In this prospective study, patients who underwent CVC, in an emergency department (ED), were divided into two groups: patients who underwent CVC using the anatomical landmark technique (n = 34) and patients who underwent CVC using a static US examination (n = 33). The procedure times, success rates, and number of catheters used were compared. A total of 67 patients who underwent CVC were included in the study. Compared with the anatomical landmark technique, the static US examination had a higher procedural success rate ( P = .001), fewer total interventions ( P = .001), and fewer postprocedural catheter dysfunctions ( P = .048). While there was no difference in the duration of the CVC between groups ( P = .222), the total time spent was longer using a static US examination ( P = .022). The static US examination was a practical, easy-to-apply method that could be used for CVC placement in an ED. Tuğ","doi":"10.1177/87564793241252164","DOIUrl":"https://doi.org/10.1177/87564793241252164","url":null,"abstract":"The aim of this study was to assess the diagnostic accuracy of lung ultrasonography (LUS) and a local clinical triage protocol complicated by varied clinical severities of COVID-19, in pregnant patients who had computed tomography (CT) and reverse transcription polymerase chain reaction (RT-PCR) results. This retrospective cohort study was conducted in a tertiary maternity unit, during the pandemic, between March 2020 and November 2021. The pregnant patients included in the study were diagnostically categorized as asymptomatic, mild, moderate, severe, and critical, upon hospital admission and their presenting disease severity. All of the patients, as well as those with mild and asymptomatic severity, LUS were evaluated accuracy based on their RT-PCR results. In total, 1531 pregnant patients underwent a primary emergency department clinical evaluation, in the pandemic clinic. The sensitivity, specificity, positive predictive value, and negative predictive values for LUS were 70.5%, 72.45%, 88.1%, and 45.8%, respectively. In this large cohort of patients, LUS was shown to be quick, safe, effective, and reproducible diagnostic tool for the screening of COVID-19 pneumonia, in this obstetric practice. A cohort of 175 uncomplicated singleton pregnancies, at 27 to 40 gestational weeks, was included in this study. The FST was measured with sonography and defined as the distance between the fetal skin and the outer table, on a cross-section of the skull, along the parietal aspect of the fetal head. Measurements were taken twice, and the average of the measurements was recorded. The estimated fetal weight (EFW) was derived from the Hadlock’s formula. Simple linear and multiple linear regression models were performed to predict the EFW, based on the FST measures. A strong positive correlation was demonstrated between FST and the EFW (r = 0.65, P < .01). The regression equation, EFW = 4.6*FST + 2.27 was derived, with a unit increase in FST yielding a 4.6 unit rise in EFW. Fetal scalp thickness and the fetal femur length (FL) accounted for about 93% (R2 = 0.93) variability in EFW, while the fetal head circumference (HC) and FST accounted for 83% (R2 = 0.83) variance in the EFW. This prospective study included pregnant women at 24 to 37 weeks of gestation referred to our institute because of TPL. Cervical length was measured at three successive time points using transvaginal sonography (upon admission, one-, and two-weeks post-admission). Data were analyzed using both parametric and non-parametric statistics. The mean age of the 126 patients included was 25.60 ± 6.48 years, with age ranging from 14 to 42 years. The mean pregnancy age, at the time of the sonogram, was 33.54 ± 2.78 weeks. The initial CL had an area under the receiver operating characteristic curve (AUROC) of 0.854. The CLs measured one and two weeks later had AUROCs of 0.868 and 0.870, respectively. Based on these data, the cut-off points of 28.5 and 27.5 mm were identified for the initial and subsequent two-week CL measurements, respectively, capable of predicting preterm delivery. If the CL measured at initial admission was >28.5 mm, the likelihood of timely delivery was 92.22%. However, if the CL measured at initial admission was <28.5 mm, the chance of premature preterm delivery increased by 2.3 times. Patients with a CL <27.5 mm in the second week were 18.75% more likely to have a preterm delivery. If the CL was >27.5 mm, the probability of timely delivery was 89.7%. In this cohort of women, serially measured CLs taken during transvaginal sonography demonstrated promise as a method for indicating the possible risk of preterm delivery. Thus, measured CL may be a useful diagnostic component for prenatal care, as well as for classifying the risk of preterm delivery. The provided case series demonstrates the unique diagnostic patient findings of a bicornuate uterus and an incompetent cervix. The patient presented over a series of multiple pregnancies that used cervical cerclage, to achieve an improved maternal-fetal outcome. In this patient, her uterine anomaly was diagnosed prior to pregnancy. During each gestation, close diagnostic attention was paid to the shape of the uterus, the location of the gestational, and the cervical length, by using transvaginal sonography. This imaging surveillance played an important diagnostic role in providing a better patient outcome for the patient’s multiple pregnancies. A total of 66 participants were recruited and consented at a medical clinic in Calabar, Nigeria, and presented from August 2019 to December 2023. The diagnostic data points recorded on this cohort were their fasting blood glucose (FBG) (mmol/L), CRA peak systolic velocity (PSV), CRA end-diastolic velocity (EDV), the resistive index (RI), and pulsatility index (PI). The data analysis was completed to provide descriptive information, as well as correlations. The statistical significance was set at P < .05. In this cohort, the women demonstrated a higher PSV and EDV, based on the vascular flow within the right CRA (Rt. PSV [20.8 ± 6.0 SD] and Rt. EDV [5.8 ±2.1 SD]), as well as the left CRA (Lt. PSV [20. 6 ± 4.4 SD] and Lt. EDV [6.2 ± 2.0 SD]). This was compared to the men’s data taken on the vascular flow within the right CRA (Rt. PSV [19.6 ± 5.3 SD] and Rt. EDV [5.2 ± 1.8 SD]), as well as their left CRA (Lt. PSV [19.2± 4.7 SD] and Lt. EDV [5.3 ± 1.3 SD]). The participants’ FBG levels did not correlate with their respective Doppler parameters. The Doppler parameters, based on the flow within the right CRA, showed moderate to weak positive correlations compared with the left CRA. The PSV recorded in the right CRA was varied across the specific participant age groups of 17 to 25 and 26 to 34 years. Potential sex and age-related variations in the Doppler velocities, recorded within the CRA, were specific to this cohort, and the sample size may have compromised the overall study’s statistical power. Furthermore, student’s prior education is typically viewed as a positive predictor of success in college. The aim of this study was to examine whether a relationship exists between employment status and prior education, compared with academic and clinical success for Diagnostic Medical Sonography (DMS) or Radiography (RT) students, in a large community college. There were 26 imaging student participants consented to this study. This convenient sample included 10 DMS and 16 RT students, enrolled in an associate degree in applied sciences program. A survey was administered that collected demographic data on the students in both programs. Data collection began at the start of each respective program and again one year later. Student academic and clinical grades were examined over a period of three college semesters. The analysis of the findings did not show a significant relationship between employment status, prior education, student academics, and clinical success among the participating DMS or RT students. The results of this study provide limited evidence that there was no significant relationship between employment status, prior education, and academic and clinical success among this cohort of DMS or RT students. These findings also suggest that students’ employment did not impede success for these DMS and RT students. In addition, this research shows that students’ prior education had no effect on their success in their DMS and RT programs.
