Vancouver type C PPFs compromise the majority type of fracture associated with high complication rates. The complexity of Vancouver C-type PPFs lies in the presence of the femoral stem in the proximal fracture fragment. This study compares the biomechanical performance of four plate fixation configurations to repair Vancouver type C periprosthetic femur fractures. Six large adult 4th generation biomechanical low-density femur Sawbones instrumented with cemented femoral stems were used to conduct this study. A comminuted Vancouver-type C PPF was simulated and fixed with a plate. Four proximal fragment construct configurations were studied with combinations of unicortical screws, bicortical screws, and cerclage cable fixation. Constructs were loaded in axial compression and four-point bending, and the stiffness was calculated. Constructs with bicortical screw fixation had significantly greater stiffness than those with unicortical screw and cerclage cable fixation in axial and four-point bending loads. Constructs with cerclage cable and unicortical screw fixation had greater stiffness than ones with unicortical screw fixation. Using bicortical screws in the proximal construct of Vancouver type C PPFs increases construct stiffness in axial and bending loads. Though controversial, using cerclage cables might have a place for construct augmentation when using only unicortical screws or a combination of unicortical and bicortical screws.","PeriodicalId":488685,"journal":{"name":"SurgiColl","volume":"4 4","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-12-22","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"138944280","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Operative fixation is often technically challenging due to limited surgical windows, limited bone stock for fixation, its articular nature, and fracture extension into the trochlea or condyles. The goal of treatment is stable internal fixation to permit an early range of motion. Aside from open reduction internal fixation (ORIF), alternative treatment options include fragment excision and arthroscopic assisted reduction and fixation. Here we detail our preferred approach to treat capitellum fractures, including, preoperative planning, surgical anatomy, surgical setup and technique utilizing a posterior utility incision and olecranon osteotomy, postoperative care, and avoidance of potential complications.","PeriodicalId":488685,"journal":{"name":"SurgiColl","volume":"3 9","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-12-22","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"138944283","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Patients were followed up for a mean of nine months. Outcomes were assessed using metrics such as radiographic union, pain, and patient-reported outcomes questionnaires. We explained a stepwise description of a novel technique for isolated tibiotalar ankle arthrodesis using a curved nail. We also presented our postoperative protocol, including weight-bearing and rehabilitation protocol. The union rate was 100% after a mean time of nine months of follow-up. There were no complications related to the hardware requiring revision or removal. Pain was measured with a visual analog scale and improved from a mean of 6.7 to 4.2. Patients undergoing tibiotalar arthrodesis using the Shotel nail have acceptable outcomes. Considering this technique as a minimally invasive intervention, we believe that soft tissue complications are less than conventional open techniques.","PeriodicalId":488685,"journal":{"name":"SurgiColl","volume":"3 12","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-12-22","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"138944887","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

In elderly patients, Cervical myelopathy is marked by multifaceted deterioration of the anatomic structures in the spinal column. Injuries can result in more acute presentations of Cervical myelopathy. Symptoms typically stem from mechanical pressure and ischemic changes exerted on the spinal cord. Common symptoms include neck pain/stiffness, spasticity/balance and gait dysfunction, loss of dexterity and fine motor movements, and upper and lower extremity dysesthesias. To diagnose accurately, a detailed history and physical examination are required. Radiographs, computed tomography (CT), and magnetic resonance imaging (MRI) are often needed, with MRI being the gold standard. Many surgical approaches exist to address spinal cord compression. The surgical approach is influenced by several factors: the exact location of cord compression (anterior/posterior), the number of levels of stenosis, overall spinal alignment, body habitus and other factors such as prior surgery.","PeriodicalId":488685,"journal":{"name":"SurgiColl","volume":"2 12","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-12-22","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"138944052","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The patient initially had an uneventful postoperative course but experienced an atraumatic increase in arm pain with radiographs consistent with nonunion and plate fatigue fracture requiring revision after eleven months. Humeral shaft nonunion in an otherwise healthy male patient taking HIV PrEP suggests these medications may be a risk factor for nonunion. However, additional studies with sufficient statistical power are required to define this interaction further and better understand the clinical