Specht","doi":"10.1111/J.1744-618X.2003.050_2.X","DOIUrl":"https://doi.org/10.1111/J.1744-618X.2003.050_2.X","url":null,"abstract":"PURPOSE \u0000 \u0000To evaluate the effectiveness of the interventions of nurse care managers in the care of family members of people with dementia. \u0000 \u0000 \u0000 \u0000METHODS \u0000 \u0000Data were collected as part of a 3-year Administration on Aging–funded Alzheimer Demonstration Project to provide expanded in-home services to rural Iowans affected by Alzheimer disease and related disorders in 8 rural Iowa counties—randomly selected to have a nurse care manager and 4 designated control counties that had traditional case management service. Nurse care managers were trained in the care of people with dementia and their caregivers, the use of role transition theory, and the Progressively Lowered Stress Threshold model of care to provide and coordinate services for enrollees. All referred people with cognitive impairment and their families in the 8 study counties were eligible for inclusion. Three selected NOC outcomes were tested in clinical settings. Interrater reliability for the outcomes was good (87%–95%). The construct validity of Caregiver Stressors Outcome was .74 when correlated with the Caregiver Stress Index. \u0000 \u0000 \u0000 \u0000FINDINGS \u0000 \u0000Of the 142 subjects with cognitive impairment enrolled within the first year of the grant, 113 had a caregiver. The outcomes were used to evaluate differences in caregiver outcomes at baseline and at 6-month intervals. The majority of caregivers at follow-up was female and had been providing care for ≥5 years. For each of the outcomes the majority of caregivers had improved scores, with only 2–4 caregivers getting scores indicating worsening conditions or remaining the same. \u0000 \u0000 \u0000 \u0000CONCLUSIONS \u0000 \u0000Preliminary analysis shows a trend of improved outcomes with the use of a nurse care manager. The NOC caregiver outcomes showed good variability among caregivers at baseline, with caregiver responses distributed throughout the scales. The NOC outcomes also provide guidance for interventions of the nurse care managers. Further evaluation of the outcomes is needed, including examining the relationships of placement, health status, and service use of each outcome. Lockhart","doi":"10.1111/J.1744-618X.2003.23_12.X","DOIUrl":"https://doi.org/10.1111/J.1744-618X.2003.23_12.X","url":null,"abstract":"BACKGROUND \u0000 \u0000Computerized physician order entry, test results retrieval, medication administration, nursing care planning, and other functions have been in place at Virginia Commonwealth University Health System (VCUHS) for more than 20 years. Today there are multiple ways for communicating the patient plan of care in the computerized information system (CIS). Methods vary depending on the clinician's role and the setting and focus of the communication, but all are essentially variations in a basic pattern. Ultimately, all physician orders — but only some of the interventions, orders, and notes by other clinical disciplines—are printed together on the nursing Kardex, the working document for organizing inpatient care on a shift-by-shift basis. \u0000 \u0000 \u0000 \u0000No similar computer-generated document exists for outpatients or emergency department patients, and none is available to disciplines other than nursing. This approach fragments communication and creates a sense that the patient care plan designed by each discipline is isolated from all others. We are currently building a replacement CIS using Cerner Millennium software. We believe the first step to improve these system-driven problems is to design the new system so that all patient care problems, interventions, and orders are entered in the same way. \u0000 \u0000 \u0000 \u0000MAIN CONTENT POINTS \u0000 \u0000Product functionality can replace traditional nursing care planning. Plans initiated by nonphysician clinicians tend to risk isolation. Software limitations (e.g., field lengths) and features such as duplicate order alerts must be taken into consideration when planning an overall redesign. To take advantage of the benefits offered by our new CIS, all licensed clinicians must have the ability to independently place orders and document problems that fall within their scope of practice. \u0000 \u0000 \u0000 \u0000Our plan for identifying diagnoses/problems is to use the “Problem List” feature. A variety of nomenclatures such as CPT, ICD-9, and SNOMED are available within the software. With the development of NIC and NOC nomenclatures, it is now possible to expand the use of standardized language to capture and track new aspects of patient care delivery. \u0000 \u0000 \u0000 \u0000The Virginia Board of Nursing does not permit or restrict the procedures a nurse is licensed to perform. Two important factors that influence scope of practice are reimbursement and organizational culture. The more important concept within scope of practice is validation of competency. The culture at VCUHS centers on the concept that the physician initiates all patient care orders. While the new system must allow nurses and other disciplines to enter orders as an agent for the physician, there are orders for activities that fall within each discipline's scope of practice that will not require order entry as an agent for the physician. \u0000 \u0000 \u0000 \u0000The VCUHS design will be implemented in phases. Two of the most frequent infections are urinary tract infections and respiratory