Therefore, proper treatment of this entity is paramount in preventing such problems. Essentially, there are three treatment approaches, namely, observational, close reduction and open reduction, and internal fixation. 1 The debate between open and close reduction has been going on ever since with the proof that open reduction aims at restoring the facial asymmetry of the patient faster: although it has its own demerits like limited access even through surgical exposure and difficulty in reduction. World Health Organization has defined it as “localized benign but sometimes aggressive osteolytic proliferation consisting of fibrous tissue with hemorrhage and haemosiderin deposits, presence of osteoclast-like giant cells with reactive bone formation.” Surgical resection is often necessary to manage the condition, and subsequent reconstruction is crucial for functional and aesthetic restoration. This case report presents a case of CGCG of the maxilla in a 61-year-old female patient who presented with a chief complaint of swelling on the right side of the palate for 2 months. Preoperative histological examination of the biopsy specimen concluded the lesion to be a CGCG. An untreated jaw fracture can lead to malunion, which can ultimately affect the neuro-muscular-occlusal coordination of the masticatory unit. The fractures of the non-dentate regions of the mandible are difficult to manage conservatively, especially in the angle region, due to various muscular activities. The difficulty is coupled when the fracture is present bilaterally, and an exclusive treatment modality should be devised on a case-by-case basis. Bilateral angle fractures of the mandible are rare occurrences and are seldom reported compared to unilateral angle fractures. The difficulty in access and the presence of three unstable fragments and third molars in this region warrant the special attention of the surgeon in devising the treatment plan. For effective management of bilateral angle fractures, surgeons require in-depth knowledge of the biomechanics and anatomy of the jaw bones. We analyzed prevalence of age, gender distribution, etiology, location and mode of treatment. In our study, the mean age was 33±14.5, gender distribution 68% males and 32% females, etiology of ZMC fractures was 114 road traffic accident (RTA), 44 falls, 12 sports injuries, five interpersonal violence, four animal bites, and four work-related. Results show that the treatment options applied are 37 conservative management and 146 open reduction internal fixation. In this study, male predilection to ZMC fractures was seen, RTA being the most common etiology and the most common site of fracture was Zygomaticomaxillary buttress region. There are different treatment protocols for the management of ZMC fractures. In our study, two-point fixation was the most common (35%). Concluding upon our experience and the results derived from our study, various methods are used to stabilize zygomaticomaxillary complex fractures. The age, gender distribution, etiology, affected side (unilateral or bilateral), mouth opening (MO), and occlusion were evaluated pre-operatively and after gap arthroplasty (GA) with follow-up two years. In this study, there were eight participants, with an average age of 24.75 ± 3.8 years, gender distribution was 62% men and 38% females with seven individuals having etiology of trauma and 1 pathology. TMJ ankylosis was found to be bilateral in 38% of patients and unilateral in 62%. Pre-op and two-year follow-up MO showed a mean of 10.75 ± 3.8 mm and 30.4 ± 5.6 mm, respectively. Re-ankylosis was not reported in any cases. The most effective care for TMJ ankylosis begins with a correct and timely diagnosis, followed by the development of a proper/comprehensive treatment plan thus improving the patient’s quality of life. We conclude that all of our GA patients had appropriate MO at follow-up of 24 months. Long-term functional outcomes of GA are satisfactory as it justifies adequate MO with minimal complications. The lesions resulting from few millimeters to centimeters can be a cyst, granuloma or an abscess combinedly comprising up to more than 90% of all periapical lesions. Various methods can be used for the treatment of the periapical lesions using modern endodontic techniques including endodontic microscopes, ultrasonic irrigation tips, intra canal calcium hydroxide medicaments, advanced cleaning and shaping endodontic files and advanced obturation techniques. The goal of the endodontic treatment is to provide a complete aseptic environment with complete sealing of root canal and coronal restoration with absolute eradication of any microbial growth for successful resolution of the lesions. Roshan, Rizwan Ahmad","doi":"10.1177/26323273231221985","DOIUrl":"https://doi.org/10.1177/26323273231221985","url":null,"abstract":"Objective: To review our clinical experience with infrahyoid myocutaneous flap which is often overlooked in the reconstruction of small to medium-sized tongue, floor of mouth, lower alveolus and gingivobuccal sulcus primary defects. Method: A retrospective study of 12 patients of squamous cell carcinoma involving the tongue, floor of mouth, mandibular gingivobuccal sulcus and alveolus was done in patients who underwent infrahyoid flap as an alternative to free radial forearm flap. This study was conducted in the Tertiary Care Institute. The study included patients treated between March 2020 to May 2021. Patients with T4 and N3 disease, recurrence cases, post-radiation, and chemo-radiation cases who refused to participate in the study were excluded. Results: Out of 12 patients, two patients developed partial flap necrosis. One patient developed a hematoma under the flap and one patient had a neck suture gaping. Postoperative results were found to be phenomenal after one month in all patients. Conclusion: The infrahyoid flap is a dependable, less arduous, and economical option for small to medium-sized defects of the oral cavity. This large, deep, diffuse swelling commonly occurs in the neck as a result of the congenital malformation of lymphatic system having an equal distribution between males and females. It results as a failure of development of the communications between lymphatics which is mostly found in the posterior triangle of neck. Its recurrence rate may be up to 10%–15%. Different treatment modalities for treating cystic hygroma are surgical excision, sclerotherapy, laser and radiofrequency. Here we report a rare case of cystic hygroma present in a 20-year-old male on left side of his neck treated by total surgical excision of the lesion.
