A scoping exercise was conducted by a clinical psychologist and an assistant psychologist to assess whether there was a role for clinical psychology and to define its functions within the service. Referrals to clinical psychology during the scoping exercise comprised requests for additional support in complex cases, team reflection, and specific psychological interventions. The scoping exercise revealed the need for a clinical psychologist to be embedded within the liaison psychiatry service, with their role encompassing various aspects of patient care, consultation, training, and service development. The psychologist should be experienced working in a medical setting and be able to prioritise their workload. There was also a defined role for an assistant psychologist. The article offers reflections on challenges faced by staff within the service, and how the integration of psychology offered valuable contributions to patient care, team dynamics, and system-wide improvements. This reflection emphasises the importance of clinical psychologists working in older adult liaison psychiatry services.","PeriodicalId":492174,"journal":{"name":"FPOP Bulletin Psychology of Older People","volume":"32 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"134935034","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Acceptance and Commitment Therapy (ACT) is an evidence-based therapy which promotes acceptance and committed actions in line with an individual’s values. The current evidence base suggests that an integrated approach, utilising aspects of both ACT and EAP, may offer a unique approach to therapy. This research update briefly outlines both therapeutic approaches, and considers their potential synergy, before positing that a combined approach could be an effective therapy for older adults, given the unique setting, emphasis on values and sensory stimulation. We conclude by discussing potential challenges and the service contexts of a combined EAP and ACT approach, and how we intend to evaluate this approach.","PeriodicalId":492174,"journal":{"name":"FPOP Bulletin Psychology of Older People","volume":"30 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"134934935","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Several services throughout the country are exploring the efficacy of multi-modal treatments for older adults that were initially designed for use in adults with a diagnosis of ‘Personality Disorder’. Manchester Later Life services’ Compass pathway offers three distinct treatments for older adults with complex emotional needs: Structured Clinical Management (SCM), Mentalisation Based Treatment (MBT) and Radically Open Dialectical Behaviour Therapy (RO-DBT). The service also has a dedicated Later Life Experts by Experience Development Group and offers a small case management service for individuals that do not easily fit into the criteria for standard treatment. This article will outline the rationale for setting up the Compass service, outline the broad structure of the pathway, highlight the positive outcomes for SCM and MBT, discuss the challenges experienced so far, and provide some information about the adaptations that have been made to meet the needs of older people.","PeriodicalId":492174,"journal":{"name":"FPOP Bulletin Psychology of Older People","volume":"187 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"134933821","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This paper describes an outreach program carried out by the Older Adult Psychology Specialty in NHS Ayrshire and Arran, which aimed to increase knowledge and understanding of psychological therapies for older people living in the health board area, and to provide information about healthy ageing from a psychological perspective to a population who may otherwise not have had access to this information. This paper summarises this work and gives suggestions for future developments. Whilst this project was set in a semi-rural Scottish context, there are elements that could be replicated in other UK board areas that have a similar service aim and that have similar third sector agencies operating in their area.","PeriodicalId":492174,"journal":{"name":"FPOP Bulletin Psychology of Older People","volume":"146 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"134934373","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Given the unique care requirements of people with dementia, a timely diagnosis is hugely beneficial as it enables healthcare professionals to understand the person’s care needs and develop tailored care plans. To address this, a pilot project was conducted in East Berkshire care homes from 2022 to 2023. The initiative employed an abbreviated memory assessment model based on the Diagnosing Advanced Dementia Mandate (DiADeM) tool, targeting residents exhibiting signs of cognitive impairment without a formal dementia diagnosis. Of the 23 residents assessed under DiADeM, 16 were diagnosed with dementia, showcasing promising outcomes for increasing diagnosis rates in the older adult population.","PeriodicalId":492174,"journal":{"name":"FPOP Bulletin Psychology of Older People","volume":"34 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"134933815","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Janet engaged in cognitive behaviour therapy (CBT) for depression and generalised anxiety, using Laidlaw’s comprehensive case conceptualisation for older people as a guide, and with a key focus on experiential avoidance as a maintaining factor for her distress in the context of grief. Experiential avoidance, is suggested to significantly mediate the relationship between depression and generalised anxiety, which are both common for older adults; however, the evidence base is limited for treatment and guidelines are unclear. Outcome measures demonstrated reductions in self-reported low mood, anxiety and worry. Further research on the role of experiential avoidance in anxiety and depression in older adults is needed to establish a clearer understanding of its role in maintaining emotional distress within this population. Janet provided consent for the case study to be published within an academic journal.
