IRB approval can require many documents and steps. It is important to start the submission early, have patience throughout the process, and determine what can help expedite the process. Research cannot begin without IRB approval, which is necessary when working with human subjects. Ultimately, the researchers and IRB have the same goal of enabling good research with minimal subject risk. This may involve either direct fixation of the posterior malleolus or indirect stabilization with the fixation of the medial and lateral malleoli and trans-syndesmotic fixation. Recently there has been a paradigm shift in the management of these injuries with a more thorough understanding of anatomy, stability, and long-term sequela of these injuries. Ankle fractures in patients with osteopenia and in diabetic patients with deranged bone remodeling constitute high-risk injuries that may result in catastrophic complications. These patients present unique care challenges and should not be approached in the same manner as their healthy counterparts. There are various treatment measures to take into consideration when managing these patients. Treatment approaches range from conservative casting to surgical reconstruction. The authors believe that when faced with deformity, aggressive reconstruction to maintain a plantigrade foot and prevent loss of function is critical. Because of the nature of the Charcot condition, timing and fixation are often debated. The concern was loss of correction and fixation failure. However, it has been shown that a prolonged period of nonweight bearing can predispose patients possible deep vein thrombosis, disuse osteopenia, cardiovascular complications, and generalized deconditioning. The authors' institution has published studies reviewing the efficacy of early weight bearing after first metatarsophalangeal joint arthrodesis, modified Lapidus bunionectomy, and open reduction and internal fixation of ankle fractures. Almost 100 years ago, Dudley Morton observed that certain retained atavistic traits are responsible for pathologic breakdown of the modern foot. {"title":"The Evolution of the Treatment of Clubfoot from Posterior Medial Release to the Ponseti Technique: My 42-Year Journey at the Permanente Medical Group.","authors":"Stephen Silvani","doi":"10.1016/j.cpm.2023.06.001","DOIUrl":"10.1016/j.cpm.2023.06.001","url":null,"abstract":"<p><p>Traditionally, the Kite manipulation and casting were utilized for the treatment of the congenital clubfoot. This was followed by an extensive posterior medial soft tissue surgical release followed by more casting. Often, the results were less than optimal with scarred, painful feet that needed further corrective surgery. Dr. Ponseti developed a different technique of manipulation, casting, and an Achilles tenotomy that fully corrected these clubfeet without the need for the extensive surgery. This was followed by the mandatory use of night braces with special shoes for a period of 4 years. The Ponseti method is now universally utilized around the world and is the standard of care for the management of clubfoot.