infections. These often routine infections in a geriatric resident can result in potentially life-threatening complications. Tracking and preventing infections in long-term care facilities is essential in maintaining residents' quality of life and assisting them to function at their highest practicable level. Federal and state regulations require the presence of an infection-control program in long-term care settings to prevent the development and transmission of disease and infection within the facility. \u0000 \u0000MAIN CONTENT POINTS \u0000By incorporating NANDA, NIC, and NOC into the infection-control program, staff members were able to create an infection-specific, shortterm plan of care for infections with minimal time requirements. The nursing diagnosis used for this example was risk for infection followed by the NIC “infection protection” and the NOC Infection Status. Outcomes appropriate for the infection were chosen from the outcome category Infection Status. Scores of 1 (severe) through 5 (none) were listed beside each intervention chosen. The resident's symptoms were rated at the onset of infection, at intervals during treatment, and at the conclusion of treatment using the 1–5 scale. The ratings reflected the improvement of symptoms as treatment progressed. If no improvement was seen, reevaluation and further treatment were initiated. \u0000 \u0000CONCLUSIONS \u0000Measurement of resident outcomes to indicate appropriate nursing interventions is vital in determining proper treatment in long-term care residents. The instrument (interrater reliability r=. 9) had five parts: abdominal pain assessment; NANDA's nursing diagnosis, pain, abdominal; the NIC intervention, “pain management”; the NOC outcomes, Symptom Severity and Pain Level; and (e) five openended questions. Of the sample, 12 were ineligible (returned, retired, not currently a school nurse), giving a net sample of 138; 64 were usable, for a 46% response rate. \u0000 \u0000FINDINGS \u0000Pain and other nursing diagnoses associated with abdominal pain (constipation, diarrhea, depression, altered family processes, fatigue, nutrition: less than body requirements, self-esteem disturbance, and sleep deprivation) were associated with abdominal pain. The NIC interventions for pain management reflect the comprehensiveness of abdominal pain management. NOC indicators from the outcomes, Pain Level and Symptom Severity, used to evaluate the effectiveness of interventions were identified, as were indicator changes postintervention. The outcome Pain Level included reported pain level, frequency of pain, oral expression of pain, facial expression of pain, and length of the pain experience. For the Symptom Severity outcome, changes were seen in symptom intensity, symptom persistence, associated discomfort, and symptom frequency. Nurses' perceptions of the most frequent etiologies of abdominal pain related to psychosocial problems of family origin, inadequate nutrition, overconsumption of junk food, and menstrual complaints. Nurses recommended education on the etiologies of abdominal pain complaints. No additional interventions were identified. Nurses articulated that additional staff members or a decreased ratio of children to school nurse would facilitate health promotion, health protection, and teaching. \u0000 \u0000DISCUSSION \u0000School nurses support and coordinate activities and resources to ensure that children's abdominal pain complaints are recognized and assessed appropriately. The NANDA, NIC, and NOC linkages helped to cluster data, accurately identify the child's complaint or problem, and determine if the interventions affected the desired outcomes. Data on abdominal pain can be aggregated over time to determine the most frequent abdominal pain etiologies, effective interventions, scope and cost-effectiveness of practice, and to make other recommendations. \u0000 \u0000CONCLUSIONS \u0000In addition to knowledge of growth and development, school nurses need to consider factors such as religion and culture to accurately assess a child's pain response. Nurses and professors met weekly to exchange information and promote discussion about nursing care, with emphasis on nursing diagnosis. In 1996 the group linked to the Brazilian Nursing Association Section in order to expand this work to the professional community. \u0000 \u0000 \u0000 \u0000MAIN CONTENT POINTS \u0000 \u0000This group committed itself to organize meetings open to all the nursing professionals interested in exchanging experiences about nursing care and nursing diagnosis, through the presentation of case studies and papers. In order to facilitate participation and integration of professionals from different health institutions and nursing schools, an itinerant group was formed. From 1999 to 2000 meetings were held in seven different hospitals in the city. The number of participants varied from 19 to 40 people and included students, nurse helpers, and technicians from 6 universities and 15 healthcare institutions. \u0000 \u0000 \u0000 \u0000CONCLUSIONS \u0000 \u0000This experience has been successful in spreading the use of and discussing nursing diagnosis. A convenience sample of 120 nurses familiar with nursing diagnoses answered a semistructured questionnaire that included demographic data, the definition, and a list of defining characteristics of anxiety. Three defining characteristics of the nursing diagnosis fear were added to the list as distractors. Subjects were asked to rate each defining characteristic on a scale from 1 (not at all