Likewise, Carlo Mauri, a famous Italian explorer/writer went to Siberia to be personally treated by Dr. Ilizarov for a recalcitrant infected non-union of his tibia. Dyatlova","doi":"10.17816/2311-2905-17431","DOIUrl":"https://doi.org/10.17816/2311-2905-17431","url":null,"abstract":"Aim: demonstrate of primary analysis of the data of local registry which contains all cases of hip periprosthetic infection from 01.01.2010 to 31.12.2020. \u0000Methods: We retrospectively have entered into the database information about patients with hip periprosthetic infection who have been treated in the clinic from 2010. Data was collected by researching medical cards and medical information system, examination of patients in the outpatient clinic, phone communication and e-mail. \u0000Results and discussion: We have treated 449 patients with hip periprosthetic infection. The average age of patients was 55,6±12,6 years(Ме -57, CI 54,4; 56,8) and turned out to be significantly younger than the mean age in well-noun national registers and 66%(n=277) were male. In more than 90% (n=407) of cases, infection was classificated as cronic (more than 3 weeks after manifestation), 76,8% (n=345) of patients had open sinus tract to the prosthesis. \u0000Our results have a significant difference from Australian register, as only 21 % of patients in Australia have a sinus or time after manifestation of symptoms for more than 30 days. Among the patients of «Ilizarov Centre» gram-positive flora was 55.7%(n=288). This result correspond with publications analyzing the National Registers of Denmark, Sweden and Australia. \u0000We were able to evaluate the results of treatment in 84% (n=377) of the patients and we couldn’t connect with patients in 16% (72/449) of cases. The average follow-up time 5,92 ± 2,55 years; Me-6; CI 2,39; 2,73. The percentage of patients who died of uncontrolled infection was 1,7% (n=8/449); reinfection of periprosthetic infection were in 6,2% (n=28/ 449) of cases, death from other diseases after treating in our clinic without any sights of infection was identified in 9,1% (n=41/ 449) of cases. Infection control has been achieved in 67% (300/449) of cases. \u0000Conclusion : The object of the local register of the National Ilizarov Medical Research Centre for Traumatology and Ortopaedics\" were only patients with periprosthetic infection. Shubnyakov","doi":"10.17816/2311-2905-17417","DOIUrl":"https://doi.org/10.17816/2311-2905-17417","url":null,"abstract":"Introduction: The purpose of this study was to examine our experience with a nonmodular tapered fluted stem (NTFS) in revision total hip arthroplasty (RTHA) to determine frequency and cause of re-revisions with stem removal, factors associated with stem subsidence and to determine functional outcomes. Patients and method: We retrospectively analyzed the results of using NTFS in 78 cases. The average follow-up period was 5.1 years. Results: In 14 (17.9%) cases, the patients underwent re-revisions with stem removal. Significant subsidence was observed in 5 (6.4%) cases. Type IV femoral defect as per the Paprosky classification (p=0.01) and bicortical contact 2.0 cm (p=0.02) were risk factors for stem subsidence. In addition, we found that the risk of obtaining a bicortical contact of less than 2 cm is higher in patients with a type IV femoral defect (p=0.05). Patients demonstrated improved functional outcomes. Patients with significant stem subsidence having worse functional outcomes compared to patients without significant subsidence. Conclusions: The use of a NTFS in RTHA shows good results in most patients when the bicortical contact threshold of 2 cm in an anteroposterior radiograph of the femur is reached. Авторы данного исследования представляют случай разобщения фиксирующего механизма полиэтиленового вкладыша после эндопротезирования с использованием имплантата с сохранением задней крестообразной связки. Данное редкое осложнение было диагностировано у пациентки через 7 суток после неосложненного стандартного тотального эндопротезирования правого коленного сустава, выполненного опытным хирургом по поводу терминального гонартроза. {"title":"Long-term result of treatment of a patient with an extensive circular soft tissue loss of the lower third of the leg (clinical case)","authors":"V. Patient B., 18 years old, received a combined injury to the head and limbs as a result of falling under a moving train. The presented clinical observation demonstrates the possibility of successful replacement of an extensive circular defect of the lower third of the leg using sequentially free and non-free blood-supplied tissue complexes. The use of a crossed non-free sural fasciocutaneous flap to close the defect that arose after partial necrosis of a previously transplanted free blood-supplied anterolateral thigh flap made it possible to completely restore the skin of the lower third of the leg. The main advantages of this type of plastic surgery are, of course, the relative technical simplicity of the surgical intervention and the short duration of the operation. At the same time, the main disadvantage of this method of closing a wound defect on the distal parts of the lower extremity is the need to keep the patient on strict bed rest with the forced position of the legs for three weeks before cutting off the flap. Revision anterior cruciate ligament (ACL) reconstruction is becoming an increasingly common knee surgery procedure due to the annual increase in the number of primary ACL reconstructions performed. Graft choice and determining the stages of surgical treatment using a preoperative assessment of the possibility of performing the most anatomical revision tunnels and their relative position with the primary tunnels are the main factors that influence the results of treatment. \u0000Aim of the study – сomparative assessment of the results of using autografts from the hamstring tendons and the peroneus longus tendon as part of a one-stage revision reconstruction of the anterior cruciate ligament. \u0000Material and methods. A retrospective analysis was performed