Hyperbaric oxygen therapy has therefore been described in some studies as an alternative in these cases, as it mitigates the effects of the diseases and liver transplantation. Objective: To describe the effects of hyperbaric oxygen therapy in the pre- and post-operative periods of patients undergoing liver transplantation. Methods: This is an Integrative Review using the PubMed and Web Of Science databases. The following descriptors were used: “Hyperbaric oxygenation”, “Liver transplantation” and “Hyperbaric oxygen therapy” with the Boolean operator “AND”, and articles of relevance to the topic were selected. Initially, 49 articles were selected, all published in the last 20 years, in Portuguese and/or English. After analysis, 6 articles matched the proposed objective. Results: It can be seen that intraoperative systemic O2 content affects postoperative recovery in patients undergoing liver transplantation. Early hyperbaric oxygen therapy acts as a protector in reducing the severity of hepatocyte ischemia/reperfusion injury. Hyperbaric oxygen therapy also influences the immune response of patients undergoing liver transplantation, reducing incompatibility. Still on the subject of the immunomodulatory effects of hyperbaric oxygen therapy, this therapy has been shown to be effective in helping to prevent post-operative infections by improving the antibacterial activity of immune cells and increasing the bactericidal effect of antibiotics. In the case of patients on the waiting list for liver transplants, a reduction in the number and intensity of encephalopathy episodes, an improvement in pruritus and a feeling of well-being were observed after treatment with oxygen therapy. In terms of early allograft dysfunction, it was shown that patients with dysfunction had lower O2 values in the anepathic and neo-hepatic phases when compared to patients without dysfunction in the post-operative period. In addition, during the anepathic phase, the content of the SatO2 level was also lower in the group with dysfunction than in those without. Com isso, a oxigenoterapia hiperbárica é descrita em alguns estudos como uma alternativa nesses casos por mitigar os efeitos das doenças e do transplante hepático. Objetivo: Descrever os efeitos da oxigenoterapia hiperbárica no pré e no pós-operatório de pacientes submetidos ao transplante hepático. Métodos: Trata-se de uma Revisão Integrativa na base de dados PubMed e Web Of Science. Foi utilizado os descritores: “Hyperbaric oxygenation”, “Liver transplantation” e “Hyperbaric oxygen therapy” com o operador booleano “AND”, e selecionados artigos de relevância para o tema. Inicialmente, foram selecionados 49 artigos, todos publicados nos últimos 20 anos, em português e/ou inglês. Após análise, 6 artigos corresponderam ao objetivo proposto. Resultados: Pode-se verificar que o conteúdo intraoperatório de O2 sistêmico afeta a recuperação pós-operatória em pacientes submetidos ao transplante de fígado. A oxigenoterapia hiperbárica precoce atua como protetor na redução da gravidade da lesão de isquemia/reperfusão dos hepatócitos. A oxigenoterapia hiperbárica também influencia na resposta imune do paciente submetido ao transplante de fígado, reduzindo a incompatibilidade. Ainda sobre os efeitos imunomodulatórios da oxigenoterapia hiperbárica, essa terapia se mostrou eficaz no auxílio da prevenção de infecções pós-operatórias por melhorar a atividade antibacteriana das células imunes e aumentar o efeito bactericida dos antibióticos. Tratando-se de pacientes em lista de espera para transplante de fígado, foi observado após tratamento com a oxigenoterapia a diminuição no número e na intensidade dos episódios de encefalopatia, melhora do prurido e sentimento de bem-estar. No quesito disfunção precoce do aloenxerto, foi demonstrado que pacientes com disfunção apresentaram valores mais baixos de O2 nas fases anepática e neo-hepática, quando comparado com os pacientes sem disfunção no período pósoperatório. Além disso, durante a fase anepática, o conteúdo do nível de SatO2 também foi menor no grupo com disfunção do que nos sem disfunção. {"title":"Existe Ainda um Papel para a Citologia Urinária no Transplante Renal? A detecção precoce de infecções pode prevenir possíveis disfunções e perda do enxerto. No entanto, a sua utilização tem decrescido. Métodos: Realizamos um estudo prospectivo em amostras de urina provenientes de 29 pacientes consecutivos, submetidos ao transplante renal ao longo de um período contínuo de 17 meses, coletadas durante seu primeiro ano pós-transplante. Um mesmo técnico preparou as amostras por citocentrifugação, coloração