Diagnosis and treatment planning for these cases necessitate a comprehensive assessment of multiple factors such as causative factors, esthetic concerns, arch alignment, and functionality, but also prognosis, preventive measures, and long-term follow-up. Although significant progress has been made in contemporary dentistry in terms of prevention, periodontal maintenance, and preservation of natural teeth, there has been a noticeable rise in the number of patients suffering from tooth wear. Consequently, it is essential for dental professionals to recognize premature indications of potential long-term irreversible harm to the teeth, as this enables early prevention and appropriate intervention. Tooth wear may necessitate comprehensive full-mouth restoration to rectify compromised appearance, occlusal function, and continuous loss of occlusal height, with the goal of achieving an optimal and lasting outcome. Nonetheless, financial constraints frequently act as an obstacle to the possibility of extensive reconstructive treatment, particularly necessary when the restorative process is prolonged over multiple years, potentially compromising esthetics and occlusal alignment. In this article, the present authors discuss the case of a patient who presented with fluorosis, gingival asymmetry, and malocclusion, including a lingually positioned maxillary left canine (tooth 23) and inadequate exposure of the coronal aspect of the maxillary premolars. The interdisciplinary treatment plan included accelerated orthodontics, crown lengthening, and porcelain veneers. The first step was a corticotomy, which facilitated the movement of tooth 23 into its proper position before any restorative procedures were carried out. Surgical crown lengthening was then performed to enhance the exposure of the clinical crowns of the maxillary premolars. Preferences of laypeople and different dental specialists.","authors":"Yasamin Babaee Hemmati, Mohammad Javad Shabanpour, Mehran Falahchai","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objectives: </strong>To assess the preferences of laypeople, general dental practitioners, orthodontists, prosthodontists, and oral and maxillofacial (OMF) surgeons regarding the anterior occlusal plane (AOP) position in patients with a symmetrical face and occlusal plane canting (OPC).</p><p><strong>Materials and methods: </strong>In this cross-sectional study, a standard photograph was obtained from a female patient with a flat OP and modified to create five photographs with OPC by 1-degree increments. For each of the five images, three different treatment plans were proposed, including correction of the dental midline, symmetrizing of the central incisors with midline correction, and symmetrizing of all four incisors with midline correction. In total, 275 raters assessed the attractiveness of images in an online questionnaire using a 4-point Likert scale. The data were analyzed statistically (α = 0.05).</p><p><strong>Results: </strong>The scores for images with 0, 1, and 2 degrees of OPC were not significantly different in any rater group (P > 0.05). Laypeople, unlike general dental practitioners and specialists, believed that 3 and 4 degrees of OPC were esthetically acceptable. Of the suggested treatment plans, only orthodontists gave a significantly higher esthetic score to the third treatment plan for 3 degrees of OPC compared with the untreated image (P 0.05).</p><p><strong>Conclusions: </strong>OPC of 3 degrees or higher was detectable by laypeople and specialists. This clinical report describes a digital workflow for the rehabilitation of a patient with a severely diminished lower facial third by incorporating facial scans with full-arch digitally designed occlusal rims for the vertical dimension, guided by a modified intraoral scan body (AFT Dental System; Seville, Spain). As a result, functional and esthetic parameters established through try-in of the rims can be transferred to the facial scan during the digitization process of the patient. This technique facilitates a predictable facially generated treatment plan, digital design, and manufacturing of interim prostheses in the determined vertical dimension of occlusion. In managing dental wear, additive methods offer predictable outcomes while safeguarding the longevity of restored teeth. Direct restorations are suitable for minor to moderate lesions due to their reversibility and cost-effectiveness, whereas deeper or functionally and esthetically significant defects often require indirect solutions. Minimally invasive approaches, particularly ceramic veneers, prioritize enamel preservation and aim to minimize unnecessary tissue removal. For severe anterior wear, three adhesive strategies stand out: the bilaminate approach (palatal and vestibular veneers), V-shaped veneers, and 360-degree peripheral veneers.
