Pekyavas","doi":"10.3109/10582452.2014.883033","DOIUrl":"https://doi.org/10.3109/10582452.2014.883033","url":null,"abstract":"Abstract Objectives: The aim of this study was to investigate the inter-rater and intra-rater reliability of pressure pain threshold [PPT] measurements in the supraspinatus muscle tendon, anterior talofibuler ligament, and the extensor digitorum communis muscle belly in healthy young subjects. Methods: A total of 100 healthy undergraduates participated in this study. Two examiners had three trials of measuring the supraspinatus muscle tendon, anterior talofibuler ligament, and extensor digitorum communis muscle belly on the dominant side, which were marked by the first examiner [E1]. Three consecutive measurements were taken with a 60-s interval between trials. The average of three measurements was calculated for each region. After a 60-min interval, the same procedure was repeated by the second examiner [E2] on the same day. Results: Our results indicated that E1 and E2 showed high intra-rater reliability. Significant difference was found between E1 and E2 in anterior talofibular ligament and extensor digitorum communis muscle belly threshold average measurements [p < 0.05], but no significant difference was found at supraspinatus muscle tendon [p > 0.05]. Low significant correlation and reliability was found between E1 and E2 for all measurement sites. Conclusions: Our study results indicated that intra-rater reliability of PPT measurements on tendon, ligament, and muscle belly was high. We found that inter-rater reliability was low in the anterior talofibular ligament and supraspinatus muscle tendon, and adequate in muscle belly. Measurements by one examiner were more reliable than measurements between examiners. Methods: Thirty-six LSS patients [14 males and 22 females] and 30 age- and gender-matched healthy normal controls [13 males and 17 females] were enrolled in the study. Sleep disturbance was assessed using the Pittsburgh Sleep Quality Index [PSQI]. Pain was measured using a visual analog scale [VAS, 0–100 mm]. Psychological status and physical disability were assessed using the Hospital Anxiety and Depression Scale [HADS] and the Oswestry Disability Index [ODI]. Results: There were no significant differences between the LSS patients and the controls in terms of age or sex. The LSS patients had higher PSQI, HADS scores compared to the controls [p < 0.05]. Sleep disturbance [PSQI total score greater than 5] was positively correlated with VAS, HADS, and ODI [p < 0.05]. Conclusions: Sleep disturbance was associated with higher levels of pain, disability, depression, and anxiety. In comparison to chronic pain, few studies have investigated sleep quality in myofascial pain syndrome [MPS]. This study investigates the association between the MPS and the quality of sleep. Methods: We investigated 100 patients who complained of muscle pain with diagnostic criteria matching that of MPS and exhibited no major medical, psychiatric, or neurological disorders, and 122 subjects who were recruited from a Health Screening Center. Participants completed the following questionnaires: the Pittsburgh Sleep Quality Index [PSQI], the Insomnia Severity Index [ISI], Epworth Sleepiness Scale, Sleep Apnea scale of the Sleep Disorders Questionnaire, and the Beck Depression Inventory. The participants were recorded on their first visiting day in an outpatient clinic. Results: The PSQI and ISI scores of patients with MPS were significantly higher than that of control subjects [PSQI: 12.3 ± 7.1 versus 5.4 ± 2.6, p < 0.001, ISI: 16.1 ± 9.2 versus 6.7 ± 4.3, p < 0.001]. Logistic regression analysis adjusted for age, sex, body mass index, the Sleep Apnea scale of the Sleep Disorders Questionnaire score, and Beck Depression Inventory score revealed that the odds ratio for MPS according to an increase in the PSQI and ISI score [PSQI: 3.60, 95% confidence interval: 2.33–7.84 versus ISI: 1.22, 95% confidence interval: 1.18–1.36]. Conclusions: This study showed that poor sleep quality is significantly associated with MPS. The aim of this study was to investigate the prevalence of some musculoskeletal complaints of dentists in Iran. Methods: Four hundred and forty-six dentists in Tehran city, Iran were surveyed to determine the prevalence of musculoskeletal problems. We administrated a Persian–language version of the Nordic Questionnaire for this purpose. Results: Data gathered from Iranian dentists showed that most dentists [81%] reported pain in at least one joint. The most prevalent musculoskeletal disorders during the previous 12 months were reported in the neck [64.4%], upper back [57%] and shoulder [50.7%]. Knee disorder and hips/thighs disorder were statistically associated with years of practice [p < 0.05] and wrist disorder was associated with weekly work time [p = 0.027]. Conclusion: The study suggests that Iranian dentists suffer from various musculoskeletal disorders. This research was designed to investigate the non-specific musculoskeletal pain and vitamin D deficiency in female nurses working in Shahid Beheshti Hospital in Kashan, Iran in 2013. Methods: In this