de Papanicolaou e um único patologista as analisou. Amostras positivas para “decoy cells” (DCs) foram preparadas por dois outros técnicos, para análise por microscopia de contraste de fase, estudo ultraestrutural e imunofluorescência (antígeno T SV40). Resultados: Foram avaliadas 252 amostras de urina de 26 pacientes. Duas amostras consecutivas (0,8%) dentre cinco, de uma mesma paciente (3,8%), referentes à quarta e quinta semana pós-transplante, apresentaram lesão intraepitelial escamosa de baixo grau; na amostra referente ao sexto mês pós-transplante foram detectadas apenas raras células epiteliais com atipias de significado indeterminado (0,4%); o posterior exame ginecológico da paciente e teste de Papanicolaou não revelaram alterações. Pseudohifas e esporos fúngicos foram encontrados em cinco amostras (2,0%) de quatro pacientes (15,4%). Trichosporon sp. foi identificado na urina de um desses pacientes. Decoy cells foram detectadas em 25 amostras (9,9%) de seis pacientes (23,1%). Dois pacientes tiveram eliminação sustentada de DCs; um deles, apresentando esfregaço de fundo sujo, desenvolveu Nefropatia associada ao poliomavírus. O estudo ultraestrutural das DCs mostrou partículas virais icosaédricas. A imunofluorescência (antígeno T SV40) foi positiva nos núcleos das DCs. A análise por microscopia de contraste de fase foi bem-sucedida em amostras com numerosas DCs. Conclusão: A citologia urinária sistemática após o transplante é útil na detecção de alguns sinais de infecção. Pacientes com eliminação sustentada de DCs e esfregaços de fundo sujo merecem atenção clínica especial. A microscopia eletrônica e a imunofluorescência (antígeno T SV40) são técnicas alternativas para detectar a reativação do poliomavírus. Living-donor kidney transplantation offers an alternative to deceased donation, but HLA or AB0 incompatibility can pose a significant obstacle. This study aimed to show the results achieved by a portuguese hospital since its integration into an international cross-over program, the South Alliance for Transplants (SAT). Methods: The SAT program was founded in 2017 and is composed of ten Spanish hospitals, three Italian hospitals and one Portuguese hospital. The program runs every 4 months and enrolled only pairs that were incompatible. Organ transportation is carried out in partnership with the Portuguese Air Force. Results: Three distinct cross-over kidney transplants were performed in partnership with three Spanish hospitals, culminating in the transplantation of three Portuguese patients out of a total of seven patients. The first swap was performed in March 2020, at the beginning of the COVID-19 pandemic, in partnership with two Portuguese and one Spanish hospital. It involved one donor/recipient pair from each country, with AB0 incompatibility between the Portuguese donor and recipient, and no complications were reported. The second swap occurred in December 2021, with three donor/recipient pairs (one Portuguese, where the recipient presented anti-donor antibodies and positive crossmatch with the potential donor, and two from two Spanish hospitals). It was complicated by type-IB cellular rejection in the Portuguese recipient, one week after transplantation, which was treated with corticosteroid therapy. The third swap, also in December 2021, involved two donor/recipient pairs (one Portuguese and one Spanish). It was complicated by delayed renal function due to acute tubular necrosis (histological diagnosis) in the Portuguese recipient. At follow-up, the patients‘ serum creatinine levels were within normal limits, and no other unexpected outcomes were recorded. Conclusion: SAT program has allowed some successful cross-over kidney transplants, probably improving the outcomes of kidney transplantation in Portugal. O transplante renal de doador vivo oferece uma alternativa ao doador cadáver, mas a incompatibilidade HLA ou AB0 pode representar um obstáculo significativo. Este estudo teve como objetivo mostrar os resultados alcançados por um hospital português desde sua integração num programa internacional de doação cruzada, o South Alliance for Transplants (SAT). Métodos: O programa SAT foi fundado em 2017 e é composto por dez hospitais espanhóis, três hospitais italianos e um hospital português. O programa ocorre a cada 4 meses e inscreve apenas pares que são incompatíveis. O transporte de órgãos é realizado em parceria com a Força Aérea Portuguesa. Resultados: Foram realizados três cruzamentos distintos em parceria com três hospitais espanhóis, culminando no transplante de três doentes portugueses de um total de sete doentes. O primeiro