Rectal biopsy is deemed critical for its testing. In some instances, regular techniques fail to detect it. This study aims to evaluate the testing role of calretinin and CD56 immunohistochemistry and correlate the results to routine hematoxylin & eosin stained samples. Methods: This retrospective study was conducted in Rizgary Teaching Hospital, Erbil, Kurdistan region, Iraq. Rectal biopsies and colonic resection specimens of the clinically suspected Hirschsprung’s disease patients were collected and stained with calretinin and CD56 then their findings were compared to the hematoxylin and eosin stained sections during the period between February 2016 to October 2021. Results: Fifty patients aged from 3 days to 8 years with a male-to-female ratio of 3.6:1 were examined for rectal and colonic biopsies. Forty out of 50 cases were detected as HD by H&E staining. The sensitivity, specificity, positive predictive value, negative predictive value, and accuracy rate of calretinin were coherent with that of H&E-stained samples (100%), the likelihood ratio was 50 and the kappa test was 1, were superior to CD56 results with sensitivity (100%), specificity (90%), positive predictive value (97.5%), negative predictive value (100%), accuracy rate (98%), likelihood ratio was 27.77 and kappa test was 0.805. Conclusions: Immunohistochemical expression of calretinin is more specific and accurate than CD56 in HD diagnosis. Calretinin is a trustworthy, additional diagnostic tool for better morphological evaluation of ganglion cells and thereby assists in making a reliable diagnosis of HD.","PeriodicalId":486511,"journal":{"name":"Advanced medical journal","volume":"67 4","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-03-10","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"140254869","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The aim was to know the optimum mode of delivery in pregnant women with preterm premature rupture of membrane. Methods: A prospective observational study was performed on 50 pregnant ladies with a gestational age of 32-37 weeks with preterm premature rupture of membrane admitted to the Sulaimani Maternity Teaching Hospital from June 2021 to June 2022. Demographic features, medical history,mode of delivery and neonatal outcome were recorded . Results: The mean±SD (standard deviation) of maternal ages was 27.2±5.8 years (ranging from 16 to 38). The mean±SD of gravidity, parity, and abortus was 2.9±1.95, 1.6±1.7, and 1.4±0.6, respectively. The associations of women's age, residency, occupation, educational level, parity, and past medical history with mode of delivery were statistically insignificant (p-values of >0.05). The association of the indications of C/Ss with the delivery outcome was statistically significant (p-value of <0.001). The associations of gestational ages at rupture and birth did not affect the delivery outcome (p-values of >0.05). Besides, the fetal outcome did not affect the delivery outcome, except for fetal gender significantly (p-value = 0.05) affected the delivery outcome. Conclusions:The associations between indications of cesarean section and fetal gender with mode of delivery were statistically significant in pregnant women with preterm premature rupture of membrane between 32-37 gestational weeks.","PeriodicalId":486511,"journal":{"name":"Advanced medical journal","volume":"37 3","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-03-10","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"140255110","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The present study aimed to determine the association of gallstone with Sickle cell disease and sickle β-thal patients compared to the control healthy group and to determine gallstone's association with several demographic and laboratory parameters among the patients themselves. Methods: Patients visiting the Akre thalassemia center for routine checks have been recruited for this study (patients=73; control=36). Blood samples were collected for biochemical and haematological analyses. All participants were subjected to abdominal ultrasonography for confirmation of gallstones. These parameters were compared with patients' subtype traits. Additional information regarding blood transfusion, hospital admission, and the use of chelating agents was also allocated. Results: The prevalence of gallstone was significantly higher in patients (12%) than in the control (3%) group (P<0.05), with similar prevalence in both trait; homozygous haemoglobin S (14%) and compound heterozygous of hemoglobin S and β-thalassemia (11%), together with significant differences in the biochemical and haematological parameters. The haemoglobin concentration was significantly (P<0.05) reduced in patients (8.5±0.86) versus control (12±1.6). Reticulocytes, lactate dehydrogenase enzyme, and bilirubin were significantly (P<0.05) elevated in patients compared to the control. No differences existed between control versus patients regarding hospital admission, blood transfusion, and the use of medication including chelating agent or hydroxyurea. Conclusion: Sickle-diseased and sickle beta-thalassemia patients have a higher risk of developing gallstones than the general population alongside interruption of metabolic and biochemical parameters with co-association with sub-traits of type of haemoglobin.","PeriodicalId":486511,"journal":{"name":"Advanced medical journal","volume":"42 6","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-03-10","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"140254710","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This study aimed to determine whether a history of cesarean section or miscarriage was related to placenta previa at Sulaimani Maternity Teaching Hospital. Methods: At the Sulaimani Maternity Teaching Hospital, this case-control study was conducted between the 11th of September 2021 to the 31st of June.2022. A total of 100 women provided information, which was divided into two groups: Group A, which included 50 pregnant women with gestational ages between 30 and 40 weeks and confirmed placenta previa, and Group B, which included 50 pregnant women randomly selected with pregnancy lengths of 30 to 40 weeks and the placenta was in its usual location. Detailed history was taken from each woman, through this the relationship of placenta previa with previous cesarean section and miscarriages was studied. Results: The results of this study revealed that prior cesarean sections were previously performed in 74% of cases of placenta previa, compared to 46% of controls (p value = 0.004). In addition; there was no correlation between placenta previa and prior miscarriages (p value= 1.000), and a highly significant correlation was seen between the rise in previous cesarean deliveries and the presence of accreta (p value = 0.003). Conclusion: The likelihood of placenta previa in subsequent pregnancies is dramatically increased in this study by past cesarean deliveries; previous miscarriages have no significant impact on placenta previa development.","PeriodicalId":486511,"journal":{"name":"Advanced medical journal","volume":"21 3","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-03-10","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"140255144","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
