Phase 1: Extracted on-road test items from the previous study. Phase 2: Conducted a semi-structured interview with occupational therapists and driving instructors, and analyzed the qualitative data. Phase 3: Developed texts for the new on-road tests. Phase 4: Conducted a web-based Delphi survey. Phase 1: Extracted the large items (1006 labels) from 18 on-road tests. Phase 2: Categorized 49 items of the on-road tests into the 11 categories. Phase 3: Created new on-road test items. Phase 4: After four rounds of the web-based Delphi survey, consent was obtained for all items from more than 80% of respondents. Finally, we reached a consensus with 44 items in the driving school evaluation and 47 in the public on-road test. Method The study design was a systematic review and meta-analysis. The search was limited to the articles published in English and available online until July 2019. The odds ratio and the lower and upper confidence intervals were converted into logarithms, and then the standard error was calculated. Result The total number of participants in the meta-analysis was 581,945. Sleep disturbances are associated with physical impairment, and this odds ratio (OR) is equal to 1.33 and the confidence interval (CI) is 1.19–1.48 (P < 0.001). In prospective cohort studies, the results are equal to OR = 1.40 and CI = 1.24–1.58 (P < 0.001), and in cross-sectional OR was 1.26 and 1.08–1.47 (P = 0.004). Discussion Sleep disturbances are associated with physical impairment. Methods Twenty-one stroke patients underwent the SPDR. Then, semi-structured interviews were conducted before and after the SPDR. The data of the interviews were analyzed through qualitative analysis based on content analysis. Results As a result of the qualitative analysis, it was found that many patients became aware of driving risks, of which they had not previously been aware, through the simulation; these were categorized as improved awareness and confusion. Methods Static and dynamic balance was assessed in 20 males before and after an upper extremity (UE) fatigue protocol. Fatigue was achieved using an arm crank ergometer protocol, which consisted of 3-minute intervals of UE exercise, increasing by 25 watts in a range of 70-80 rpms, until exhaustion. Results The UE fatigue protocol did not negatively impact dynamic or static balance levels in either young or old populations. In fact, UE fatigue was shown to have a positive influence on dynamic balance in the posterolateral direction in the young population. Conclusion Although old population demonstrated preexisting balance impairments, UE exercise does not appear to further impair balance. Objective to assess the effects of “Abdominal Drawing-in Maneuver” and “Diaphragmatic Breathing Maneuver” in improving the stability of trunk in stroke patients. Methods A quasi-experimental study including 36 patients with confirmed diagnosis of the first incident of stroke was conducted. Patients were assigned randomly in two groups ADIM and DBM. Outcome measurements were evaluated using the “Trunk Impairment Scale”. Results after 4 weeks of training; “Trunk Impairment Scale” score improved significantly. Between groups comparison shows that trunk stability significantly improved in the group DBM as compared those in the group ADIM. {"title":"Validity of Instrumented 360° Turn Test in Older Adults with Cognitive Impairment.","authors":"Ellen L McGough, Molly Gries, Linda Teri, Valerie E Kelly","doi":"10.1080/02703181.2019.1710319","DOIUrl":"10.1080/02703181.2019.1710319","url":null,"abstract":"<p><strong>Aims: </strong>To examine concurrent and construct validity of inertial sensor 360°turn measures in relation with motion capture and mobility assessments in cognitively impaired older adults.</p><p><strong>Methods: </strong>Data was collected in 31 participants, mean age 85.2 (<i>SD</i> 5.2), during clockwise (CW) and counter clockwise (CCW) 360° turns using (1) APDM body-worn inertial sensors and (2) Qualisys 8-camera laboratory-based motion capture.</p><p><strong>Results: </strong>Absolute agreement between inertial sensor and motion capture measures was excellent for turn duration and turn peak velocity (ICC = 0.96-0.98). Strong to moderate correlations were present between inertial sensor turn measures and performance on the Timed Up and Go, Short Physical Performance Battery and 90-s Balance Test. Quantitative data was gathered using the PROMIS Physical Function Short Form-20. Qualitative data was obtained from in-depth semi-structured interviews on participants' self-management strategies.</p><p><strong>Results: </strong>The average T-score (35.96) was below the national average. Methods This study implemented a single-subject, reversal design with additional pre/post- measures. Seven participants with chronic hemiparesis and aphasia completed the study. Participants received an individualized, repetitive task practice upper extremity home program and a preferred music playlist to listen to during intervention phases. The primary outcome was adherence to practice as measured by self-reported session duration and number of repetitions recorded in an aphasia-friendly logbook. All logbook data were analyzed visually for within phase and between phase differences. Results Visual analysis revealed no consistent effect of preferred music listening on adherence to home practice for either variable. C-statistic analyses confirmed these findings. Conclusions In this sample, preferred music listening during practice did not modify adherence variables. Future studies should explore other strategies to increase home program adherence. Methods: Nine participants enrolled through convenience sampling were randomly assigned to a wear group: continuous, sleeping, or waking. Descriptive statistics were utilized to describe participant demographics and examine the data for changes across the six to ten weeks of study. Results: The study successfully obtained feasibility data regarding participant recruitment rate, demographics, methods as well as orthotic wear comfort, functionality, adverse effects and wearing compliance. The rate of recruitment was 1.9 participants per month. Conclusions: The results suggest it is feasible to perform a RCT, though recruitment was challenging. Methods 1598 participants (71.53 ± 4.99 y, 64.1% women) were recruited. The body mass index (BMI), muscle strength, health status, and all TUG phases (sit-to-stand, gait-to-go, turning, gait-return, and stand-to-sit) were evaluated. 5-TUG performance-group scores are reported for the <20th; 20–40th, 40–60th, 60–80th, 80–100th percentiles, as there is no health standard cutoff for Portuguese elderly. The Pearson’s correlations were assessed between variables (p < 0.05). Results The best TUG performers (<20th) presented better results than the other groups for all tests, with the exception of the Turning phase. Both gait, sit-to-stand and stand-to-sit were inversely correlated with health status and muscle strength, and positively correlated with BMI.
