Afterwards she was soporific with signs of impaired consciousness and required intubation and intensive medical care surveillance. A cerebral fat embolism syndrome could be detected as the reason, which was characterized by acute respiratory insufficiency, neurological symptoms and petechiae. Subsequently, definitive treatment was performed by intramedullary nailing. Timely and reliable diagnostics are obligatory to be able to carry out the treatment of non-unions in a patient-specific and efficient manner. Contrast-enhanced ultrasound (CEUS) is an important interface between clinical signs, imaging investigations and the results of the paraclinical diagnostics, e.g. C‑reactive protein (CRP) and leukocyte count. It can display the microperfusion inside the non-union gap in real time and provide valuable information for exclusion of an infection or on the healing progress after revision surgery. Based on a thorough clinical and ultrasound examination 25 ligamentous injuries of the anterior talofibular ligament (ATFL, 9.8%), 8 osseous ligament avulsions (6.3%), 7 injuries of the lateral calcaneocuboid ligament (CC ligament, 5.6%), 5 lesions of the calcaneofibular ligament (CFL) each combined with ATFL injuries (3.9%) and 1 syndesmosis injury (0.6%) were recorded. This didactic article presents a comprehensible ultrasound examination course in an instructive manner from the clinical practice. Identification of osseous and ligamentous landmarks is often difficult clinically and even in an anatomical preparation but it can easily be done using ultrasound. Hemarthrosis of the ankle has a high predictive value with respect to an associated ligament tear and is easily detectable by ultrasound. Injuries of the ATFL, the anterior syndesmosis and damage to the lateral malleolar epiphysis can easily be clearly distinguished sonographically. A positive decompression test confirms the distal avulsion of the ATFL. Sonographic stability testing of the AFTL can be carried out immediately after the clinical examination in the same position and visualized on the monitor. The talofibular advance can be measured on the monitor. Instability of the anterior syndesmosis can be visually demonstrated on the monitor using the sonographic Frick test. Bodelschwingh Foundation Bethel can look back on a tradition of more than 150 years in the treatment of people with disabilities. The trauma patient collective is regularly characterized by the most severe mental and physical developmental disorders and (multiple) disabilities, with and without behavioral disorders and concomitant internal diseases. This special combination requires treatment and indication assessment both medically and in nursing, which must be specifically weighed up and adapted in all treatment steps. Treatment guidelines or recommendations for this patient group do not exist in the literature. Furthermore, the treatment of fractures in people with disabilities cannot always follow established concepts. Due to a high postoperative complication rate, conservative treatment plays a crucial role. The decision for surgical treatment must be made on an interdisciplinary and individual basis, taking all factors into account. The recognition and treatment of life-threatening disorders and the assessment of the severity of the injury are just as challenging for the entire treatment team as long-term operative management, reconstruction strategies and rehabilitation of the complex injuries. Through close cooperation between plastic and orthopedic surgery, great progress and success in limb salvage could be achieved over the last decades. The risk for extremity malperfusion is especially high in older patients and after trauma. To maximize the success rate for free flap transfer there is need for interdisciplinary clinical examination and diagnostics. In addition to clinical methods radiological procedures are necessary to evaluate and optimize lower extremity perfusion before surgery.Vascular ultrasound provides important information about the arterial and venous status; however, DSA, CTA and MRA are well-established and exact methods to evaluate arterial inflow. The use of less invasive methods makes it much more feasible, economic and comfortable to perform preoperative selection of patients requiring interventional procedures.In the case of intraluminal stenosis without any option for PTA, a vascular surgeon can be involved at an early stage to evaluate further surgical options. In some cases, similar surgical revascularization and free flap transfer can be performed in a single surgery. Der Unfallchirurg, initially named the Monthly Journal for Trauma Medicine (Monatsschrift für Unfallheilkunde) had already endured 125 years as the specialist journal of the DGU in 2019; however, in 1944 in the 51st year the publication came to a halt due to the upheaval and serious consequences of the Second World War and only reappeared in 1949 with the 52nd year. In its 100-year history the DGU passed through 4 temporally definable phases with respect to content, politics and personnel, to which must be added its preliminary phase as the Division of Trauma Medicine (Abtheilung für Unfallheilkunde) within the Society of German Natural Scientists and Physicians (GDNÄ).
