (V˙ O2peak) were assessed by cardio pulmonary exercise testing (CPET). Results: Fifteen patients completed the study. The RMT intervention led to a significant improvement of subjective symptoms (SGRQ reduction: -11.41±8.28, p<0.001, d=-1.38, 95% CI [-0.65, -2.08], Subjective health: 12.43±11.51%, p=0.001, d=1.08, 95% CI [0.40, 1.73]) and improved maximum ventilation (8.94±9.23 l/min, p=0.003, d=0.97, 95% CI [0.31, 1.59]). Exercise capacity and V˙ O2peak significantly increased in a subgroup of patients with low VO2peak values at study inclusion (relPmax: 0.24±0.26 W/kg, p=0.022, d=0.95, 95% CI [1.13, 1.72]), relVO2peak: 2.98±5.79 ml/min*kg, p=0.013, d=1.10 95% CI 0.21,1.89]). During the control period (only IsG) symptoms, maximum power output and VO2peak remained unchanged with only maximum ventilation significantly decreasing. Conclusions: Patients with persistent shortness of breath after SARS-CoV-2 infection benefit from intense, regular RMT over one month. Body composition (BC) determinations can help to make statements about nutritional status and health. However, there is no BC database among ARGs in Germany. Method: BC determinations were performed among German ARGs using calipometry and different bioelectrical impedance analysis (BIA). All devices were compared in terms of percentual fat mass (FM%) using ANOVA. Additional parameters for the BIA devices were tested for differences using t-tests. Bland-Altman plot was created to compare the BIA devices in terms of FM%. Results: Twenty-five ARGs (M=15.2 years, SD±2.11) were measured. FM% was determined by calipometry (M=7.51, SD±1.35), InBody (M=11.87, SD±5.1), and BIVA 101 devices (M=16.2, SD±4.26). ANOVA showed significant differences for FM% (F[2, 48]=64.46, p<0.001; partial η²=0.729). For Bonferroni-corrected post-hoc tests, significant differences (p < 0.001) between the three measurement methods were determined. The t-tests in BC parameters also showed significant differences (p<0.001) between the two BIA devices. Discussion: Compared to calipometry, BIA provides additional BC parameters. Nevertheless, the use of different measurement methods and devices in the comparison of BC parameters is not recommended. Reference values must be adapted to the respective devices. Although the use of dietary supplements, non-steroidal anti-inflammatory drugs and alcohol does not fall into the category of ‘doping’, it nevertheless harbors risks and side effects during sporting activity. Methods: An online questionnaire developed specifically for this study was used to research the consumption of supplements, analgesics, and alcohol among golfers. Results: A total of 877 golfers (257 women, 619 men; 1 diverse person; age: M=56; SD=15 years) participated in the survey. The results of the survey showed that 40.1% of golfers consumed supplements, 46.6% analgesics, and 40.4% alcohol. Dependent on participation in golf tournaments, the consumption of supplements (43.0%), analgesics (52.1%) and alcohol (47.2%) increases significantly. Gender correlates significantly with the consumption of alcohol at golf tournaments (p=0.001); more men consume alcohol than women. Age correlates significantly and negatively with the consumption of supplements at golf tournaments (r=- 0.131; p=0.001) and the consumption of alcohol at golf tournaments (r=- 0.238; p=0.000). Discussion: The frequency of consumption was alarming for a large proportion of the golfers surveyed. {"title":"Relative Energy Deficiency in Sport (RED-S): Scientific, Clinical, and Practical Implications for the Female Athlete.","authors":"H E Cabre, S R Moore, A E Smith-Ryan, A C Hackney","doi":"10.5960/dzsm.2022.546","DOIUrl":"https://doi.org/10.5960/dzsm.2022.546","url":null,"abstract":"<p><strong>Problem: </strong>If athletes develop low energy availability (LEA), it can lead to a Relative Energy Deficiency in Sport (RED-S) syndrome which has severe health consequences if not treated.</p><p><strong>Methodology: </strong>A narrative review of the most recent and pertinent literature on the topic, with special emphasis on women.</p><p><strong>Results: </strong>In assessing the current literature, we have synthesized: i) the scientific implications of LEA and RED-S, ii) the clinical manifestations of the conditions currently available for detection, as well as iii) the practical implications for healthcare and support for female athletes and teams in planning intervention or prevention strategies (maintaining EA >45 kcal/kg FFM/day).</p><p><strong>Discussion: </strong>The 'Female Athlete Triad\" emerged in the 1990s as researchers understood more of the etiological adaptation of female athlete health to sports training. In the last 10 years, the scientific community has recognized that the 'Triad' approach was too narrow in focus, and the broader concept of RED-S emerged. Both the Triad and RED-S are consequences of a frequently prevalent LEA in athletes (<30 kcal/kg FFM/day). Developing LEA and RED-S compromises training adaptation, performance capacity, and health in athletes. For these reasons, it is critical that an athlete's support team recognize the behaviors that may indicate RED-S evolution.
