This study analyzes how career satisfaction influences health workers’ intention to leave. The study further argues that the effect of career satisfaction on intention to leave is moderated by burnout and this moderation is dependent on gender. Drawing on a theoretical framework underpinned by the conservation of resource (COR) theory, the relationships in this study are tested with primary data from self-administered questionnaires collected from 230 health workers using multi-stage sampling techniques. SPSS with Hayes Process Macro was employed to analyze the direct relationship, moderation, and moderated-moderation mechanisms. Consistent with expectations, findings indicate that career satisfaction negatively affects health workers’ intention to leave. Also, this study extends the COR theory by establishing that high levels of burnout exacerbate the negative relationship between career satisfaction and intention to leave. Suneja, Nallaiah Manoharan","doi":"10.1080/20479700.2023.2217564","DOIUrl":"https://doi.org/10.1080/20479700.2023.2217564","url":null,"abstract":"ObjectiveTo estimate the length of stay and proportional mortality in COVID patients in a COVID-dedicated hospital.MethodsA retrospective record review was done using medical records of COVID-19 in-patients, capturing the demographic, clinical, and laboratory details of admitted COVID patients, including serial samples for RTPCR/CBNAAT for Coronavirus. These details from electronic medical records were compared and collated for patients of different comorbidities to arrive at the average length of stay and case fatality rate and time duration for patients to turn COVID-negative.ResultsPatients with Diabetes Mellitus (DM) had the highest Average Length of Stay (ALS) of 12.09 days in the hospital followed by patients with Hypertension (11.5 days). Patients without any comorbidities had ALS of 8.8 days. A combination of HTN, DM, coronary artery disease (CAD), and chronic kidney disease (CKD) had the highest ALS of 14.5 days. The average duration for patients to test negative is 16 days for patients without any comorbidities. The average duration between the first symptom and the negative test is the longest for DM (21 days) followed by HTN (19.5 days), cancer (19 days), and obesity (16 days). Among the 731 people who died in the observed time, the proportional mortality rate was highest with HTN (10.80%) followed by carcinoma (7.66%) and DM (6.56%), 32.55% had a combination of two or more comorbidities. 33.70% deceased didn't have any comorbidities.ConclusionThe COVID-19 pandemic has highlighted the importance of preparing for future outbreaks and sudden increases in cases. {"title":"Machine Learning-Based Time in Patterns for Blood Glucose Fluctuation Pattern Recognition in Type 1 Diabetes Management: Development and Validation Study.","authors":"Nicholas Berin Chan, Weizi Li, Theingi Aung, Eghosa Bazuaye, Rosa M Montero","doi":"10.2196/45450","DOIUrl":"10.2196/45450","url":null,"abstract":"<p><strong>Background: </strong>Continuous glucose monitoring (CGM) for diabetes combines noninvasive glucose biosensors, continuous monitoring, cloud computing, and analytics to connect and simulate a hospital setting in a person's home. CGM systems inspired analytics methods to measure glycemic variability (GV), but existing GV analytics methods disregard glucose trends and patterns; hence, they fail to capture entire temporal patterns and do not provide granular insights about glucose fluctuations.</p><p><strong>Objective: </strong>This study aimed to propose a machine learning-based framework for blood glucose fluctuation pattern recognition, which enables a more comprehensive representation of GV profiles that could present detailed fluctuation information, be easily understood by clinicians, and provide insights about patient groups based on time in blood fluctuation patterns.</p><p><strong>Methods: </strong>Overall, 1.5 million measurements from 126 patients in the United Kingdom with type 1 diabetes mellitus (T1DM) were collected, and prevalent blood fluctuation patterns were extracted using dynamic time warping. The patterns were further validated in 225 patients in the United States with T1DM. Hierarchical clustering was then applied on time in patterns to form 4 clusters of patients. Patient groups were compared using statistical analysis.</p><p><strong>Results: </strong>In total, 6 patterns depicting distinctive glucose levels and trends were identified and validated, based on which 4 GV profiles of patients with T1DM were found. They were significantly different in terms of glycemic statuses such as diabetes duration (P=.04), glycated hemoglobin level (P<.001), and time in range (P<.001) and thus had different management needs.</p><p><strong>Conclusions: </strong>The proposed method can analytically extract existing blood fluctuation patterns from CGM data. Thus, time in patterns can capture a rich view of patients' GV profile.
