To improve the results of treatment of patients with post–pancreatic resection bleeding. \u0000Materials and methods. The results of treatment of 827 patients who underwent radical resection of the pancreas for malignant tumours in the period from 2010 to 2021 were analysed. The patients were divided into two groups: the main group – 449 patients who were treated at the clinic from 2016 to 2021, and the control group – 378 patients who were treated in the clinic from 2010 to 2015. Postpancreatectomy bleeding occurred in 27 (6.0%) patients in the main group who were treated according to the developed diagnostic and therapeutic algorithm with the maximum use of endovascular techniques, and in 20 (5.3%) patients in the control group who received standard treatment. \u0000Results. X–ray endovascular bleeding control was performed in 14 (51.9%) of 27 patients in the main group: X–ray endovascular occlusion – in 9, by means of stent graft placement – in 5. In 3 (11.1%) patients of the main group, the source of bleeding was not detected during angiography, and they underwent laparotomy with subsequent bleeding control. Complications after X–ray endovascular occlusion occurred in 1 (11.1%) patient, and there were no complications after stent graft placement. Open surgical interventions were performed in 13 (48.1%) patients. One (3.7%) patient died after laparotomy with bleeding control due to the development of further purulent–septic complications. In the control group, X–ray endovascular bleeding control was performed in 3 (15%) patients, and relaparotomy with bleeding control in 17 (85%) patients. 6 (30%) patients died after open reoperative interventions. \u0000Conclusions. Endovascular techniques for stopping post–pancreatic transplantation bleeding are highly effective and safe with favourable technical and clinical results. {"title":"Immediate results of biliodigestive reconstruction in patients with peripheral cholangiocarcinoma with invasion of visceral veins","authors":"O. Usenko, A. I. To evaluate the immediate results of biliodigestive reconstruction in patients with peripheral cholangiocarcinoma with invasion of the visceral veins. \u0000Materials and methods. From 2004 to 2018, 28 patients with peripheral cholangiocarcinoma with invasion of the visceral veins were radically operated at the Department of Liver Transplantation and Surgery of the Shalimov National Institute of Surgery and Transplantation of the National Academy of Medical Sciences of Ukraine (now the Shalimov National Scientific Centre of Surgery and Transplantation of the National Academy of Medical Sciences of Ukraine). The main group consisted of 8 (28.6%) patients who underwent biliodigestive reconstruction simultaneously with resection and reconstruction of visceral veins, the control group consisted of 20 (71.4%) patients who underwent resection and reconstruction of visceral veins, and biliodigestive reconstruction was not performed. \u0000Results. Postoperative clinically significant (grade IIIaIV according to the ClavienDindo classification) complications during the 90day postoperative period were noted in 12.5% of patients in the main group and 25% of patients in the control group (p=0.002), postoperative liver failure according to the criteria of the International Study Group on Liver Surgery in 25 and 30%, respectively (p=0.415). Repeat surgery was performed in 12.5 and 10% of patients, respectively (p=0.923). There was no postoperative mortality in both study groups. \u0000Conclusions. To delineate new approaches to surgical treatment in patients with an acute pancreatitis as development of principles of the Atlanta–2012 consensus and guidelines of the World Society for Urgent Surgery.
 Materials and methods. The results of treatment of patients in The Lviv Municipal Pancreatologic Centre were analyzed, using general clinical, the laboratory, biochemical, radiological, pathomorphological, bacteriological, pH–metric, cytomorphological and mathematic–statistical methods.
 Results. There was established, that local complications of an acute pancreatitis occur as solitary or multiple or coexistent foci of pathological processes. Diagnostic algorithm, taking into account the most informative indices (in accordance to criteria “sensitivity”, “specificity” and “diagnostic efficacy”), was proposed. Own predictors of severity of the disease – visual, pH–metric, cytomorphological and bacteriological – were elaborated. Taking into account the significance of early enteral feeding, the procedures of its transintestinal probe realization was proposed and their efficacy proved. The methods of analgesia without preparations–opiates and minimization of invasive way of the medicines administration were elaborated. Surgical tactics, using application of miniinvasive and standard operative technologies, was depicted.
 Conclusion. To determine clinical efficacy of the preparation ethylmethylhydroxypyridine succinate application in early period of the burn disease.
 Materials and methods. In 14 patients with burns ethylmethylhydroxypyridine succinate (the main group) was applied in complex treatment, while 22 patients were treated in accordance to standard scheme (control group). The content of TBK–active products, superoxide dismutase, tumor necrosis factor–α and S–nitrosothiols in peripheral blood on the 1–2, 7–8 and 13–14th days after the burn trauma were determined.
 Results. In the main group versus initial values there were registered: the content of TBK–active products lowered in 1.22 times, the content of SOD raised in 1.73 times, the content of the S–nitrosothiols lowering in 117 times, while content of the tumor necrosis factor–α versus certain index in control group reduced in 1.15 times on 7–8th day and in 1.08 times on the 13–14th day after the burn. In the main group of patients the average square of necrectomies have reduced, volume of performance of autodermoplasty was less in 1.12 times, in them less surgical interventions performed, and the stationary stay has been shortened in 1.48 times.
 Conclusion.