characteristic) to 5 (very characteristic). Descriptive analysis was used to identify the nurse experts and clinicians. Assigned weights were determined for each characteristic: 1 = 0, 2 = 0.25, 3 = 0.50, 4 = 0.75, and 5 = 1. Variance analysis using F statistics to compare averages and chi-squares for frequency distribution were applied. The defining characteristics with weight ratios >.80 were considered critical, those with weight ratios >.50 and <.80 were noncritical, and those defining characteristics with weight ratios of <.50 were not considered representative. An overall DCV score was obtained by summing individual scores and dividing by the total number of defining characteristics. Comparative analysis was used to verify differences in expert and clinician judgments using significant differences analysis. \u0000 \u0000 \u0000 \u0000FINDINGS \u0000 \u0000Among 120 nurses, 51 (44%) were expert nurses and 69 (58%) had expertise by caring for people with anxiety. Three fourths (76%) of the expert nurses and 75% of the clinicians considered the NANDA definition appropriate. Judgment of both groups of nurses was equivanent (p= .93). Experts classified 8 (11%) defining characteristics as critical, but only 2 (3%)—anxious, fidgeting—were judged critical by both expert nurses and clinicians. Two defining characteristics (increased tension, preoccupation) of the 8 had statistically different scores between the two groups. The other defining characteristics were statistically similar. A total DCV score of 0.67 ± 0.10 was obtained from expert nurses and 0.65 ± 0.10 from clinicians (nonsignificant). \u0000 \u0000 \u0000 \u0000DISCUSSION \u0000 \u0000Anxiety is a common and frequent manifestation and a nursing concept identified and recognized by Brazilian nurses who considered it clearly defined and established by NANDA. Both groups of Brazilian nurses judged signs and symptoms of anxiety in a very similar way. \u0000 \u0000 \u0000 \u0000CONCLUSIONS \u0000 \u0000According to Brazilian nurse experts, anxiety is well understood and in their view there are 8 critical defining characteristics (anxious, fidgeting, jittery, increased tension, insomnia, irritability, preoccupation, sleep disturbance) and 38 noncritical defining characteristics. The other characteristics (24) were not judged to be relevant. METHODS.A triangulation method using multiple sources of data, including an environmental survey, fish consumption survey, government reports, publications, and community assessments FINDINGS.Three priority community environmental nursing diagnoses for aggregates within the community: Knowledge deficit among community residents related to lack of awareness of contaminants in the Anacostia River; “risk for adverse human health effects” among pregnant and nursing women and preschoolers related to consumption of chemically contaminated fish; and knowledge deficit among community residents related to lack of awareness of health effects of consuming chemically contaminated fish. CONCLUSIONS. The diagnoses serve as the basis for community education and other interventions. The methods are practical and useful for this type of research. PRACTICE IMPLICATIONS. The nursing profession is experiencing a growing awareness of the close relationship between environmental health and the health of individuals and communities. To explicate the opportunities in life that support or enhance spirituality in well adults. \u0000 \u0000 \u0000 \u0000METHODS. Grounded research methodology using semistructured interviews with 12 well adults explored spiritual factors and opportunities for spiritual growth. \u0000 \u0000 \u0000 \u0000FINDINGS. Seven themes emerged from the data: connectedness, beliefs, inner motivating factors, divine providence, understanding the mystery, walking through, and life events. \u0000 \u0000 \u0000 \u0000CONCLUSIONS. The current resurgence in spirituality in society has increased the demand for sensitivity to patients' spiritual needs. Spiritual care is central to nursing's holistic philosophy. When opportunities that enhance spirituality are explicated from research, the knowledge can be applied to nursing assessments and interventions to promote positive health outcomes. \u0000 \u0000 \u0000 \u0000PRACTICE IMPLICATIONS. Since nurses are present during events that evoke individualized, varied human responses in the spiritual domain, they need the language to describe assessment findings. A new NANDA wellness nursing diagnosis for human responses in the spiritual domain, “Potential for Enhanced Spirituality,” is supported and presented. \u0000 \u0000 \u0000 \u0000Opportunites d'enrichissement de la spiritualite chez les adultes bien-portants \u0000 \u0000 \u0000 \u0000BUT.Expliciter les opportunites au cours de la vie qui soutiennent et enrichissent la spiritualite des personnes bien-portantes. \u0000 \u0000 \u0000 \u0000METHODES.La methodologie de theorie ancree, utilisant des entretiens semi-structures avec 12 adultes bien-portants, a permis d“explorer les facteurs spirituels et les opportunites de croissance spirituelle. \u0000 \u0000 \u0000 \u0000RESULTATS.Sept themes ont emerge des donnees: unite harmonieuse, croyances, facteurs internes de motivation, providence divine, comprehension du mystere, traversee et evenements de la vie. \u0000 \u0000 \u0000 \u0000CONCLUSIONS.Le renouveau actuel de la spiritualite dans la societe a augmente notre sensibilite aux besoins spirituels des patients. Les soins spirituels sont au coeur de la philosophie des soins holistiques.