cross-sectional study, 200 female nurses were investigated for their 25-hydroxy vitamin D level [25-OH] and musculoskeletal non-specific pain. The Extended Nordic Musculoskeletal Questionnaire [NMQ-E] was used to determine the existence of pain in nine anatomic regions. The relationship between pain in every region and vitamin D deficiency was analyzed by the Mann–Whitney U and chi-square tests. All analyses were conducted with the SPSS computer package version 16. Results: The mean age of the nurses was 32.05 years. The mean of 25-OH vitamin D level was 16.96 ng/ml. The 178 nurses [89%] had vitamin D deficiency; in 91 nurses [45.5%] the deficiency was severe. The 178 nurses had pain in at least one region in the previous month. The pain in their upper and lower back and in ankles and feet had a significant relationship with vitamin D deficiency. The 25-OH, vitamin D level, and age could explain the 6.2% of total painful regions. Conclusions: The study showed that both hypovtaminosis D and musculoskeletal pain were common among nurses. Biomechanical changes in the vertebral column caused by changes in the lumbosacral angles [LSAs] may lead to LBP. The purpose of this study was to assess body mass index [BMI] and LSAs in patients with LBP and investigate the association between LBP, LSAs and BMI. Methods: Lumbar lordotic angle [LLA], LSA, sacro-horizontal angle [SHA] and sacral inclination angle [SIA] were measured in 117 patients with chronic LBP and 85 healthy normal controls [HNCs] by means of lumbosacral radiography. In addition, association between LSAs, BMI and LBP was investigated. Results: There were no significant differences between patients and HNCs regarding LSAs and BMI. LLA was lower in male patients with LBP compared to male HNCs without LBP [p = 0.013]. In addition, SIA [p = 0.002] and BMI [p = 0.006] were higher in female patients with LBP compared to male patients with LBP. It was found that an increase in LLA increased the risk of having LPB by approximately 1.04-folds [ranging from 1.01 to 1.08; p = 0.045]. On the other hand, no association was found between LSAs and BMI. Conclusion: Changes in LSAs may cause LBP. An increase in LLA may be influential in increasing the risk of LBP. Findings: The present report details the case of a 34-year-old physically fit male who presented with chronic pain, localized in the left buttock. This pain had persisted for 3 years. The patient consulted multiple physicians, exhausted conservative treatment modalities, and underwent lumbar laminectomy without success. When treated with a single PRP injection, the patient had significant improvements in pain and quality of life. Methods: We asked the outpatients attending all the clinical departments of Mie University Hospital to complete a questionnaire that included a visual analog scale [VAS] for evaluating pain within the last week and the Maudsley Personality Inventory. Results: Out of a total of 5013 outpatients, 3055 answered the questionnaire [collection rate: 60.9 percent]. Based on the VAS, subjects with a VAS = 0 mm were defined as the no pain group [n = 2,196], those with 0 mm < VAS < 100 mm as the pain group [n = 839] and subjects with a VAS = 100 mm as the worst pain group [n = 20]. Each score of the Maudsley Personality Inventory was compared among the three groups. The E score of the pain group was significantly lower than that of the no pain group [p < 0.01]. The N score of the pain group was significantly higher than that of the no pain group [p < 0.01]. The L score of the worst pain group tended to be higher, but there was no significant difference with that of the other two groups. Conclusion: There was a significant relationship between pain and personality. In clinic, while etiologies of chronic knee pain are generally known, its effects on cognitive functions are not fully understood. This study aims to measure and evaluate the relationship between cognitive function and chronic knee pain. Methods: Ninety-eight patients between 18 and 70 years of age having knee pain for at least 6 months were included in the study. Two groups were divided between A1 and A2, both patients of chronic knee pain. Cognitive functions were evaluated in the A1 group before treatment and in the A2 group after taking treatment. Both groups had the same number of patients [N = 98]. The Montreal Cognitive Assessment [MOCA] was applied to the patients before and after treatment. The patients with a MOCA score 26 and over were accepted as normal in terms of cognitive functions. Results: The MOCA score of the A2 group was significantly higher than that of A1 group. Visual analog scale values in group A2 were significantly decreased compared to A1. Although orientation scores were decreased, patients in group A2 showed a statistically significant increase in cognitive functional indicators like visual functions, language, abstract thinking, and delayed recall compared with group A1. Conclusions: The chronic knee pains might lead to cognitive function disorders in the long term. This can be prevented with the effective treatment of pain so that quality of life of patients can be increased.