on 36 patients who underwent revision ACL reconstruction. The study group \"PLT\" (n=19) - patients with an autograft from the peroneus longus tendon and the comparison group - \"HT\" (n=17) - a graft from the hamstring tendons. Subjective and objective assessments were carried out using the KOOS, IKDC and Lysholm scales. the position of the central entry points into the primary and revision canals was also determined and assessed. Results. There were no statistically significant differences in the objective assessment of knee stability. Significantly better results of subjective assessment of knee joint function using the Lysholm and KOOS scales were obtained in the PLT group (p=0.042 and p0.001, respectively). The position of the revision tunnels corresponded to the standard values, however, the position of the femoral tunnel had a slight displacement cranially and anteriorly. It was also revealed that the diameter of the PLT graft was statistically significantly larger than the HT graft (p0.001). \u0000Conclusion. One-stage revision ACL reconstruction is a safe and effective surgical procedure that provides satisfactory objective and subjective clinical results. To conduct a comparative evaluation of the results of surgical treatment of patients with early stage osteonecrosis of the femoral head using different methods of core decompression and autologous bone grafting of the femoral head. \u0000Material and methods. A comparative analysis of the results of treatment of patients with early stages of ONFH was carried out. The patients were divided according to the method of treatment into two groups: control and main. In the control group (n=19), surgical treatment was performed using open decompression and autologous bone grafting of the femoral head using the Rosenwasser M.R. technique \"light bulb\". The main group (n=17) included patients whose surgical treatment was performed using the developed method of combined impaction autoplasty of the femoral head (RF patent for invention No. 2583577 dated April 13, 2016). Clinical and functional evolution of treatment results was performed using the Harris Hip Score (HHS) and Western Ontario and McMaster University Osteoarthritis Index (WOMAC). Evaluation was performed preoperatively and at 3, 6, and 12 months postoperatively. \u0000Results. The comparative analysis showed a significant difference in clinical and functional results after surgical treatment in patients of the control and main groups: patients of the main group demonstrated the best results at all periods of observation, however, their most pronounced difference was noted at a follow-up period of 6 months after surgery, with further preservation of this trend up to 12 months after surgery. \u0000Conclusion. The number of surgical interventions on the musculoskeletal system increases every year. Simultaneously with the increasing availability of orthopedic care, the number of patients with orthopedic infection is increasing. Etiological agents in the development of orthopedic infection can be representatives of various taxonomic groups. Staphylococcus aureus and various types of coagulase-negative staphylococci (СоNS), including S. epidermidis and S. lugdunensis, together are pathogens in 70% of cases \u0000Aim of the study. To analyze the dynamics of the spectrum of leading pathogens isolated from patients of the department of purulent osteology for the period from 2011 to 2022. \u0000Materials and methods. A retrospective analysis of the spectrum of microorganisms isolated from patients treated in the department of purulent surgery from January 1, 2011 to December 31, 2022 was performed. The leading pathogens included microorganisms whose share in the spectrum exceeded 3.5%. Of the isolated pathogens, 48.8% were the only etiological agents, 51.2% determined the presence of microbial associations. \u0000Results. Over a 12-year period, 10,327 strains of various bacterial species were identified. The leading microorganisms isolated from patients with orthopedic infection were Staphylococcus spp., Enterococcus spp., Propionibacterium spp., Pseudomonas aeruginosa, Corynebacterium spp., Streptococcus spp., Klebsiella spp. and Acinetobacter spp. Several trends were revealed in the dynamics of isolated pathogens during the analyzed period: a decrease in the proportion of S. aureus, Enterococcus spp., P. aeruginosa, Acinetobacter spp. and an increase in CoNS and Corynebacterium spp. \u0000Conclusion. The study showed that for 12 years gram-positive bacteria, mainly belonging to the genus Staphylococcus, were included in the spectrum of the main pathogens of orthopedic infection in patients with purulent surgery of our center. At the same time, there was a significant decrease in the proportion of S. aureus strains and an increase in the frequency of isolation of various types of cons. The patients were divided according to the method of treatment into two groups: control and main. In the control group (n=19), surgical treatment was performed using open decompression and autologous bone grafting of the femoral head using the Rosenwasser M.R. technique \"light bulb\". The main group (n=17) included patients whose surgical treatment was performed using the developed method of combined impaction autoplasty of the femoral head (RF patent for invention No. 2583577 dated April 13, 2016). Clinical and functional evolution of treatment results was performed using the Harris Hip Score (HHS) and Western Ontario and McMaster University Osteoarthritis Index (WOMAC). Evaluation was performed preoperatively and at 3, 6, and 12 months postoperatively. \u0000Results. The comparative analysis showed a significant difference in clinical and functional results after surgical treatment in patients of the control and main groups: patients of the main group demonstrated the best results at all periods of observation, however, their most pronounced difference was noted at a follow-up period of 6 months after surgery, with further preservation of this trend up to 12 months after surgery. \u0000Conclusion.
