This is caused by an alteration in the expression of the Na+/Cl cotransporter sensitive to thiazides, which generates a series of characteristic hydroelectrolytic alterations, but with non-specific symptoms. We present the case of a 37-year-old woman who presented with of novo convulsive syndrome in which severe and refractory hypokalemia was documented accompanied by metabolic alkalosis, hypomagnesemia and hypocalciuria, being diagnosed with Gitelman syndrome. Since potassium disorders are common in emergency departments, it is important not to overlook this syndrome in the differential diagnosis of persistent hypokalemia, even in atypical presentations.","PeriodicalId":485469,"journal":{"name":"Revista médica de Trujillo","volume":"28 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-06-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"136369825","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Methods: An observational, descriptive, and retrospective case series study was carried out evaluating medical records of patients with urinary tract infections hospitalized in a prestigious clinic in Lima between 2019 and 2021. Positive urine cultures and their antibiograms were analyzed. Results: The highest prevalence corresponded to Escherichia coli with 65% and, particularly 15%, that of Escherichia coli ESBL+, followed by Klebsiella pneumoniae ssp (14.77%), Staphylococcuss saprophyticus (5.68%), Proteus mirabilis (3.41%), Enterococcus sp. (2.27%), among others. According to the antibiograms, there was a higher prevalence of resistance for E. coli ESBL to the following antibiotics: Amoxicillin- clavulanic acid (100%), ampicillin (100%), cephalotin (100%), cefazolin (100%), cefepime (100%), cefotaxime (100%), ceftazidime (100%), ceftriaxone (100%), cefuroxime (100%), ciprofloxacin (92.31%), among others. On the other hand, E. coli ESBL+ had greater sensitivity to fosfomycin (100%), imipenem (100%), meropenem (100%), nitrofurantoin (92.31%). Conclusion: that the highest prevalence of urinary tract infection was by E.coli non-ESBL, which were highly sensitive to cephalosporins and quinolones, while for E.coli ESBL +, they were totally sensitive to fosfomycin, imipenem and meropenem.","PeriodicalId":485469,"journal":{"name":"Revista médica de Trujillo","volume":"5 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-06-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"136369822","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

SARS-CoV-2 infection is the cause of the COVID-19 pandemic. During this period, many patients who had a clinical similar to COVID-19 sought medical attention and that particular diagnosis was attributed to all of them. Given this situation, in many cases it was suspected that the patients had an infection by SARS-CoV-2, resulting in incorrect in-hospital treatment and measures. We present the case of an HIV-infected male who presented with pneumonia associated with probable COVID-19 or P. jirovecii infection. The aim of this case report is to establish the differences between SARS-CoV-2 and P. jirovecii pneumonia.","PeriodicalId":485469,"journal":{"name":"Revista médica de Trujillo","volume":"26 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-06-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"136369824","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Results: The presence of carotid atherosclerosis was found in 18.3% of the patients. The relationship between periodontitis and atherosclerosis is significant. For its part, menopause is defined as the permanent cessation of menstruation involving what was found in the respective review, two thirds of the population with Alzheimer's disease are female and of these, 60% are postmenopausal and over 60 years, since the main risk elements for the development of AD is the APOEε4 gene, located in the CNS responsible for producing astrocytes and microglia. ApoE, a protein encoded by the gene, affects the uptake and oligomerization of Aβ, making the elimination of Aβ less effective and accelerating aging, expressed in the reduction of T cells, especially in menopausal women carrying APOEε4, who are more susceptible to developing AD. Methods: the methodology used was of an applicative and descriptive level, the results were measured during the Pandemic generated by COVID-19 in the year 2020, between the months of April and June. Data collection was performed with informed consent and using a virtual questionnaire, using the Maslach Burnout Inventory Student Survey Questionnaire (MBI-SS) as a measurement instrument. Results: were analyzed with the statistical program IBM SPSS version 26, finding a High reliability level (Crombach's Alpha of 0.87). Conclusion: The results indicated that there is 9% of students with a High level of the SBA, 85% with an average level and 7% with a Low level. In relation to the dimensions of the SBA (Emotional Exhaustion, Cynicism and Academic Self-Efficacy), it was found that 15.3% of students presented a High level in High Emotional Exhaustion, 64.7% presented a Medium level and 20% a Low level. . In relation to Cynicism, 18% of students presented a high level, 49.3% a Medium level and 32.7% a Low level.