implications.","PeriodicalId":488685,"journal":{"name":"SurgiColl","volume":"37 22","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-12-22","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"138946891","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Studies included in this systematic review were conducted per PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines with the following eligibility criteria: (1) single case or limited case series, (2) established diagnosis of necrotizing fasciitis in the hand, wrist, or forearm, (3) English language publication. Most of the patients in this review (38, (70.37%)) presented with an inciting injury. A minority of patients had other associated co-morbidities independent of injury, including Diabetes Mellitus, immunosuppression, and IV drug use. The most common infectious agents were gram-positive cocci, identified in 32 (59.26%) cases, followed by gram-negative bacilli in 21 (38.89%) cases. The most common pathogen was Group A streptococcus (GAS), reported in 15 (27.78%) cases, followed by Vibrio vulnificus in 6 (11.11%) cases. Most patients were managed with empiric antibiotic treatment. Fifty patients (92.6%) reported surgical intervention and 45 (83.3%) had at least one debridement. Most patients in the cases reviewed had either no or minimal long-term sequelae following their infection. Early recognition of the signs and symptoms of necrotizing fasciitis is imperative to avoid loss of limb and/or death. Further, aggressive management with empiric antibiotic coverage and rapid surgical intervention improves outcomes in patients with necrotizing fasciitis of the upper extremity.","PeriodicalId":488685,"journal":{"name":"SurgiColl","volume":"27 15","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-12-22","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"138947179","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Controversy exists over the preferred method of treatment for purely ligamentous injuries. The study aims to evaluate early postoperative complications and time to return to work/play in patients with ligamentous Lisfranc injuries who underwent ORIF using flexible fixation. A retrospective chart review was performed on nine patients who underwent ORIF with flexible fixation using the InternalBrace (Arthrex Naples, FL) for ligamentous Lisfranc injuries. Patients were evaluated at 2- weeks, 6- weeks, and 12- weeks postoperatively with radiographs at 6- weeks, 12- weeks, and 24- weeks postoperatively. The average follow-up time was 6.6 months (SD = 5.1), the average time to unrestricted weight-bearing was 6.8 weeks (n=9), and the average time to return to work/sport as tolerated was 14.3 weeks (n=7). The only complication noted at the 2-week follow-up was hypersensitivity along the foot [1 (11%)], resolved by the 12-week follow-up. No radiographic hardware complications were noted at either 6-week or 12-week postoperative x-rays. Flexible fixation using the InternalBrace (Arthrex, Naples, FL) is an effective procedure for patients with ligamentous Lisfranc injuries. The present study found no early postoperative complications, and the time to return to work/sport was comparable to other methods of ORIF reported in the literature.","PeriodicalId":488685,"journal":{"name":"SurgiColl","volume":"5 7","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-12-22","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"138945115","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Of these determinants, it remains unclear how education level, access to transportation, and the home environment influence Patient-Reported Outcomes (PROs) following orthopaedic trauma surgery. Methods A cross-sectional survey was conducted at an urban level 1 trauma center to collect data on demographics, education level, access to transportation, and the home environment of 120 patients who underwent fracture fixation surgery. Patient data was correlated with PRO data as measured by Patient Reported Outcomes Measurement Information System (PROMIS) survey scores, including global mental health (GM), global physical health (GP), pain interference (PI), and physical function (PF). A multivariable mixed-effects regression analysis was performed to determine associations between SDOH and PROs. Results Multivariable regression analysis results demonstrated that patients with high school or lower education levels were associated with better PF (P=0.007). Patients with access to transportation were associated with higher GM (P = 0.016), while those who lived greater than 5 miles from the hospital were associated with higher GP (P = 0.0498). Those with difficulty accessing the bathroom were associated with lower GP (P = 0.007) and GM (P = 0.0008), increased PI (P = 0.0002), and lower PF (P = 0.004). People living in apartments had higher GM (P = 0.0008). Navigating stairs inside the home was associated with lower GP (P = 0.030) and higher PI (P = 0.0007), while those who navigated stairs outside of their home reported higher GP (P = 0.038). Conclusion This study found that in our population, education level, access to transportation, and the home environment appeared to be associated with PROs following orthopaedic trauma surgery.","PeriodicalId":488685,"journal":{"name":"SurgiColl","volume":"64 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-09-22","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"136060161","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
