{"title":"Inpatient rehabilitation outcomes in neoplastic spinal cord compression vs. traumatic spinal cord injury.","authors":"Sevgi Ikbali Afsar, Sacide Nur Saraçgil Cosar, Oya Umit Yemişçi, Hüma Bölük","doi":"10.1080/10790268.2020.1794713","DOIUrl":"https://doi.org/10.1080/10790268.2020.1794713","url":null,"abstract":"<p><p><b>Objective:</b> To compare neurological and functional outcomes, and complications of patients with neoplastic vs traumatic spinal cord injury (SCI) after in-patient rehabilitation.<b>Design:</b> This study is a retrospective analysis.<b>Setting:</b> In-patient rehabilitation unit of a tertiary research hospital.<b>Participants:</b> A total of 252 patients with a SCI were included; 43 with neoplastic SCI (mean age: 60.9 ± 15.7 years, 60.5% were males) and 209 with traumatic SCI (mean age: 43.1 ± 16.8 years, 71.3% were males).<b>Outcome measures:</b> Comparisons were made of demographic characteristics, etiology, American Spinal Injury Association (ASIA) impairment scale, functional independence measurement (FIM) and Functional Ambulation Categories (FAC) scores, length of stay (LOS), bladder independence, medical comorbidities and complications in both groups.<b>Results:</b> Patients with neoplastic SCI were significantly older than those with traumatic SCI (P < 0.01). No difference was present between the groups in terms of sex and lesion level (P > 0.05). Incomplete SCI was significantly higher in the neoplastic group when compared with the traumatic group (P < 0.01). The LOS was significantly shorter in the neoplastic group than traumatic group (34.8 ± 41.03 vs. 60.02 ± 53.1, P < 0.01). There were no differences in the admission FIM scores (69.3 ± 24.7 vs. 58.7 ± 18.9, P > 0.05), discharge FIM scores (82.1 ± 25.1 vs. 74.02 ± 23.3, P > 0.05) and FIM efficiencies (0.43 ± 0.72 vs. 0.36 ± 0.51, P > 0.05) for the neoplastic and traumatic groups, respectively. However, neoplastic SCI patients demonstrated lower FIM gains compared to traumatic patients (12.9 ± 11.9 vs. 15.4 ± 15.2, P < 0.05). During rehabilitation, urinary tract infection (48.4% vs. 69.4%) and decubitus ulcer (11.6% vs. 35.9%) were significantly more common in the traumatic group than the neoplastic group (P < 0.05).<b>Conclusion:</b> Neoplastic SCI patients who commonly present at rehabilitation units exhibit different characteristics from traumatic SCI patients but the rehabilitation results are similar. {"title":"Assessment of the unmediated relationship between neurological impairment and health-related quality of life following spinal cord injury.","authors":"Manuel Zwecker, Eyal Heled, Vadim Bluvstein, Amiram Catz, Ayala Bloch, Gabi Zeilig","doi":"10.1080/10790268.2020.1788884","DOIUrl":"https://doi.org/10.1080/10790268.2020.1788884","url":null,"abstract":"<p><p><b>Objective:</b> To assess the unmediated association between neurological impairment and quality of life (QoL) among persons with spinal cord injury (SCI), in the context of both early post-acute and long-term rehabilitation settings.<b>Design:</b> An observational prospective cross-sectional study.<b>Setting:</b> Two neurological rehabilitation centers, specializing in spinal cord injury, within a university hospital.<b>Methods:</b> 156 adults with spinal cord injury in the early post-acute and chronic stages of rehabilitation.<b>Outcome Measures:</b> Participants were categorized into tetraplegia or paraplegia groups based on neurological level of injury, and into complete or incomplete groups based on American Spinal Injury Association (ASIA) Impairment Scale (AIS) score. QoL was assessed by means of the World Health Organization Quality of Life Assessment-BREF (WHOQOL-BREF), Satisfaction with Life Scale (SWLS), Life Satisfaction Questionnaire (LISAT-9), and Personal Well-Being Index (PWI).<b>Results:</b> WHOQOL-BREF and PWI predicted Level of Injury group classification and WHOQOL-BREF predicted Setting group classification. None of the questionnaires differentiated between the Type of Injury groups. At the early post-acute stage of rehabilitation, the QoL scores of participants with tetraplegia and paraplegia did not differ significantly, while significantly higher QoL scores were revealed in paraplegics in the long-term setting group.<b>Conclusions:</b> A direct, unmediated effect of severity of neurological impairment was revealed with the WHOQOL-BREF, which distinguished between the tetraplegia and paraplegia groups, but not between the complete and incomplete injuries. MR imaging showed intracanalicular mass lesions with compression of the spinal cord and allowed precise localization of lesions in the extradural or intradural space. Diagnosis was straightforward for the patients with the posterior and anterior epidural disc fragments, whereas various differential diagnostic considerations were entertained for the patient with the intradural mass lesion. All patients underwent surgical removal of the sequestered disc fragments, and recovered full motosensory function. Surgical repair of the dura mater due to CSF leak was required for the patient with intradural disc herniation.