Patients were contacted at least one year postoperatively and assessed using standardized patient-reported outcome measures (PROMs), including Visual Analog Scale (VAS) for pain, Derkash classification, Disabilities of the Arm, Shoulder, and Hand (DASH) score, Michigan Hand Outcomes Questionnaire (MHQ), and Cervical Brachial Symptom Questionnaire (CBSQ). Data were analyzed using t-tests and linear regression models.</div></div><div><h3>Results</h3><div>The NTOS site of involvement was supraclavicular (SC) in 12 (27.3%) patients, infraclavicular (IC) in 16 (36.4%), and both (SC and IC) in 16 (36.4%). A distal compression site was treated in 20 (45.5%) patients. The overall success rate, as assessed through the Derkash classification, was 87.5%. The mean postoperative VAS score (2.6 <u>+</u> 3.2) was significantly improved from the preoperative VAS score (8.0 <u>+</u> 2.9). Mean postoperative DASH, MHQ, and CBSQ scores were 32.8 ± 21.8, 68.1 ± 26.5, and 39.2 ± 32.6, respectively. Multiple linear regression analysis showed that a longer duration of symptoms before surgery was the most significant factor for a higher mean postoperative VAS pain score.</div></div><div><h3>Conclusion</h3><div>Longer time to surgery was associated with higher postoperative VAS pain scores, suggesting that earlier surgical intervention in NTOS patients may be beneficial. A comparative trend analysis of five facial aesthetic procedures, rhinoplasty, blepharoplasty, facelift, brow lift, and other head and neck aesthetic procedures, showed that resident case volume increased significantly for rhinoplasty (4.35% annually, p=0.046), brow lift (4.22%, p=0.002), and other head and neck procedures (5.60%, p=0.048), while no significant growth was observed for blepharoplasty or facelift. In contrast, U.S. market demand for blepharoplasty and facelift increased by 4.90% and 5.70% annually, respectively. This study seeks to determine the impact of performing partial trapezoidectomy in the treatment of STT OA.</div></div><div><h3>Methods</h3><div>An IRB-approved retrospective review of patients who received trapeziectomy for isolated STT OA was conducted. Patients who received partial trapezoidectomy (PT group) were matched to patients who did not (NT group) via propensity-score matching. Patient demographics, clinical findings, and patient-reported outcomes (PROMs) were collected. Second metacarpal subsidence was measured. Radiographic and clinical outcomes were compared between the PT and NT groups.</div></div><div><h3>Results</h3><div>Fifty-three patients underwent 56 trapeziectomies, with 28 hands receiving partial trapezoidectomy. The median pain score reduction was 5 [3−7] in the PT group and 2.75 [1–5.9] in the NT group (p=0.03). QuickDASH scores decreased from 47.73 [30.68–67.61] to 11.36 [6.82–29.55] (p=0.002) and from 60.23 [50.57–75] to 23.87 [5.12–33.52] (p=0.005) for PT and NT, respectively. VR12 scores improved from 33.86 [30.05–40.62] to 46.77 [43.7–53.68] for PT (p=0.0003) and from 37.21 [30.61–42.84] to 51.97 [37.7–55.7] for NT (p=0.05). Change in QuickDASH (p=0.66) and VR12 scores (p=0.88) were not significantly different between groups. Median second metacarpal subsidence was 12.6% [−0.6–31.5] and 3.5% [−15.9–15.4] respectively for PT and NT (p=0.031). There was no correlation between subsidence and pain in either group (PT: p=0.43, NT: p=0.31).</div></div><div><h3>Conclusions</h3><div>Both procedures improved pain and PROMs for STT OA, with partial trapezoidectomy demonstrating superior pain reduction compared to trapeziectomy alone. Schols","doi":"10.1016/j.bjps.2025.07.014","DOIUrl":"10.1016/j.bjps.2025.07.014","url":null,"abstract":"<div><h3>Objective</h3><div>The purpose of this review is to investigate the speech outcomes of speech therapy only versus speech therapy in combination with Furlow palatoplasty for the treatment of velopharyngeal insufficiency (VPI) in submucous cleft palate (SMCP).</div></div><div><h3>Design</h3><div>Systematic review and 2 meta-analyses of speech therapy only and speech therapy in combination with Furlow palatoplasty in the treatment of VPI in SMCP.</div></div><div><h3>Setting</h3><div>Secondary and tertiary care centres in 10 different countries spread over 6 continents.</div></div><div><h3>Patients, participants</h3><div>Articles were selected after an elaborate literature search using PubMed, Embase and the Cochrane Library.</div></div><div><h3>Interventions</h3><div>Speech therapy only and speech therapy in combination with Furlow palatoplasty</div></div><div><h3>Main outcome measure</h3><div>The primary outcome measure of this study is the reduction of VPI-associated speech difficulties after treatment.</div></div><div><h3>Results</h3><div>Speech therapy only showed a risk difference (RD) of 0.015 to 0.036 for the presence of hypernasality; 0.045 for the presence of nasal emission; 0.015 for the presence of compensatory articulation and 0.089 for the presence of unintelligible speech. Speech therapy in combination with Furlow palatoplasty showed RDs of 0.46; 0.29 and 0.37 for the presence of hypernasality, clinically relevant hypernasality and nasal emission, respectively.</div></div><div><h3>Conclusion</h3><div>Both speech therapy only and speech therapy in combination with Furlow palatoplasty demonstrate a decrease in VPI-associated speech difficulties in SMCP. According to the current findings, speech therapy should accompany surgical correction by Furlow for the treatment of SMCP. Following resection of a pre-sternal DFSP, a DIEP flap was used for chet wall reconstruction, with the arterial anastomosis performed robotically. The procedure was completed safely, marking a significant step in integrating robotic technology into open microsurgery. However, their \"black box\" nature often prevents surgeons from understanding the reasoning behind AI-generated recommendations, limiting clinical adoption and trust. This manuscript presents Explainable Artificial Intelligence (XAI) approaches that can transform opaque AI systems into transparent decision support tools for plastic surgeons. We outline methods for individual case explanations, population-level insights, and visualization techniques specifically relevant to plastic surgery applications. Webster, known as the \"father of plastic surgery education\" in the United States, established Columbia University's first plastic surgery professorship and residency program. His educational philosophy was shaped by formative training with William Halsted, Sir Harold Gillies, and Vilray Blair, combined with World War I service and international teaching experience. This study examines Webster's case logs from 1943-1948, digitized from the Columbia University Medical Center Archives, to understand his educational approach and surgical practice. We categorized 820 documented procedures by type, anatomical location, and reconstructive technique while analyzing accompanying illustrations and patient notes. Webster's practice focused primarily on head, face, and neck procedures (51.6%), with ear reconstruction (107 cases), nasal procedures (94 cases), and lip repairs (46 cases) predominating. Hand and upper extremity procedures constituted 15.3% of cases, while breast and torso procedures accounted for just 5.5%. Common surgical techniques included split-thickness grafting (57 cases), pedicle flaps (121 cases), and tube pedicles (28 cases). Major etiologies included burn reconstruction (13.7%), congenital anomaly correction (8.7%), and scar revision (16.3%). Webster's educational methodology emphasized detailed visual documentation through illustrations co-created with painter Ivan Webster, hands-on technical training, and systematic recording of clinical judgment through resident notes.