cruzamento foi realizado em março de 2020, no início da pandemia de COVID-19, com a parceria de dois hospitais portugueses e um hospital espanhol, envolvendo 1 par doador/recetor de cada país, em que o português apresentava incompatibilidade AB0 e onde não se registraram complicações. O segundo ocorreu em dezembro de 2021 com 3 pares doador/receptor (1 português em que o recetor apresentava anticorpos anti-dador e crossmatch positivo com o potencial doador; e 2 de dois hospitais espanhóis), complicado com rejeição celular tipo-IB uma semana após o transplante no receptor português, tratada com corticoterapia. O terceiro cruzamento ocorreu também em dezembro de 2021 com 2 pares doador/recetor (1 português e 1 espanhol), complicado com atraso na função renal por necrose tubular aguda (diagnóstico histológico) na receptora portuguesa. Durante o follow-up, os níveis de creatinina sérica dos doentes mantiveram-se dentro dos limites normais, e não foram registradas outras intercorrências. Conclusão: O programa SAT permitiu a realização de alguns transplantes renais cruzados bem-sucedidos, provavelmente melhorando os resultados do transplante renal em Portugal. {"title":"Is There Still a Role for Urine Cytology in Kidney Transplantation? The early detection of infections can prevent possible dysfunction and graft loss. However, its use has been decreased. Methods: We conducted a prospective study on voided urine samples from 29 consecutive patients undergoing kidney transplantation over a continuous period of 17 months, collected during their first-year post-transplantation. One technician prepared the samples by cytocentrifugation, Papanicolaou staining, and one pathologist analyzed them. Decoy cells (DCs) positive samples were prepared, by other two technicians, for contrast phase analysis, ultrastructural study, and immunofluorescence for SV40 T antigen. Results: We evaluated 252 urine samples from 26 patients. Two consecutive samples (0.8%) out of five, from the same patient (3.8%), referring to the fourth- and fifth-week post-transplant, showed low-grade squamous intraepithelial lesions; only rare epithelial cells with atypia of undetermined significance were detected (0.4%) in the sample corresponding to the sixth month post-transplant. The patient’s subsequent gynecological examination and Pap smear revealed no changes. Pseudohyphae and fungus yeasts were found in five samples (2.0%) from four patients (15.4%). Trichosporon sp. was identified in the urine of one of these patients. Decoy cells were detected in 25 samples (9.9 %) from six patients (23.1%). Two patients had sustained DCs shedding; one of them, presenting dirty background smears, developed Polyomavirus-associated nephropathy. Ultrastructural study of DCs showed icosahedral viral particles. Immunofluorescence (SV40 T antigen) was positive in DCs nuclei. Analysis by contrast phase was successful in samples with numerous DCs. Conclusion: Systematic urinary cytology after transplantation helps detect some infection signs. Patients with sustained DCs shedding and dirty background smears deserve special clinical attention. Electron microscopy and immunofluorescence (SV40 T antigen) are alternative techniques to detect polyomavirus reactivation. Com base nos resultados encontrados em pesquisa no PubMed sem limitação de data ou língua, discutimos a importância do planejamento reprodutivo para mulheres com transplante de órgãos sólidos e que deve ser feito desde antes da realização do transplante. Existem contraindicações formais de gestação, entretanto quando planejada, dentro de critérios clínicos, laboratoriais e uso de imunossupressão adequada e em seguimento de pré-natal especializado essa gestação é possível e com bons resultados perinatais. Juliato, Ilka de Fátima Santana Ferreira Boin","doi":"10.53855/bjt.v26i1.542_eng","DOIUrl":"https://doi.org/10.53855/bjt.v26i1.542_eng","url":null,"abstract":"The objective of this literature review is to provide some pregnancy results in transplanted women, proposals for preconception, prenatal and postpartum evaluation, including postpartum contraception, and therapeutic guidance during pregnancy and lactation. Based on the results found in research on PubMed without date or language limitations, we discuss the importance of reproductive planning for women with solid organ transplants and what should be done before the transplant. There are formal contraindications to pregnancy; however, when planned, within clinical and laboratory criteria, and with adequate immunosuppression and specialized prenatal care, pregnancy is possible with good perinatal results.