<b>Conclusion/clinical relevance:</b> Posterior and anterior epidural, and intradural disc migration may manifest with clinical symptoms indistinguishable from those associated with non-sequestered lumbar disc hernias. Missed, migrated disc fragments can be implicated as a cause of low back pain, radiculopathy or cauda equina syndrome, especially in the absence of visible disc herniation. Low-molecular-weight heparin (LMWH) may be used in SCI patients who develop DVT, but can lead to subcutaneous bleeding. If subcutaneous bleeding occurs, then lymphedema, cellulitis, muscle or tendon tearing, or baker's cyst rupture should be considered in the differential diagnosis.<b>Findings:</b> Herein, we present a 61-year-old female patient who was hospitalized for rehabilitation due to paraplegia, and used LMWH due to DVT development. The patient suddenly developed pain, swelling, and discoloration of the left lower extremity. Although subcutaneous hemorrhage was considered initially, ultrasound and MRI revealed a ruptured Baker's cyst. In addition to supportive therapy, ultrasound-guided aspiration was performed.<b>Conclusion/Clinical Relevance:</b> In this report, we present a case of clinically severe Baker's cyst rupture, which occurred in the lower extremity of a SCI patient using LMWH due to DVT in the same extremity. Opioid medications have historically been prescribed to this population with great frequency. As opioid abuse disorder becomes an ever-worsening public health issue, more attention must be placed upon non-opioid options. This paper reviews non-opioid medications to be considered when treating spinal cord injury-related pain. The pertinent literature is reviewed, and the advantages and pitfalls of various medication options are discussed in the complicated context of the individual with a spinal cord injury.<b>Methods:</b> Peer-reviewed journal articles and medication package insert data are reviewed.<b>Results:</b>. The non-opioid medications with the greatest evidence for efficacy in the treatment of chronic spinal cord injury-related pain are drawn from the antiepileptic drug and antidepressant categories though the specific selection must be nuanced to the particular individual patient. {"title":"Test-retest reliability of the Valsalva maneuver in spinal cord injury.","authors":"Michael J Berger, Tristan Dorey, Hirmand Nouraei, Andrei V Krassioukov","doi":"10.1080/10790268.2020.1798134","DOIUrl":"https://doi.org/10.1080/10790268.2020.1798134","url":null,"abstract":"<p><p><b>Objective:</b> To determine the test-retest reliability of quantitative and qualitative baroreflex sensitivity (BRS) parameters derived from the Valsalva maneuver (VM) in individuals with traumatic cervical SCI.<b>Design:</b> Test-retest reliability.<b>Setting:</b> Tertiary rehabilitation center.<b>Participants:</b> Fourteen participants with cervical SCI (ranging from C3-C8 neurological level).<b>Outcome Measurements:</b> Beat-to-beat systolic blood pressure (SBP) traces (finger photoplethysmography) were obtained during a 15-second forced expiration at two time points (7.6 ± 2.9 days between sessions) to assess VM reliability. Test-retest reliability of BRS metrics from derived from the VM (Valsalva ratio; VR, pressure recovery time; PRT, vagal baroreflex sensitivity; BRSv, adrenergic baroreflex sensitivity; BRSa<sub>1</sub>, and total recovery; TR) were assessed by intra-class correlation coefficient (ICC, with 95% confidence interval; CI) and by qualitative reproducibility (V, N, or M pattern).<b>Results:</b> ICCs for quantitative parameters were (CI): VR = 0.894 (0.703-0.965), TR = 0.927 (0.789-0.976), BRSa<sub>1</sub> = 0.561 (0.149-0.911), PRT = 0.728 (0.343-0.904), BRSv = 0.243 (-0.309-0.673). Qualitatively, 12 subjects (85.7%) demonstrated reproducible VM patterns at both time points (3 \"M\" pattern, 8 \"V\" pattern and one \"N\" pattern).<b>Conclusion:</b> VR (a measure of cardiovagal function) and TR (a measure of sympathetic adrenergic function) are reliable quantitative parameters that can be derived from SBP response to VM in participants with SCI. Qualitative waveform analysis was reproducible in 12/14 participants. Seventy-seven (53.1%) veterans did not visit any ED. Of 68 (46.9%) veterans with ≥ one ED visit, 20 (29.4%) visited the VA ED only, 34 (50.0%) visited non-VA EDs only, and 14 (20.6%) visited both VA and non-VA EDs. Among 212 Veterans for whom hospitalization data were available, 247 hospitalizations were identified: 82 (33.2%) non-VA hospitalizations and 165 (66.8%) VA hospitalization with a mean of 1.17 (±1.62) hospitalizations/person. One hundred-seven (50.5%) veterans had no hospitalizations. Of 105 veterans with ≥ one hospitalization, 58 (55.2%) were hospitalized at the study VA hospital, 15 (14.3%) at a non-VA hospital, and 32 (30.5%) at both VA and non-VA hospitals.<b>Conclusion:</b> Non-VA ED and hospital usage among veterans with SCI/D occurred frequently. While <i>macro</i>vascular remodeling and function after SCI is well documented, changes in the <i>micro</i>vascular structure and function are comparably understudied, but importantly predict CVD risk. Specifically, the integrity of venoarteriolar (VAR), myogenic (MYO) and maximal vasodilation responses are largely unknown after SCI, especially in persons with tetraplegia (TP) at highest risk of CVD. This is the first to examine the differences in VAR (cuff inflation), MYO (limb dependency) and maximal vasodilation responses of the microvasculature between able bodied (AB) versus those with TP and paraplegia (PP).<b>Design:</b> Observational.<b>Setting:</b> Laboratory.<b>Participants:</b> Eight AB, 6 TP, and 8 PP persons.<b>Interventions:</b> One forearm and calf were treated topically with lidocaine 2.5%/prilocaine 2.5% while contralateral limb served as a control. Laser doppler flowmeters were applied over treated and control sites during limb dependency, cuff inflation and local skin heating (Tloc) up to 42°C.<b>Outcome measures:</b> Skin vascular resistance (SkVR) change with cuff inflation and limb dependency and maximal cutaneous vascular conductance (CVC) during local heating.<b>Results:</b> Change in SkVR was not significantly different between groups or extremity (upper vs. lower) during cuff inflation or limb dependency. LE fractures are the most common fracture location in SCI/D. Physical therapists (PTs) and occupational therapists (OTs) play an important role in rehabilitating LE fractures in Veterans with SCI/D. This paper describes their role in assisting persons with SCI/D and LE fractures to return to previous function and levels of participation.</p><p><strong>Design: </strong>Cross-sectional semi-structured interviews were conducted by telephone. Setting: VA SCI centers.</p><p><strong>Participants: </strong>Purposive sample of therapists (PTs and OTs) experienced in LE fracture rehabilitation in SCI/D Interventions: NA.</p><p><strong>Outcome measures: </strong>Coding of responses used a data-driven thematic and deductive approach, dictated by a semi-structured interview guide addressing the entire treatment process.</p><p><strong>Results: </strong>Participants strongly advocated for early PT/OT involvement in post-fracture rehabilitation in order to recommend braces and devices to minimize skin breakdown, and needs for patient equipment, skills training and/or caregiver assistance resulting from post-fracture mobility changes. Seating specialists should be involved in post-fracture seating assessments in wheelchair users to address changes in alignment, deformities, limb length discrepancies and/or seating posture during and following fracture management.</p><p><strong>Conclusion: </strong>PTs and OTs are critical in rehabilitating LE fractures in persons with SCI/D and LE fractures, bringing expertise in patient function, ambulatory status, transfer strategies, mobility equipment, spasticity, lifestyle, and home and caregiver support. {"title":"Comparison of the gut microbiome composition among individuals with acute or long-standing spinal cord injury vs. able-bodied controls.","authors":"Jia Li, William Van Der Pol, Mualla Eraslan, Amie McLain, Hatice Cetin, Baris Cetin, Casey Morrow, Tiffany Carson, Ceren Yarar-Fisher","doi":"10.1080/10790268.2020.1769949","DOIUrl":"10.1080/10790268.2020.1769949","url":null,"abstract":"<p><p><b>Objective</b>: Compare the gut microbiome composition among individuals with acute spinal cord injury (A-SCI), long-standing SCI (L-SCI), vs. able-bodied (AB) controls.<b>Design</b>: Cross-sectional study.<b>Setting</b>: The University of Alabama at Birmingham.<b>Participants</b>: Seven adults with A-SCI (36 ± 12 years, 2F/5M, C4-T10, and American Spinal Injury Association Impairment Scale [AIS] A-D), 25 with L-SCI (46 ± 13 years, 6F/19M, C4-L1, and AIS A-D), and 25 AB controls (42 ± 13 years, 9F/16M).<b>Methods</b>: Stool samples were collected after a median of 7 days and 18 years after injury in the A-SCI and L-SCI groups, respectively. Gut microbiome composition was analyzed using the 16S rRNA sequencing technique and QIIME software. The abundances of bacteria communities among groups were compared using the Kruskal-Wallis test adjusted for age.<b>Results</b>: Several alpha diversity indices were different among groups (Chao1, Observed species, and Phylogenetic Diversity), but not others (Shannon and Simpson). Beta diversity differed among each pair of groups (<i>P</i> < 0.05). A number of microbial communities were differentially abundant among the groups (<i>P</i> < 0.05).<b>Conclusion</b>: Our results revealed differences in the gut microbiome composition among groups. Compared to the AB controls, the SCI groups demonstrated microbiome profiles that shared features linked to metabolic syndrome, inflammation-related bowel disorders, depressive disorders, or antibiotics use, whereas the L-SCI group's microbiome included features linked to reduced physical activity compared to the A-SCI and AB controls.
